Abstract
짧은 수면 시간과 낮은 수면의 질은 높은 비만 위험과 관련이 있는 것으로 보고되고 있다. 그러나 수면과 비만 사이의 관련성에 대해 식사요인 특히 전체적인 식사의 질을 함께 살펴본 연구는 드물다. 본 연구는 수면과 비만 사이의 관련성에 있어 식사의 질이 confounding factors 중 하나로서 관련되어 있는지 혹은 effect modifier로서 관련되어 있는지를 확인하고자 하였다. 본 연구의 대상자는 2015년 4월부터 2016년 8월까지 서울대학교 보라매병원 건강검진센터에 건강검진을 위해 방문한 19-64세 성인 남녀로서 이 중 연구 참여에 대한 동의를 얻은 1165명 (남자 737명, 여자 428명)을 대상으로 연구를 진행하였다. 수면시간은 지난 한 달간 평균 수면시간에 대한 설문자료를 이용하였으며, 수면의 질은 Pittsburgh Sleep Quality Index (PSQI)를 이용하여 평가하였다. 식사의 질 평가는 Recommended Food Score (RFS)를 이용하였다. 수면상태와 비만과의 관계에 있어 식사의 질이 confounding factor으로서 관련되어 있는지를 알아보기 위해 남자와 여자에서 수면상태와 비만과의 관계에 대한 multiple linear regression 분석과 multiple logistic regression 분석에서 RFS 변수를 confounding factor으로 포함시킨 결과와 포함시키지 않은 결과를 비교하였다. 그 결과 수면상태와 비만과의 관련성은 RFS 변수의 모델 포함 여부에 따라 차이가 나지 않는 것으로 나타났으며, 이에 RFS는 confounding factor으로 관련되어 있지 않음을 확인하였다. 모델에 함께 포함시킨 confounding factors은 나이, 결혼상태, 흡연, 운동, 그리고 폐경 여부(여자의 경우)였다. 수면상태와 비만과의 관계에 있어 식사의 질이 effect modifier 로서 관련되어 있는지를 알아보기 위해 남자와 여자에서 수면상태와 비만과의 관계에 대한 multiple linear regression 분석과 multiple logistic regression 분석 시, RFS 중위값(남자 23점, 여자 21점)을 기준으로 두 그룹으로 나누어 각각 분석하였다. 수면의 질이 낮은 (PSQI 5점 이상) 여자는 수면의 질이 높은 여자와 비교할 때 비만에 대한 위험이 높은 것으로 나타났으며(OR = 2.198, 95% CI: 1.027-4.704), 이러한 관련성은 RFS가 중위값 보다 높은 그룹에서만 나타났다. 즉, 여성에서는 RFS가 수면의 질과 비만 사이의 관련성에 있어 effect modifier로서 작용함을 확인하였다. 한편, 남성에서는 이러한 결과를 관찰할 수 없었다. 결론적으로 여성에서의 낮은 수면의 질과 높은 비만 위험 사이의 관련성에 있어 식사의 질이 effect modifier로서 작용할 수 있다는 가능성을 확인하였다. 본 연구결과는 여성에서 수면의 질이 낮은 것과 관련되어 있는 비만에 대해 전반적으로 질이 높은 식사가 이로운 역할을 할 수도 있다는 가능성을 제시한다. ;Short sleep duration and poor sleep quality are associated with a higher risk of obesity. However, few studies have examined dietary factor, especially overall diet quality, in this association. The purpose of this study was to investigate whether diet quality acts as one of the confounding factors or an effect modifier in the association between sleep status and obesity. For a cross-sectional analysis, we used data and samples of baseline information from a prospective cohort study. The study subjects consisted of 1,165 adults (737 men and 428 women) aged 19–64 years, undergoing an annual health examination at Seoul National University Boramae Medical Center (Seoul, Korea) between April 2015 and August 2016. Sleep duration was measured by average hours of sleep per night, and sleep quality was assessed using the Pittsburgh Sleep Quality Index (PSQI). Diet quality was assessed by the Recommended Food Score (RFS). To find out whether diet quality is related as a confounding factor in the association between sleep status and obesity, in multiple linear regression analysis and multiple logistic regression analysis of the association between sleep status and obesity, the result of including the RFS variable as a confounding factor and the result of not including it were compared. As a result, it was found that the association between sleep status and obesity was not different depending on whether the RFS variable was included in the model, and thus it was confirmed that RFS was not related as a confounding factor. Other confounding factors included in the model were age, marital status, smoking, physical activity, and menopause status (women only). To find out whether diet quality is related as an effect modifier in the association between sleep status and obesity, multiple linear regression analysis and multiple logistic regression analysis of the association between sleep status and obesity was performed by dividing into two groups based on the median value (23 in men; 21 in women) of RFS in men and women respectively. Women with poor sleep quality (PSQI score of 5 or higher) were found to have a higher risk of obesity compared to women with high sleep quality (OR = 2.198, 95% CI: 1.027-4.704); this association was found only in the group with RFS higher than the median value. Therefore, in women, it was confirmed that RFS acts as an effect modifier in the association between sleep quality and obesity. On the other hand, these results could not be observed in men. In conclusion, it was confirmed that diet quality may act as an effect modifier in the association between poor sleep quality and high risk of obesity in women. This study suggests the possibility that high diet quality may play a beneficial role in obesity, which is associated with poor sleep quality in adult women.
