가족부양자의 부양행동, 부양관계망과 기능취약노인의 삶의 질의 관계

Abstract

The main purpose of this study was to examine the relationship with a caregiver's caregiving network and caregiving service for functionally impaired elderly and also examine closely how caregiving service affects a quality of the aged's life. This study is based on a sample of the data which were collected for the project titled with 'Caregiving Index' of Choi et al(1999). The subjects were composed of 251 functionally impaired elderly and 251 their caregivers. The major findings of this study are summarized as follows ; First, the study shows that sex, educational background and income in socio-domographic characteristics of caregivers are related to caregiver's caregiver network. and caregiver's sex, age, educational back ground, income and residing with the aged are related to caregiving service. Second, the caregiving services of caregivers significantly affect the aged quality of life. caregiver's emotional service, functional service, negative service is related to ego integrity of functional impairment of the elderly. and caregiver's functional service, negative service is related to despair of functional impairment of the elderly. but caregiver's service is not affect the life satisfaction of elderly. Third, main helper's intimacy degree in caregiving network of caregivers is related to caregiver's emotional service. main helper's intimacy degree, main helper's number is related to caregiver's functional service. and main helper's satisfaction degree is related to caregiver's negative service. Fourth, An interaction with the functional impairment of the aged and the main helper's number , it reduces a negative effects of caregiver's negative service . And an interaction with the functional impairment of the aged and the main helper's satisfaction, it reduces a negative effects of caregiver's negative service. this result demonstrate that caregiving service is related to a quality of caregiver's life. And it also shows that a negative service of caregiver's is affected by an interaction between caregiving network and functionally impaired elderly.;본 연구는 기능취약노인을 부양하는 부양자의 부양관계망과 부양행동의 관계를 살펴보고 아울러 부양자의 부양행동이 노인의 삶의 질과 어떻게 관련되는지를 규명하는데 그 목적이 있다. 본 연구의 자료는 최혜경 등(1998)의 '노인부양지표 개발'을 위한 연구의 2차년도 자료 중 정상노인이 아닌 기능취약 노인과 그 부양자를 대상으로 한 자료를 사용하였다. 분석대상은 부양자 251명, 기능취약 노인 251명으로, 자료의 분석은 SPSS 10.0 프로그램을 이용하여 빈도, 백분율, 평균, 신뢰도 분석, 상관관계분석, 요인분석, t검증, 회귀분석을 실시하였다. 본 연구의 주요 결과를 요약하면 다음과 같다. 첫째, 부양자의 사회인구학적 특성 중 성별, 교육연수, 소득은 부양자의 부양관계망에 유의한 영향을 미치고, 부양자의 성별, 연령, 교육연수, 소득, 노인과의 동거여부는 부양행동에 유의한 영향을 미친다. 둘째, 부양자의 정서적 부양행동, 도구적 부양행동, 부정적 부양행동은 노인의 자아통합감에 유의한 영향을 미치고, 부양자의 도구적 부양행동, 부정적 부양행동은 노인의 절망감에 유의한 영향을 미친다. 반면, 노인의 생활만족감은 부양자의 부양행동에 영향을 받지 않는다. 셋째, 부양자의 부양관계망 중 주요도움제공자와의 친밀도는 부양자의 정서적 부양행동에 영향을 미치고, 주요도움제공자의 크기와 주요도움제공자와의 친밀도는 부양자의 도구적 부양행동에 영향을 미친다. 또한, 부양자의 주요도움제공자와의 만족도는 부정적 부양행동에 영향을 미치는 것으로 나타났다. 넷째, 노인의 기능취약과 부양자의 주요도움제공자의 크기가 상호작용하여 노인의 기능취약이 부양자의 부정적 부양행동에 미치는 부정적인 영향을 감소시키며, 노인의 기능취약과 부양자의 주요도움제공자간의 만족도가 상호작용하여 기능취약이 부양자의 부정적 부양행동에 미치는 영향을 감소시키는 것으로 나타났다. 즉, 노인의 기능취약과 부양자의 부양관계망간의 상호작용은 부양자의 부정적 부양행동에 영향을 미치는 것을 알 수 있다. 그러나, 긍정적 부양행동에는 영향을 미치지 않았다. 본 연구결과를 통해 볼 때 기능취약노인을 부양하는 부양자의 부양행동은 노인의 삶의 질과 유의한 관계를 맺고 있음을 알 수 있었다. 즉, 노인의 삶의 질, 특히 취약노인의 삶의 질이 가족을 포함한 부양자들의 부양에 의해 영향을 받는다는 점을 알 수 있다. 또한, 이러한 부양자의 부양행동 중 부정적 부양행동은 노인의 기능취약과 부양관계망간의 상호작용에 의해 영향을 받는 것을 알 수 있었는데, 노인의 기능취약이 부양자의 부정적 부양행동에 미치는 영향을 부양자의 부양관계망이 중재하는 것을 알 수 있었다.
