Abstract
많은 임상적인 자료와 역학적인 자료에서 영양결핍과 감염성 질병의 인과적 상관관계를 지적하고 있다. 영양결핍과 감염성 질병 모두 아동기의 이환율과 사망율을 높이는 주된 원인이 되고 있다. 특히 사회 경제적으로 혜택을 받지 못하는 계층의 경우에는 더욱더 주된 원인이 되고 있다. Protein-calorie malnutrition (PCM)은 많은 면역기능을 감소시킨다. PCM인 사람은 지연성 피부반응에 손상이 오고, 순환하는 T-lymphocyte의 수가 감소를 하는 반면 B-Iymphocyte의 수나 immunoglobulin 수준, 항체반응 등은 대체로 정상수준이다. 그러나, 혈청내 보체성분은 감소하는데 생체 실험을 통한 보체 C_(3)의 소모가 그것을 입증해 주고 있다. 본질적으로 감염은 영양소의 실제적인 손실을 가져오거나 영양소의 소모를 급증시켜 영양소를 고갈시킴으로써 영양소의 손실을 야기하고 면역능력의 감소를 초래한다. 본 연구에서는 만 3세 미만 52명의 어린이를 대상으로 영양상태와 면역반응의 상관관계를 보고자 했다. 신체계측의 기준은 Weight for height for age를 근거로 했다. 그 결과 19명의 어린이가 normal group이었고, 17명의 어린이가 moderate PCM group, 16명의 어린이가 severe PCM group으로 각각 분류되었다. 본 실험 은 Hemoglobin, hematocrit, serum albumin, immunoglobulin G, complement 3, WBC 수준을 측정하였다. Hemoglobin, hematocrit, serum albumin 과 C3수준은 moderate와 severe PCM group에서 각각 감소하여 나타났다. 그러나 Immuno-globulin G 수준이나 WBC는 영양상태에 영향을 받지 않았다. 영양불량시 보체기능이 감소하게 되며, 영양불량인 사람에게서 질병에 대한 감수성이 증가함을 알 수 있었다.;Clinical and epidemiologic data point to a causal interrelationship between nutritional deficiency and infectious illness. 30th are major contributors to childhood morbidity and mortality, particularly in underprivileged population groups. Protein-calorie malnutrition depress a variety of immunity functions. Individuals with protein-calorie malnutrition show consistent impairment of cutaneous delayed hypersensitivity and a reduced number of circulating T lymphocytes. B lymphocytes, immunoglobulin levels and antibody responses are generally normal. Serum complement components are low and there is evidence of in vivo consumption of complement 3. Infection per se results in nutrient losses, either actual or by sequestrations and produces immunosuppression The interrelationship between nutritional status and immune response was studied in 52 children aged under 3 years The anthropometric categories used were based on weight for height for age. Results indicated that 19 children were normal group, 17 children were classified as moderate PCM group, and 16 children as severe PCM group. The following determinations were made; Hemoglobin, hematocrit, serum albumin, immunoglobulin G, complement 3 and WBC levels. Results indicated that levels of Hb, Hct, serum albumin and C3 concentration were decreased in moderate and severe PCM group, respectively. However, levels of Ig G and WBC were not affected by the nutritional status. It is suggested that reduced complement function in malnutrition and that increased susceptibility to infection in undernourished individuals.
