리듬 큐 기반 타악기 연주가 파킨슨병 환자의 상지 운동수행력 및 주의조절 변화에 미치는 영향

Abstract

The purpose of the study is to evaluate the effects of upper extremity motor control and attention control by drum playing intervention with rhythmic cueing in Parkinson's disease. The participant of the study were recruited through Parkinson's Disease Community for about a month. The participants were random assigned into experimental groups and control groups and two groups secured homogeneity though non-parametric tests. The mean age of the participants was 62.16 years and the mean duration of disease was 5.25 years. The drum playing intervention with rhythmic cueing was conducted for about 4 weeks in the experimental group, and the group continued for 50 minutes per day, 3 days per week for 4 weeks. The results showed that the experimental group participating in the drum playing intervention with rhythmic cueing significantly increased the duration of entrainment compared to the control group. Also, the time taken for the entrainment reaction immediately after the tempo change, the experimental group was significantly decreased. In the case of attention control, the experimental group were significantly reduced the execution time and the number of errors in the Stroop Test-Color Reading condition compared with the control group. This means that the drum playing intervention with rhythmic cueing can be effectively applied to the duration of motor and immediate upper extremity motor control in Parkinson's disease patients. Besides, this results offer the possibility of improving attention control for Parkinson's disease patients though the drum playing intervention with rhythmic cueing. These findings support the improvement in the effect on upper extremity motor control and attention control in Parkinson's disease. This study suggests that the drum playing intervention with rhythmic cueing should provide potential new approaches on upper extremity motor control and attention control for Parkinson's disease in rehabilitation training. ;본 연구는 파킨슨병 환자를 대상으로 리듬 큐 기반 타악기 연주 프로그램을 시행하고 상지 운동수행력과 주의조절에 미치는 영향을 알아보고자 하였다. 연구의 대상자는 (사)대한파킨슨병협회 홈페이지 및 파킨슨병 환우회 커뮤니티를 통해 약 한달 간 모집되었으며, 총 12명이 최종 선정되었다. 대상자들은 각각 실험집단 6명, 통제집단 6명으로 무작위 배정되었으며, 대상자들의 평균연령은 62.16세이며, 유병기간은 평균 5.25년으로 확인되었다. 본 연구는 실험집단을 대상으로 약 4주간 리듬 큐 기반 타악기 연주 프로그램을 시행하였으며, 사전·사후검사를 포함한 총 12회기, 회당 50분의 그룹 형태로 진행하였다. 이후 통제집단과 사전·사후 검사의 변화 폭을 비교하여 연주 프로그램이 파킨슨병 환자의 상지 운동수행력 및 주의조절 변화에 미치는 영향을 비교·분석하였다. 결과값은 양손전자드럼 연주과제와 수지기능 평가를 통한 운동수행력과 주의집중력 평가과제를 통한 주의조절로 나누어 기술하였으며, 결과는 맨 휘트니-U 검정을 통해 도출하였다. 결과적으로 운동수행력의 경우 느린 속도의 리듬 큐가 제시된 상태에서 양손으로 드럼을 연주했을 때 리듬 큐와 운동수행 간 동조화 지속 시간이 집단 간 유의한 차이를 나타냈으며, 빠른 속도에서 느린 속도로 리듬 큐가 변화되는 상태에서 양손으로 드럼을 연주했을 때 속도의 변화 직후 리듬 큐와 운동수행 간 동조화 반응이 일어나기까지 걸린 시간에서 집단 간 유의한 차이를 확인할 수 있었다. 주의조절력의 경우 주의집중과제 중 Stroop Test-Color Reading 조건에서 집단 간 과제수행시간의 유의미한 차이를 확인할 수 있었다. 이는 리듬 큐 기반 타악기 연주 프로그램이 파킨슨병 환자의 상지 운동수행의 지속과 즉각적인 운동조절에 효과적으로 적용될 수 있다는 것을 의미하며, 인지적 측면에서도 주의조절을 개선시킬 수 있다는 가능성을 제시한다. 즉, 파킨슨병 환자를 대상으로 시행한 리듬 큐 기반 타악기 연주 프로그램은 상지 운동수행력 및 주의조절을 향상시킬 수 있는 효과적인 중재 방안이 될 수 있음을 시사하는 바이다.
