dc.description.abstract오메가 3와 오메가 6 지방산 계열의 장쇄다중불포화지방산(long chain polyunsaturated fatty acid, LCPUFA)은 태아발생 초기부터 성장 및 발달에 영향을 준다. 특히 docosahexaenoic acid (DHA) 나 arachidonic acid (AA) 가 태아성장 및 발달, 출생 후 인지기능에까지 영향을 주기 때문에 다양한 선행 연구들을 통해 매우 중요한 영양소로 알려져 있다. 이에 본 연구는 모자환경보건(MOCEH) 연구를 통해 임신부의 지방산 섭취량과 6개월 때의 체중, 신장, 머리둘레, 인지발달과의 관련성을 알아보기 위하여 수행되었다. 본 연구는 960명을 대상으로 진행되었으며, 임신부의 식이 섭취 조사는 24시간 회상법을 이용하여 실시되었다. 6 개월 때 영유아의 식품 섭취는 모유와 이유식을 합쳐 계산되었다. 모유 섭취량은 한 번 수유 시의 양으로 추정되었고, 영유아의 식이 섭취 조사는 24 시간 회상법을 이용하여 실시되었다. 6개월 때의 인지발달은 베일리 점수 (BSID-Ⅱ)를 이용하여 조사되었다. 이 연구에서 6개월의 인지 기능 점수는 임신부의 오메가 6 / 오메가 3지방산 (MDI, β=-0.1819, p=0.0177; PDI, β=-0.2146, p=0.0221)과 리놀레산 (linoleic acid, LA) / 알파-리놀렌산 (alpha-linolenic acid, ALA) (MDI, β=-0.1788, p=0.0141; PDI, β=-0.2129, p=0.0168)의 섭취와 각각 음의 관련성을 보였다. 임신부들의 오메가 6 / 오메가 3 지방산 섭취 비율과 LA/ALA 섭취 비율을 4분위수로 나누고, 6개월의 인지발달 점수는 85점을 기준으로 나누어 인지발달의 지연 위험성을 알아보았다. 그 결과 LA/ALA 섭취가 가장 많은 군이 가장 적은 군에 비해 인지발달 지연의 위험성이 높게 나타났다. [OR=2.595; 95% CI=1.178-5.716, p-value=0.0228]. 이러한 결과들을 통해 본 연구에서는 한국 임신부의 지방산 섭취, 특히 LA/ALA 비율은 영유아의 인지발달과 관련성이 있음을 알 수 있다.;The aim of this study is to identify the relationship between maternal dietary fatty acids intake, and motor and cognitive development of infants at age 6 months. We used data from 960 participants in the Mothers and Children's Environmental Health (MOCEH) study, which is a multi-center prospective cohort study. Dietary intake of pregnant women was assessed by a 24-hours recall method. Food consumption of infants at 6 months was estimated on volume of breast milk and weaning foods. The duration of each feed was used to estimate the likely volume of milk. Dietary intake of infants at 6 months was also assessed by a 24-hours recall method. Cognitive and motor development of infants was assessed at 6 months of age through the Korean Bayley Scales of Infant Development Ⅱ (BSID-Ⅱ) including mental developmental index (MDI) and psychomotor developmental index (PDI). The ratio of n-6 to n-3 fatty acid (n-6/n-3 PUFAs) and linoleic acid (LA) to alpha-linolenic acid (ALA) (LA/ALA) were 9.7 ± 6.3, 11.12 ± 6.9, respectively. A multiple regression analysis after adjusting for covariates showed that n-6/n-3 PUFAs was negatively associated with the MDI (β=-0.1819, P=0.0177) and PDI (β=-0.2146, P=0.0221) at 6 months. These inverse associations also observed with LA/ALA (MDI, β=-0.1788, P=0.0141; PDI, β=-0.2129, P=0.0168). A multiple logistic regression analysis with the covariates showed that the odds ratio of delayed performance on psychological tasks (score below 85) having the highest LA/ALA in women was significantly higher compared with the control group (1st quartile vs. 2nd quartile; OR=2.976; 95% CI=1.380-6.421, 1st quartile vs. 3rd quartile; OR=3.182; 95% CI=1.428-7.091, and 1st quartile vs. 4th quartile; OR=2.595; 95% CI=1.178-5.716; P=0.0228). In conclusion, the ratio of both maternal dietary n-6 to n-3 PUFA and LA to ALA from well-balanced diet may influence infant's brain development. Further researches are needed to explore the well-balanced ratio responsible for the beneficial effects on psychomotor development of infants.-
