여성암 환자를 위한 지지적 노래심리치료(S-SPT) 프로그램 개발

Abstract

본 연구는 성인 여성암 환자의 심리·정서적 지원을 위한 노래 심리치료 프로그램을 개발하는데 목적이 있다. 본 프로그램은 여성암 환자의 다양한 심리·정서적 문제에 대한 내·외부의 대처자원을 탐색 및 재확인하고 격려하는 지지적 수준(supportive level)의 프로그램으로 개발하였으며, 지지적 노래심리치료(Supportive Song Psychotherapy, 이하 S-SPT) 프로그램으로 명명하였다. 프로그램에는 여성암 환자의 각 개인 간 신체·심리·정서적 수준의 편차에 따른 고려와 치료적 목적에 초점화된 개입을 위해 의사결정 트리(decision tree) 진행방식을 접목시켰다. 각 세부 활동은 환자의 에너지 수준, 수용 태도, 참여 의지 정도에 따라 노래심리치료 중재형태와 음악심리치료 기법을 선별적으로 적용하여 구성하였다. 개발된 프로그램은 음악치료 전문가 2인과 의료현장 전문가 2인, 음악치료 전공의 현직 간호사 2인, 총 6인에게 검증을 받아 수정·보완을 하였다. 이후 여성암 환자의 고려점, 노래심리치료 기법, 지지적 기능으로의 음악이 적용된 총 9개의 세부 프로그램으로 최종 프로그램을 제시하였다. 본 연구는 조기 완화의료 개입의 맥락에서 치료 및 생존단계에 있는 여성암 환자의 심리·정서적 지지를 위한 음악심리치료 프로그램의 실제를 제시하고자 하였다. 또한 국내 여성암 환자의 특성과 대처방식이 고려된 노래 심리치료 프로그램을 제안하였으며, 의사결정 트리 방식의 활동 진행구조로 임상 적용 가능성을 높인 프로그램 개발이라는 점에 본 연구의 의의가 있다.;The purpose of this study was to develop a song psychotherapy program for women cancer patients to provide psychological and emotional support. The program aims to explore and identify internal and external resources to cope with various psychological and emotional problems of women cancer patients on a supportive level. The decision tree process method was applied to set therapy goals and provide appropriate music activities considering different physical, psychological, and emotional levels of women cancer patients. A song psychotherapy and music psychotherapy intervention was provided according to patients’ energy level, attitude of acceptance, and participation motivation. The program was advised and approved 2 Music Therapy specialists, 2 medical experts, and 2 nurses with Music Therapy degree. As a result, 9 music intervention programs using song psychotherapy techniques and supportive elements of music were developed for women cancer patients were developed and named the Supportive Song Psychotherapy (S-SPT). The study aimed to presents a practical music psychotherapy program providing psychological and emotinal support for women cancer patients in context of early intervention in palliative care, Additionally, the study propose a song psychotherapy program which took specific features and coping skills of women cancer patients’ into consideration. Also the use of the decision tree method enhances the clinical applicability.
