중증 치매환자의 행동심리증상 완화를 위한 다감각 자극 중심 음악활동

Abstract

본 연구의 목적은 다감각 자극 중심 음악활동이 중증 치매환자의 행동심리증상 완화에 어떠한 영향을 미치는지 알아보는 것이다. 연구 대상은 경기도 소재 A 요양원에 거주하는 중증 치매환자로 총 22명을 대상군으로 모집하여 법정 대리인으로부터 연구 참여의 동의를 받았다. 실험군과 대조군을 각각 11명으로 나누어 실험군에게 매일 10-15분간의 다감각 자극 중심 음악활동으로 구성된 음악치료 세션을 총 10회 시행하였으며 대조군에게는 동일한 시간과 기간 동안 녹음 음악을 들려주었다. 중재 효과 확인을 위해 신경정신행동검사 간편형(NPI-Q), 문제행동, 정서 평가 척도를 사전사후에 측정하였고, 실험군은 매 회기 세션을 녹화하여 음악 활동 내의 반응을 분석하였다. 다감각 자극 중심 음악활동은 진동촉각 및 시청각자극을 제공할 수 있는 음악 활동으로 구성하였다. 연구 결과 실험군의 NPI-Q, 문제행동 평가에서 통계적으로 유의미한 감소가 나타났고(p < .01), 정서 평가에서 통계적으로 유의미하게 향상되었으며(p < .01) 관찰 분석 결과 세션의 회기가 진행됨에 따라 정서의 긍정적인 변화와 반응 수준의 향상, 자발적인 움직임의 증가가 관찰되었다. 이를 통하여 다감각 자극 중심 음악활동이 중증 치매환자의 행동심리증상 완화에 효과적인 중재 도구임이 입증되었다. 본 연구 결과는 일상생활과 중재 프로그램 참여에 제한이 많은 중증 치매환자도 다감각 자극 중심 음악활동을 통해 행동심리증상의 완화와 문제행동의 감소, 긍정적 정서의 향상 및 자발적인 참여 증가에 긍정적인 영향을 유도할 수 있음을 시사한다.;The purpose of this study is to evaluate the effects of music therapy that apply multisensory stimulations on Behavioral and Psychological Symptoms of Dementia(BPSD). Twenty two elderly people with severe dementia residing in a sanatorium located in Gyeonggi Province participated in this study and with informed consents obtained from the legal representatives. Participants were randomly assigned into two groups; an experimental group and a control group, with 11 individuals per group. Sessions were individually conducted from 10 to 15 minutes for 10 consecutive days. The experimental group received music therapy interventions stimulating auditory, visual and tactile senses as an independent variable, and the control group simply listened to recordings of old pop songs. To confirm the effect of musical intervention, Neuropsychiatric Inventory Questionnaire(NPI-Q), Dementia Problematic behavior(DPB) assessment tool and Apparent Emotion Rating(AER) scale were administered at each pre and post sessions, and also for qualitative assessment of the experimental group, each session was filmed to be analyzed. The musical intervention using multisensory stimuli for the experimental group consisted of diverse forms of musical instruments that can provide percussive vibration were employed to give auditory, visual and tactile stimulations. In results, there was significant reduction in NPI-Q and DPB(p < .01), and a significant increase in AER(p < .01) of the experimental group after the music therapy sessions. Moreover, the analysis of films recorded during sessions of the experimental group showed that as the music therapy session proceeded, more active and voluntary movements of patients such as reaching their arms to touch and to make a sound of musical instruments were observed. These results prove that music therapy with multisensory stimulations is efficacious in mitigating BPSD of severe dementia patients. In conclusion, this research implies that music therapy with multisensory stimulations is positively effective intervention for people with severe dementia, in assuaging BPSD, decreasing behavioral disturbance, evoking positive emotions, and attracting voluntary participation.
