음악치료가 정신질환자의 우울감과 스트레스에 미치는 영향

Abstract

급격한 과학문명의 발달로 현대 산업화 사회는 사회구조의 변화와 함께 현대인의 정신적 갈등을 가져와 이에 따른 심각한 인간소외 현상을 가져왔고 이러한 변화 속에서 정신질환에 이완된 환자 수는 점점 증가 하게 되었다. 그러나 우리나라의 정신질환자에 대한 치료는 아직까지 약물치료에 의존하고 있는 실정이다. 정신질환은 그 원인과 증상에 있어서 매우 복합적이기 때문에 다각적인 각도에서의 치료적 접근이 요구된다 하겠다. 따라서 본 연구에서는 이러한 치료적 요구를 충족시키기 위하여 미국을 중심으로 발전해 현재 하나의 전문적 치료 영역으로서 그 역할을 수행하고 있는 음악 치료를 정신질환자에게 적용해 보고자 한다. 본 연구는 정신질환자에게서 보편적으로 나타나는 우울감과 스트레스 감소에 음악치료가 미치는 영향에 대해 알아보고자 한다. 본 연구는 서울시내 1개 대학 정신과 병동에 정신질환으로 진단되어 입원한 환잔 8명을 대상으로 사례연구 하였다. 5주에 결처 1주일에 2회씩 총 10회의 음악치료를 시행하였다. 본 연구에서 사용한 조사도구로는 우울감의 변화를 알아보기 위하여 우울평정 척도(BDI : Beck Depression Inventory)와, 스트레스 지각척도(GARS : Global Assessment of Recent Stress)를 사용하였다. 이 조사 도구를 사용하여 음악치료의 실시전과 후에 검사하였으며, 수집된 자료는 SAS(Statictical Analysis System) 통계프로그램을 통하여 Wilcoxon 부호검정과 부호순위검정을 이용하여 분석하였다. 본 연구의 주요 연구 결과는 다음과 같다. 1. 우울척도 점수에 대한 Wilcoxon text의 결과는 음악치료 전에 비해 음악치료 후에 유의수준 p<.05의 수준에서 유의미한 결과를 얻어 음악치료가 정신질환자의 우울감 감소에 효과적이라는 결론을 얻었다. 2. 스트레스 척도에 대한 Wilcoxon test의 결과는 음악치료 전에 비해 음악치료 후에 차이가 없게 나타나 유의미하지 못한 결론을 얻었다. 이상해서 얻은 결론을 바탕으로 향후 정신질환자의 우울감과 스트레스에 대한 음악치료의 적용에 있어서 보다 효과적인 기법의 개발 및 우리나라의 문화와 정서에 적합한 음악의 연구, 지속적인 음악치료적 접근에 대한 프로그램 개발을 위한 노력이 필요하며, 우울감과 스트레스의 증상을 중심으로 한 음악치료의 적용에 대한 후속연구가 이루어져야 할 것을 제언한다.;As science civilization has abruptly developed, modern industrial society has brought modern men's mental conflicts and serious human alienation, and the number of mental patients has gradually grown in this change. However, treatment of mental patients in Korea still depends mostly on medicines. Mental disease is very complex in its reason and symptoms, and various approached for treatment are needed. Accordingly, to fulfill this need, this study attempts to apply music therapy, developed from America an now performing its role as one of specialized treatment fields, to mental patients. This study examines th influence of music therapy on depression and stress, university occurring to mental patients. This paper took a case study with the subject of 8 mental patients hospitalized in the psychiatry ward of a university hospital in Seoul. This music therapy was practiced twice a week for five weeks(total ten times). As a research tool for this study, BDI(Beck Kepression Inventory) and GARS(Global assessment of Recent Stress)were used. Using these tools, I inspected state before and after music therapy, and analyzed them through SAS(Statistical Analysis System), applying Wilcoxon's mark exam and mark order exam. The following are the results of this study. 1. The results of Wilcoxon's test on depression standard score were meaningful at p.05 after music therapy compared to before music therapy, and it means music therapy is effective in reducing mental patients' depression. 2. The results of Wilcoxon's test on stress standard was now meaningful, yielding no big difference between before and after music therapy. Based on the results written as above, we should make efforts to develop more effective skills in applying music therapy to mental patients' depression and stress, and develop music appropriate for Korean emotions and programs for continuous music therapy, and I suggest there should be more studies on the application of music therapy based on symptoms other than depression an stress.
