Substantial evidence is accumulating that large-scale anti-pandemic policies related to COVID-19 may disrupt eating habits. Adequate nutrient intake is important during adolescence when children experience rapid growth and sexual maturation. However, there is a lack of research on changes in adolescents' overall dietary quality due to COVID-19. The present study analyzed nationally representative cross-sectional data of 3,785 Korean adolescents aged 12 to 18 years from the Korea National Health and Nutrition Examination Survey (2013-2021). Overall diet quality was assessed using the Korean Healthy Eating Index (KHEI). Mean differences and 95% confidence intervals (95% CI) in the KHEI and key dietary factors between pre-pandemic (2016-2019) and post-pandemic (2020-2021) were calculated to estimate the impact of the COVID-19 on adolescents’ diet. There was no change in the overall diet quality of adolescents during the pandemic, compared to the pre-pandemic period (mean difference pandemic vs prepandemic, -1.496; 95% CI: -3.41, 0.41), whereas a significant decrease was observed in several dietary factors such as breakfasts frequencies, and intakes of saturated fatty acid, sodium, carbohydrate, and energy. Interestingly, there was a suggestion of U-shape of diet changes over the time-course of pandemic. The mean of KHEI (95% CI) decreased from 60.164 (59.14, 61.18) in pre-pandemic (2016-2019) to 57.005 (54.64, 59.37) in the first pandemic (2020) and increased to 60.445 (58.37, 62.52) in the second pandemic (2021). Furthermore, in stratified analyses, the greater decrease in KHEI was found among those with higher household income (P-interaction =0.036) and urban residents (P-interaction =0.029), compared to their counterparts, respectively. In this national representative study of Korean adolescents, there was a suggestion of modest change in diets during the pandemic, compared to the pre-pandemic period. Long-term impact of diet changes during the COVID-19 pandemic on adolescents’ health needs to be determined.
