dc.description.abstractAim: This study aimed to investigate the overall survival and success rates and risk factors affecting the long-term prognosis of autotransplantation of mature and immature third molars to provide a clinical reference for surgical planning and donor tooth choice. Methods: Patients who had received tooth autotransplantation of the 3rd molar between Feb 2002 and Dec 2018 were included. A total of 134 patients were enrolled. Forty-three mature and 107 immature third molars were followed up for a mean of 52.7 ± 46.0 months. Autotransplantation outcome was analyzed from clinical and radiographic criteria. Kaplan-Meier survival analysis and a Cox proportional regression were carried out for risk assessment. Results: Overall cumulative survival rates were 86.4% at 5 years and 76.60% at 10 years. Failure to obtain initial stability occurred in 6 cases, and inflammatory root resorption in 4 cases. Immature third molars showed slightly higher survival compared to mature third molars; however, differences were not significant. Cumulative success rates were 81.5% at 5 years and 70.4% at 10 years. Ankylosis was the most frequent reason for failure. (32.4%). Cox proportional regression model identified older age (>20) and fixation methods as risk factors in autotransplantation. Conclusion: Both immature and mature third molar autotransplantation showed favorable long-term prognosis and demonstrated these modalities' reliability. Careful patient selection, including the age and the appropriate fixation method, should be considered.;목적 : 본 연구는 수술 계획과 치아의 선택을 위한 임상적 참고를 위해 성숙 및 미성숙 제 3대구치의 이식 후 전체 생존율과 성공율 및 위험 요인을 파악하여 제 3대구치의 성숙도가 의도적 재식술의 장기 예후에 미치는 영향을 조사하였다. 방법 : 조사된 인원은 2002 년 2 월부터 2018 년 12 월 사이에 제 3 대구치 사랑니의 이식을 받은 환자들이었으며 총 134 명의 환자가 조사되었다. 43 명은 성숙한 사랑니의 이식을 받았고 107 명은 미성숙 사랑니를 이식 받았으며, 평균 52.7 ± 46.0 개월의 추적이 시행되었다. 자가 이식의 결과는 임상적 및 방사선학적으로 분석되었다. 위험 평가를 위해 카플란 마이어 생존 분석과 콕스 비례 위험 회귀를 실시했다. 결과 : 전체 누적 생존율은 5 년에 86.4 %, 10 년에 76.60 %였다. 초기의 안정성을 얻을 수 없었던 것은 6 건, 염증 뿌리 흡수는 4 건이었다. 미성숙 제 3대구치의 이식이 성숙한 제 3대구치의 이식에 비해 약간 높은 생존율을 나타냈으나 차이는 크지 않았다. 누적 성공률은 5 년에 81.5 %, 10 년에 70.4 %였다. 유착이 실패의 가장 빈번한 원인이었다. (32.4 %). 콕스 비례 회귀 모델을 통해 자가 이식의 위험 인자로 많은 나이(> 20)와 고정 방법인 것을 확인했다. 결론 : 미성숙 및 성숙 제 3대구치의 이식은 좋은 장기 예후를 보여 임상적 사용의 신뢰성을 보여주었다. 그러나 연령과 적절한 고정 방법, 신중한 환자의 선택이 이루어져야 한다.-
