MIDI 키보드 연주 시 노인의 건반타력과 손 기능 검사 간 상관관계

Abstract

본 연구는 음악치료 현장에서 손 기능 재활에 효과적인 중재 및 평가도구로 활용되는 MIDI 키보드의 적용효과를 탐색하고 효율성을 보다 높이고자 설계되었다. 연구 대상은 서울시와 경기도의 지역주민센터와 경로당을 이용하는 만 60세 이상 90세 미만의 일반노인 65명이 선정되었다. 노인들의 MIDI 키보드 타력값(velocity) 측정을 위해서 MIDI 단자가 내장된 전자키보드와 큐베이스(Cubase) 프로그램, 손목지지대가 연구도구로 사용되었다. 표준화된 손 기능 검사도구로는 손의 근력 측정을 위한 악력계(dynamometer), 상지 대소근육의 기민성 측정을 위한 상자와 나무토막 검사(Box and Block test, BBT)와 젭슨 손 기능 검사(Jebsen-Taylor hand function test, JHFT)가 사용되었고, 각 검사도구로 측정된 결과들과 MIDI 키보드로 측정한 타력값 간의 상관관계 검정이 이루어졌다. 연구 결과, 60세 이상 90세 미만 노인의 MIDI 키보드 타력(velocity)의 평균값은 최소 72.74부터 최대 93.58로 측정되었고, 검지와 중지손가락 순으로 최대 수치를 나타내었다. 또한, MIDI 키보드 타력값은 성별 간 유의미한 차이를 보이지 않았다. MIDI 키보드 타력값와 각 검사도구 간 상관관계를 검정한 결과에서는 양손 전체 손가락별 타력값에서 BBT와 JHFT의 ‘문장쓰기’항목과 상관성이 확인되었다. 이는 MIDI 키보드가 타 검사도구들과 같이 노인들의 손 기능을 양적으로 평가할 수 있는 도구라는 사실을 입증하고, 손가락별 타력값이 기민성을 측정하는 도구에서 상관을 나타낸 것은 MIDI 키보드가 손가락의 협응이나 사물조작능력을 향상시키는 목적으로 적용될 수 있음을 시사한다. 결론적으로, MIDI 키보드는 손 기능 저하를 경험하는 노인 또는 신경학적 질환으로 상지기능에 장애를 입은 환자들에게 효과적인 중재도구로 활용될 수 있으며, 아울러 손가락별 기능을 양적 수치로 산출해낼 수 있는 기존도구들과 차별화된 평가도구로서 적용성을 확대할 수 있음을 보고한다.;This study aimed to examine the relationship between the velocity measures of the MIDI-keyboard playing and hand function test results in older adults. The participants were 65 healthy older adults aged 60 to 89 years who regularly visit community senior centers located in Seoul and Kyunggi areas. During the MIDI-keyboard assessment trials, each participant was asked to press keys in sequential patterns with the use of the wrist support board. Also, the hand function tests, including hand grip strength test, Box and Block test(BBT), and Jebsen-Taylor hand function test(JHFT), were performed for each participant. The velocity of key stroke, as an index for finger pressing forces during keyboard playing, and hand function test scores were measured. The relationship between paired test scores was also analysed. As a result of the study, the older adults showed the velocity ranged 72.74 to 93.58 during MIDI-keyboard playing. The highest velocity was observed with the index finger, followed by the middle finger. There were no significant differences in the MIDI data between genders. As for the correlation between outcomes measured from MIDI keyboard and hand function tests, the MIDI scores were found to be significantly correlated with BBT and a subtest of writing in JHFT. These results support that MIDI-keyboard playing would be a reliable tool to assess the hand function of older adults and provide quantitative data. Also, its significant relationship with measures for fine motor dexterity could be interpreted as the applicability of the MIDI-keyboard playing as a hand function task to perform more coordinated movements and manipulate smaller objects. In conclusion, MIDI-keyboard playing would be used not only as an effective assessment tool, but also as an intervention strategy for the rehabilitative purpose for old adults who show deteriorated hand function due to aging or neurological impairment.
