Abstract
Symptomatic treatments in Parkinson's disease are effective in the early stages of the disease. However, as the disease progressed, most Parkinson's disease patients develop motor complications mainly due to the near complete loss of nigral dopamine neurons. Unfortunately neuroprotective therapies that can slow the disease progression are not available. Advances in the knowledge of the possible pathogenic mechanisms that can lead to dopamine cell death have expanded significantly. Oxidative stress is one of the major pathological mechanism in human patients and experimental models of Parkinson's disease. However, antioxidant treatment alone, including vitamin E analogue and its derivatives, demonstrates the marginal neuroprotection against oxidative stress. In one dopamine neuronal cell model, SN4741, oxidative stress induces both apoptosis and autophagic cell death. Apoptosis is known to contributes to dopamine neuronal loss in the substantia nigra of Parkinson's disease patients as well as neurotoxin models of Parkinson's disease. In contrast autophagic cell death has been suggested as an alternative mechanism of cell death in the neurotoxin models, the familial Parkinson's disease gene mutant model and postmortem human Parkinson's disease brains. Accordingly, autophagy inhibitor, 3-methyladenine (3-MA), exerted the significant neuroprotection against oxidative stress in our DA neuron model. Moreover the combined treatment with the 3-MA, an autophagy inhibitor, and Trolox an antioxidant, demonstrated the significant protection against oxidative stress induced cell death. Genetic blockage of apoptosis by caspase 3 gene knockout help increase dramatically the protective effect of 3-MA against the oxidative stress-induced cell death. And we also identified novel neuroprotective agent, LMK02 and KR-23240. Further mechanistic studies on the autophagic dopamine cell death will provide significant insights into the combinational neuroprotective therapy in Parkinson's disease.;파킨슨씨병의 치료법은 도파민 전구체를 투여하는 대증요법이 대부분이다. 이는 질병 발생 초기에는 효과적이나, 병의 특징인 도파민 신경세포의 사멸 진행을 막지 못하여 질병이 진행될수록 치료효과가 감소하고 부작용이 증가한다. 현재까지 이러한 도파민 신경세포의 사멸을 막을 수 있는 치료법은 개발되지 않았다. 도파민 신경세포의 사멸 기작에 관한 연구가 진행됨에 따라 관련되어 있는 여러 요인이 밝혀지고 있다. 그 중에서도 산화 스트레스(oxidative stress)가 환자와 동물 모델의 연구를 통해 주요 병리 기작으로 대두되고 있다. 비타민 E와 같은 항산화제의 단독 투여 시 그 효과는 그리 크지 않다. 도파민 신경세포 모델인 SN4741에서 산화 스트레스는 apoptosis와 autophagic cell death를 모두 일으킨다. Apoptosis는 파킨슨씨병 환자와 신경독소를 이용한 실험 모델에서 모두 도파민 신경세포 사멸에 기여하는 것으로 알려져 있다. 그리고 Autophagic cell death도 파킨슨씨병 관련 여러 모델(신경독소 모델, 파킨슨씨병 관련 유전자 변형 모델, 파킨슨씨병 환자 사후 신경 모델)에서 또다른 세포사멸 기작으로 확인되었다. Autophagy 길항제인 3-metyladenine (3-MA)가 산화 스트레스가 일으키는 세포사멸을 막아주는 것을 도파민 신경세포 모델인 SN4741에서 확인하였다. 또한 수용성 비타민 E 유도체인 Trolox와 3-MA을 혼합하여 처리하였을 시, 산화 스트레스에 대하여 각각의 보호효과 보다 더 큰 신경세포 보호 효과를 확인할 수 있었다. Caspase 3 유전자 삭제를 통한 유전적 apoptosis 저해 상태에서 3-MA를 처리했을 시 autophagic cell death가 수반되리라 추정되는 조건에서 산화 스트레스에 대한 세포보호 효과가 큰 폭 상승하였다. 새로운 화합물인 KR-23240 (Trolox 유도체), LMK02 (생약 추출물)가 SN4741에서 산화 스트레스에 대하여 보호 효과가 있음을 확인하였다. 앞으로의 도파민 신경세포에서의 산화 스트레스에 의한 autophagic cell death 기전 연구 및 혼합 투여를 통한 두 가시 세포 사멸 기작의 효율적 저해 연구는 신경세포 보호를 통한 파킨슨씨병 치료법 개발을 위한 새로운 도구를 제공함은 물론 새로운 병리기전 연구에도 기여할 수 있을 것이다.
