Abstract
Severe insulin resistant syndrome 에서 Wnt signaling의 역할. Rabson-Mendenhall syndrome, Leprechaunism, Type A insulin resistance등과 같은 severe insulin resistannce syndrome은 insulin에 대한 반응성이 떨어지는 것으로 알려져 있었는데, 최근의 여러 보고에서 이들이 growth factor pathway에 결함이 있을 가능성을 제시하고 있었다. 이들 환자에서의 growth factor pathway를 살펴보기 위해서 insulin 자극에 의한 tyrosine phosphorylation정도를 알아보았고, 그 결과 환자세포에서 tyrosine phosphorylation이 감소되어있는 것을 알 수가 있었다. Tyrosine phosphorylation은 ROS와 연관되어 있기 때문에, 환자세포에서 insulin과 growth factor에 의해 유도되는 ROS를 살펴 보았는데, 마찬가지로 환자세포에서 ROS level이 감소되어있음을 알 수가 있었다. 따라서 이들 환자에서는 insulin뿐만 아니라 growth factor pathway에 관련된 ROS생성 mechanism자체에 문제가 있는지를 알아보기 위해서 NADPH oxidase complex의 component들을 조사해 보았다. 그 결과 NADPH oxidase complex의 component들 중에서 p67phox의 발현정도가 감소되어 있는 것을 확인할 수가 있었으며, 이것이 환자세포에서 growth factor에 의한 ROS생성에 문제를 일으키는 요인임을 알 수가 있었다. 따라서 severe insulin resistance syndrome 과 일반인 사이의 근본적인 차이점을 알아보기 위해서 이들 사이에서 다르게 발현되는 유전자를 찾아 보았고, 그 결과 Wnt signaling의 inhibitor로 알려진 Dickkopf1 유전자가 이 환자에서 과 발현 되어있는 것을 확인할 수가 있었다. 이들 환자에서 나타나는 분자적 특이성을 알아보기 위하여 Wnt signaling의 하부 signaling인 β-catenin-TCF pathway를 살펴보았고, 그 결과 이들 환자에서는 Wnt2자극에 의한 β-catenin-TCF pathway가 과발현되어 있는 Dickkopf1 유전자에 의해서 저항성을 나타내는 것을 알 수가 있었다. 그리고 정상세포에 Dickkopf1을 과발현 시켰을 경우에도 환자세포에서와 같은 저항성을 보이는 것을 확인할 수가 있었고, 환자세포에서 과발현되어 있는 Dickkopf1 을 siRNA를 이용하여 줄였을 때 Wnt signaling이 어느 정도 회복되는 것도 확인할 수가 있었다. 그 다음으로는 환자세포에서 과 발현된 Dickkopf1유전자와 처음에 살펴본, 환자세포에서 감소되어있는 p67phox사이의 연관성을 찾기 위해서 p67phox의 promotor에 binding 하는 요소들 가운데 Wnt signaling과 관련 있는 것으로 알려진 AP-1과 NFAT를 실험에 도입하게 되었다. 그 결과 정상세포에서 Wnt2자극에 반응을 나타낸 NFAT를 환자세포에서 실험해본 결과 p67phox가 감소된 것과 같이 NFAT도 환자세포에서 비활성화 되어있는 것을 알 수가 있었다. 따라서 이 실험들을 통해서 알 수 있는 사실은 severe insulin resistance syndrome에서 과 발현되어있는 Dickkopf1에 의해서 NFAT를 통한 p67phox의 조절이 억제 된다는 것이다. ; Several lines of evidence suggested that severe insulin resistance syndrome, such as Rabson-Mendenhall Syndrome and Leprechaunism showed low responsive properties against growth factors compared to normal cells. To investigate the difference between normal and severe insulin resistance syndrome, we have established the patient cell lines and screened differential expressed proteins in the patient cell lines. Collegue in my lab has found that Dickkopf1 (DKK1) is overexpressed in patient cell lines. Dickkopf1 is secreted developmental regulator and acts as intrinsic Wnt inhibitor through binding with Wnt coreceptor, LRP6. To determine the molecular characteristics of patient cell lines, we used TCF transcription reporter (TOPFLASH) luciferase assay. Several patient cell lines showed poor responsive properties against Wnt2 stimulation. Moreover, Wnt2 failed to activate β-catenin-TCF pathway in normal cells expressing of DKK1. To identify impaired cellular mechanism in patient cells, tyrosine phosphorylation of cytosolic proteins and ability of ROS generation upon insulin and growth factor stimulation are investigated. Tyrosine phosphorylation of protein and ROS generation are increased by insulin and growth factor stimulation in normal cells, whereas we failed to detect of increase of tyrosine phosphorylation of protein and ROS generation upon those agonsts stimulation in patient cells. It is well known that ROS is essential component for growth factor-mediated signaling. The scavenging of ROS by the addition of catalase diminished PDGF- or EGF- induced cell signaling. Although molecular pathway for ROS generation is not well established, previous reports indicated NADPH oxidase complex including gp91^phox (Nox1), p67phox, p47phox, p40phox, rac1, and PI3K are essential for the ROS generation. Expressions of several components (Nox1, p47phox, PI3K and rac1) in patient cells are not changed, whereas expression of p67^phox is downregulated. The result suggests that impaired ROS generation by defect of p67^phox induced low tyrosine phosphorylation of cytosolic protein upon growth factor and insulin stimulation. Next question is that how can connect the overexpression of DKK1 and downregulation of p67^phox protein. It is known that Wnt protein activates not only TCF-responsive genes but also AP-1 and NF-AT transcriptional factor which can bind to upstream sequence of p67phox. It means that Wnt signaling mediates p67^phox expression. Luciferase tagged-TOPFLASH, AP-1, and NF-AT are transfected into normal cells, respectively. NF-AT is activated by Wnt2 stimulation in normal cells, whereas patient cell lines overexpressing cannot activate NF-AT following Wnt2 stimulation. The result indicates that overexpression of DKK1 in severe insulin resistant syndrome inhibited p67^phox expression through downactivation of NF-AT.
