- 생체 대사의 중추인 간은 알코올 중독에 가장 영향을 많이 받는 조직이다. 따라서 장기간의 알코올 섭취는 간지질 대사장애로 인하여 조기에 지방간을 초래할 뿐만 아니라 이중 다수에서 회복키 어려운 간 경화증까지 초래한다. 한편 알코올은 기질특이성을 갖는 독특한 hepatic microsomal cytochrome P-450을 유발시켜서 다른 간독성인자를 활성화시킴으로 간손상을 일으킬 수 있다.
간경화증과 간암 간의 밀접한 관계는 잘 알려진 사실이지만 간경화증 그 자체가 전암성 (premalignant)인지 혹은 알코올이 공통원인 인지로서 두 질환을 공히 유발시킬 수 있는지는 아직도 중요 연구대상이다.
저자는 16주간 15%, 30% ethanol 용액 및 시판 알코올음료를 일정량 투여한 흰쥐의 간조직내 지질변화와 microsomal cytochrome P-450 및 b_5의 함량을 8주와 16주 두차례에 걸쳐 측정하고, 그 toxic metabolite가 강력한 간암 유발물질인 2-acetylaminofluorene (2-AAF)을 모델화합물로 사용하여 알코올 투여로 유발된 P-450의 N-OH-AAF(활성형)와 ring-OH-AAF(비활성형)의 형성을 살펴봄으로써 2-AAF의 발암성에 대한 ethanol 장기 투여의 영향을 증명하고자 한 바 다음과 같은 결과를 얻었기에 보고하는 바이다.
1. 간조직내 총 cholesterol과 triglyceride 함량은 장기간 ethanol 투여로 증가 추세에 있었으며, phospholipid 함량에는 큰 변화가 없었다.
2. 간조직내 microsomal cytochrome P-450은 15%, 30% ethanol 용액 및 시판 알코올음료 투여시 8주까지는 증가하였으나 16주에는 감소하였고, cytochrome b_5에 있어서는 큰 변화가 없었다.
3. ethanol이나 시판 알코올음료로 처리한 간조직의 AAF화합물 ring^- 및 N-hydroxylation에 있어서는 ring-hydroxylation은 큰 변화가 없었으나 N-hydroxylation은 계속 증가하고 있음을 알 수 있었다.
4. 간조직내 지방함량의 증가와 cytochrome P-450 및 AAF N-hydroxylation과는 관계가 있는 것으로 사료되었다.
; It is now well established that ethanol excerts different effects on hepatic cellular metabolism, depending mainly on the duration of its intake. And induction of quantitative and qualitative alterations in the cytochrome P-450 of hepatic microsomes after chronic ethanol feeding is a well-documented phenomenon.
But the effect of ethanol ingestion on the drug metabolism appear paradoxic and the ability of chronic ethanol ingestion to potentiate the carcinogenicity of 2-acetylaminofluorens (2-AAF) by microsomal activation is not confirmed yet.
The present study was undertaken to demonstrate the effects of 15%, 30% ethanol and a commercial liquor feeding for 16 weeks on the lipids in the rat liver, hepatic microsomal cytochrome P-450 and b_5, and to elucidate whether or not ethanol-induced rat microsomes can increase the risk of cancerby increasing the rate of metabolic activation of 2-AAF in vitro.
The results are followings.
1. The chronic alcohol feeding increased the contents of total cholesterol and triglyceride in the liver, but did not make a significant influence on the phospholipid contents of liver.
2. The 8 week alcohol feeding increased the hepatic microsomal cytochrome P-450 without the change of cytochrome b_5, but the 16 week feeding decreased P-450 significantly.
3. The chronic alcohol feeding elevated the AAF N-hydroxylation continuously but did not influence on AAF ring-hydroxylation.
4. The correlation of AAF N-hydroxylation and the increases of hepatic lipid contents was indicated.
