간호간병통합서비스 병동과 일반병동의 욕창과 낙상 발생률 비교

Abstract

본 연구에서는 환자 안전 지표가 강화되고 지표관리의 중요성이 대두됨에 따라 간호간병통합서비스 시행 전후 낙상과 욕창 발생 환자를 비교하고 간호간병통합서비스 시행 후 일반병동과 간호간병통합서비스 병동에 낙상과 욕창 발생 환자를 비교하여 간호간병통합서비스 도입이 낙상과 욕창 발생률의 변화에 영향을 연구한 후향적 연구이다. 이에 본 연구 목적은 간호간병통합서비스의 확산을 위한 기초적인 자료를 제공하며 제도적 정착에 도움이 되기 위함이다. 자료수집은 경기도 소재의 1개 종합병원의 간호간병통합서비스 시행 전인 2014년 6월부터 2015년 6월 말까지 와 간호간병통합서비스 시행 후인 2022년 6월부터 2023년 6월 말 기간에 병동 입원 환자 중 욕창, 낙상이 발생한 환자를 대상으로 전자 의무기록에서 얻은 자료를 이용하였다. 연구를 수행하기 전 연구자가 소속된 기관의 연구윤리심의위원회(IRB) 승인받은 후 연구(2023-10-006-001)를 수행하였다. 수집된 자료는 IBM SPSS/WIN 28.0 프로그램을 활용하여 분석하였다. 본 연구의 주요 결과는 다음과 같다. 낙상과 욕창을 경험한 환자와 경험하지 않은 사람으로 나눠 그룹 간 발생의 차이를 알아본 결과 일반병동과 간호간병통합서비스 병동 비교에서 일반병동에 비해 간호간병통합서비스 병동에서 유의하게 감소하였다. 일반병동은 시행 전후 비교에서 증가하는 양상을 보였다. 간호간병통합서비스 시행 전 일반병동에서 시행 후 간호간병통합서비스 병동으로 전환되는 병동 간 비교에서는 간호간병통합서비스 시행 후 전환 병동에서 증가하는 양상을 보였다.;The purpose of this study is to compare the incidence of pressure injury and falls in the Comprehensive Nursing Care wards and general wards to study the effect of the introduction of the Comprehensive Nursing Care system on the change in the incidence of pressure injury and falls. Data was collected from electronic medical records for patients admitted to the ward from June 2014 to the end of June 2015 before the Comprehensive Nursing Care service of one general hospital in Gyeonggi-do and from June 2022 to the end of June 2023 after the Comprehensive Nursing Care service was implemented.The collected data were analyzed using the IBM SPSS/WIN 28.0 program. The main results of this study are as follows. As a result of examining the difference in occurrence between groups by dividing it into patients who have experienced falls and pressure injury and those who have not, the general nursing service ward significantly decreased compared to the general ward. The general ward showed an increasing pattern in comparison before and after implementation. In the comparison between wards that are converted from the general ward to the general nursing service ward after implementation before the implementation of the comprehensive nursing service, there was an increasing pattern in the conversion ward after the implementation of the comprehensive nursing service.
