발달장애성인 형제자매의 돌봄에 대한 인식과 지원 요구

Abstract

우리나라의 장애인 평균 수명이 증가하면서 장애 성인에 대한 미래 돌봄의 중요성이 강조되고 있다. 더욱이 대부분의 발달장애인은 성인기 및 그 이후에 사회 속에서 살아가는 데 어려움이 있기에 가족의 돌봄을 받으며 살아가며, 부모의 노화로 장애 자녀의 돌봄이 어려워지면, 돌봄에 대한 책임은 형제자매의 몫이 된다. 하지만 발달장애인의 지원은 아동기 또는 청소년기에 집중되어 있기에 형제자매의 돌봄에 대한 걱정은 가중된다. 이에 본 연구는 발달장애 형제를 둔 형제자매가 미래 돌봄을 준비하는 데 필요한 구체적인 요구를 알고 향후 형제자매를 대상으로 한 가족 지원의 기초가 되는 자료를 제공하고자 돌봄에 대한 형제자매의 경험 및 인식과 이에 따른 구체적인 지원방안을 조사하였다. 본 연구의 참여자는 발달장애성인의 형제자매 11명으로 연구 참여자는 목적표집(purposive sampling)과 눈덩이 표집(snowballing sampling)방법을 통해 모집하였다. 연구 참여자와의 개별 면담을 위해 반구조화된 면담지를 개발하였으며, 면담지 초안에 대하여 예비 면담 실시 및 내용타당도 검증을 받았다. 이후 검증 내용을 통해 면담지를 수정 및 보완하여 최종 면담지를 완성하였으며, 이를 토대로 개별 심층 면담을 실시하였다. 이를 통하여 수집된 자료를 분석한 결과, 최종적으로 4가지 대주제와 12가지 하위주제로 범주화되었다. 첫 번째 주제는, 돌봄에 영향을 미치는 요인으로, 연구에 참여한 형제자매는 발달장애형제 돌봄에 영향을 미치는 요인으로 어린 시절의 경험, 형제자매 관계 그리고 가족으로서 책임감을 꼽았다. 어린 시절 장애 형제의 돌봄 경험으로 인한 스트레스와 장애에 대한 이해의 부족이 성인이 된 이후 돌봄에도 영향을 미쳤으며, 장애 형제와의 관계와 다른 형제자매와의 유대감이 돌봄에 영향을 미쳤다고 하였다. 또한 형제자매라는 책임감이 돌봄에 큰 영향을 주었다. 두 번째 주제는 돌봄을 위한 현재의 노력으로, 형제자매가 발달장애 형제의 돌봄을 위해 돌봄을 분담하며, 돌봄자로서 스스로의 미래계획을 세우고 있었다. 형제자매는 성인이 되며 부모의 돌봄 역할을 분담하고 있었고, 가족 구성원과 미래 장애 형제의 돌봄에 대한 고민을 함께 나누고 있었다. 또한 미래의 돌봄자로서 자신의 진로계획 또는 결혼계획 등 미래 돌봄에 영향을 미칠 수 있는 결정에 대하여 고민하였다. 세 번째 주제는 돌봄 관련 고충으로, 형제자매는 장애 형제 돌봄에 불충분한 기관 서비스, 서비스에 대한 정보 부족, 재정 및 주거 관련 고민 그리고 부정적인 사회 인식으로 인하여 어려움을 겪는다고 하였다. 장애 형제를 돌보는 데 있어서 서비스 기관이 부족하고 서비스의 질적인 측면이 불만족스럽다고 인식하였으며, 서비스의 홍보가 부족하고 해당 전문가의 정보가 부족하다고 하였다. 더하여 돌봄에 대한 경제적 지원과 주거 배치를 돌봄의 어려움으로 꼽았으며, 부정적인 사회 인식 또한 돌봄에 걸림돌로 인식하였다. 네 번째 주제는 돌봄을 위한 지원 요구로, 장애 가족의 휴식 지원, 응급 돌봄 서비스 마련, 미래계획 지원 서비스 마련 등 가족 지원 서비스가 강화되어야 한다고 느꼈다. 또한 주간 활동 프로그램을 확대하고, 의료 지원 서비스, 직업 지원 서비스, 권리 옹호 지원 서비스 및 심리적 지원 서비스를 제공하는 등의 발달장애인을 위한 지원 서비스 확대의 필요성을 이야기하였다. 더하여 돌봄 지원 원스탑 기관 마련 및 지역 간의 균등한 서비스 제공 등의 행･재정적 지원 확대가 제안되었다. 본 연구는 발달장애성인 형제를 둔 형제자매의 돌봄 경험 및 인식을 탐색하고, 그 과정에서의 어려움을 확인하였으며, 이에 대한 구체적 지원 요구를 제시하였다는 점에서 의의가 있다. ;As the average life expectancy of the people with disabilities in Korea increases, the importance of future caregiving for adults with disabilities is being emphasized. Moreover, most people with developmental disabilities have difficulties in social life, so they live under the care of their family members. But when it becomes difficult to care for children with disabilities due to the aging of parents, siblings naturally take responsibility for caring for their sibling with disabilities. However, as the support services for people with developmental disabilities are concentrated in childhood or adolescence, concerns about the future care by siblings are increasing. The participants of this study were 11 siblings of adults with developmental disabilities, and through the purposive sampling method and snowballing sampling method, the participants who could provide the information of their perceptions and support needs in order to care their siblings with developmental disabilities were selected. A semi-structured questionnaire was developed by reviewing preliminary studies and modified to complete the final questionnaire. By using the revised final questionnaire, the individual in-depth interviews were conducted. As a result of analyzing the data, 4 major themes and 12 sub themes were derived. 4 major themes are as follow; the first major theme was ‘The factors effecting caregiving’. Participants mentioned that childhood experiences, relationship of siblings and responsibilities as factors affecting caring for their siblings with developmental disabilities. The stressess that they got by caring their siblings with developmental disabilities during childhood and their misunderstandings about disability affect caring for their siblings with developmental disabilities into adulthood. Also, their relationship with siblings with developmental disabilities and their responsibilities to care their siblings with disabilities have had a huge effect on caregiving. The second major theme was ‘Their efforts to care’. The siblings shared care with their parents and they also planned their future as caregivers. As they became adults, they began to share the role of caring, and shared concerns about future caring of their sibling with disabilities with other family members. They also mentioned that they thought about decisions that could affect future care as a future caregiver, such as their career plan or marriage plan. The third major theme was ‘The difficulties about caregiving’. Siblings have noticed that they are suffering from insufficient services, lack of information on services, financial and housing concerns, and negative social perceptions. They recognized that the service institution was insufficient and the quality aspect of the service was unsatisfied, the promotions of service were insufficient and the information that they could get from the experts was limited. The fourth major theme was ‘The support needs for caregiving’. They pointed out that the family support services such as supporting rest for families with disabilities, preparing for emergency care services, and support services on future planning for families should be strengthened. Specifically they mentioned about the need to expand administraive and financial support, such as personal assistance services, establishing one-stop institutions of care support and providing equal services between regions. In addition, support services for adults with developmental disabilities such as expansion of daily activity programs, medical support services, vocational support services, rights advocacy support services, and psychological services were proposed. Based on the results derived from this study, specific factors on the caring experience and difficulties of siblings were identified and discussed in terms of expanding support needs for caregiving.
