dc.description.abstractInfants are in a significant period in which proper eating habits need to be established. In such period, they experience various kinds of food and can choose food due to development of self-ego, so that they might eat only what they want because they can distinguish between likes and hates. Feeding disorder of infants means ‘refusing or not being to eat food to a certain extent in which infants cannot maintain the required nutritive condition for development.’ The reported rate of possession of infants’ feeding disorder is about 25 ~ 45% of normally developing infants, and 33 ~ 80% of developmental disorder infants; 3~10% among them may develop their feeding problem into a serious one. Infants with feeding problems accompany various problematic behaviors during meal time. For instance, only one refuses to have a meal, vomits, keeps the food in one’s mouth, moves around during meal time, or etc; they show such negative emotions or behaviors. Because a family prepares the fundamental environment for dietary life of infants, eating education is effective linking and teaching with the family like other education. Furthermore, many preceding studies hoped for parents of infants with feeding problems to educate properly. However, the introduced programs in Korea are in sufficient. Hereupon, coming up to such parents’ expectations, the study aims to provide parent education program for feeding problem infants, and to find out the effect of parent education on eating behaviors of infants. This study conducted parent education program on parents of 4 infants with feeding problem once a week and total 4 times. feeding problem infants were selected through beforehand interview on parents by categories; eating 30 or less kinds of food, refusing certain food group or new food, or infants who are low in weight or short. To observe the change of infants’ behaviors, the meal time of infants were filmed before and after the parent education program. The filmed video recorded observed behavior frequency according to a behavior list of 10 problematic behaviors. Parent education program is composed based on Sequential Oral Sensory Approach to Feeding and Food Chaining. The program consists of 4 sessions. First session was to introduce the program and identify the problem, Second session was to introduce ‘Step to Eating’ and investigated food with parents and children. In the third session, parents learned food hierarchy and food chaining. Fourth session was to explain the reason to eat by combining recognition activity. For infants who are too young or delayed in development to understand recognition activity, food investigation through sensory play was proceeded. Two expert occupational therapists of 3 or more years as observers participated for data analysis. Watching the filmed video, they recorded the frequency when a behavior corresponding to an item in problematic behavior occurred. As a result of the study, there was a tendency which frequency of infants’ problematic behaviors reduced more after the parent education than before. Furthermore, there were differences in observed behaviors of 10 behaviors list which provides a basis for behavior observation by infants, However, the all infants tended to express less behaviors in one item, ‘He/she does not participate in meal time easily.’ Based on the study results, implications and limitations of the study are as follows. Parent education program can help the parents with feeding problem infants. Also, providing opportunities to try various kinds of food within a range in which the infants allow due to food system affects infants to allow new food. However, the study could not find the statistical meaningful changes due to lack of study subjects and investigated changes according to only the 10 behaviors list. Because the researcher of the study and the observer were the same person, the possibility of influence of the researcher’s expectation about the subjects on the study cannot be ignored. Furthermore, because the study was proceeded for 4 weeks, a relatively period, it could not identify if the changes of the infants continues. Based on the study, multilateral follow-up studies in depth about parent education program for disordered eating infants are expected to be continued.;영유아기는 올바른 식습관이 확립되는 중요한 시기이다. 다양한 종류의 식품들을 먹어보게 되고 자아성의 발달로 음식을 선택 할 수 있게 되어 좋아하는 음식과 싫어하는 음식을 뚜렷이 구별하게 되며 편식이 생기기도 한다. 영유아의 섭식 장애란 성장에 필수적인 영양 상태를 유지할 수 없을 정도로 음식 섭취를 하지 못하거나 섭취를 거부하는 것을 의미한다. 영유아의 섭식장애의 유병율은 정상발달을 하는 영유아의 약 25~45%와 발달장애 영유아의 33~80%에서 문제가 보고되고 있으며, 이 중 3~10% 정도는 심각한 섭식 문제로 발전할 수 있다. 섭식 문제가 있는 영유아는 식사 시간에 다양한 문제 행동을 동반한다. 예를 들면 혼자서 식사를 하지 않으려 하거나, 음식을 토하거나, 삼키지 않고 입안에 물고 있거나, 식사를 하면서 계속 돌아다니는 등의 부정적인 정서나 행동을 보인다. 가정은 영유아의 식생활 형성에 기초적인 환경을 마련하고 있기 때문에, 섭식 교육은 다른 교육들과 마찬가지로 가정과 연계하여 교육하는 것이 효과적이다. 또한 여러 선행 연구에서 섭식 문제를 가진 영유아의 부모들이 적절한 교육을 희망하였다. 그러나 아직 국내에 소개된 프로그램은 부족한 실정이다. 이에 본 논문은 이와 같은 부모의 기대에 부응하여 섭식문제 영유아를 위한 부모교육 프로그램을 제공하고 부모교육이 영유아의 섭식 행동에 어떠한 영향을 미치는지 알아보는 것이 목적이다. 본 연구는 4명의 섭식 문제 영유아의 부모를 대상으로 부모교육 프로그램을 주 1회씩 4회 시행하였다. 섭식 문제 영유아는 사전 부모 인터뷰를 통하여 30가지 이하의 한정된 음식만을 먹거나, 특정 식품군을 먹지 않거나, 새로운 음식에 거부 반응을 보이거나, 키나 몸무게가 또래에 비해 현저하게 적은 영유아를 선별하였다. 영유아의 행동 변화 관찰을 위해 부모 교육 프로그램 전후에 유아의 식사 시간을 30분간 비디오 촬영하였다. 촬영된 영상은 10가지 문제 행동이 나열된 행동 목록에 근거하여 관찰된 행동 빈도를 기록하였다. 부모교육 프로그램은 섭식을 위한 순차적구강감각접근(Sequential Oral Sensory Approach to Feeding)과 먹기체계(Food Chaining)를 바탕으로 구성되었다. 프로그램은 총 4회기로, 1회기는 프로그램 소개 및 문제 확인을 목적으로 하였으며, 2회기는 먹기단계(Step to Eating)를 안내하고 아이와 같이 음식을 탐색하였다. 3회기는 음식선택체계(Food Hierarchy)와 먹기체계(Food Chaining)를 교육하였고, 4회기는 인지 활동을 접목하여 음식을 먹어야 하는 이유를 설명하였다. 나이가 어리거나 발달 지연으로 인지 활동을 이해하기 어려운 유아는 감각 놀이를 통한 음식 탐색을 진행하였다. 자료 분석은 3년 이상의 숙련된 작업치료사 2명이 관찰자로 참여하였다. 촬영 된 동영상을 보면서 문제 행동 목록표에 해당되는 행동이 발생 할 때마다 빈도를 기록하였다. 연구 결과 부모 교육을 받기 전보다 부모 교육을 받은 후에 영유아의 문제 행동이 감소되는 경향을 보였다. 또한 각 유아에 따라 행동 관찰의 근거가 되는 10가지의 행동 목록 중에서 관찰된 행동에 차이가 있었는데, ‘식사에 쉽게 참여하지 않는다’의 한 개 목록에서 네 유아 모두 행동이 감소된 경향을 보였다. 연구 결과를 토대로 본 논문의 시사점 및 제한점은 다음과 같다. 부모교육 프로그램은 섭식 문제 영유아의 부모에게 도움이 될 수 있다. 또한 음식 체계에 따라 아이가 허용하는 범위 내에서 다양한 음식을 탐험 할 수 있는 기회를 제공한 것이 아이가 새로운 음식을 허용하는 것에 영향을 미친다. 그러나 본 연구는 연구에 참여한 대상자가 적어 통계적으로 유의미한 변화를 알 수 없었고, 10가지의 행동 목록에 대해서만 변화 여부를 알아보았으며, 본 연구의 연구자와 관찰자가 동일 인물이었으므로 실험 참가자에 대한 연구자의 기대가 연구에 영향을 주었을 가능성을 배제할 수 없다. 또한 비교적 짧은 기간인 4주에 걸쳐 진행되어 영유아의 변화가 지속적으로 진행되는지에 대한 검증을 하지 못했다. 본 논문을 바탕으로 섭식문제 영유아를 위한 부모교육 프로그램에 대한 심층적이고 다각적인 후속 연구가 이어지기를 기대한다.-
