Antibody responses in hematopoietic cell transplantation recipients after vaccination against Haemophilus influenzae type b and Streptococcus pneumoniae

Abstract

Purpose: Hematopoietic cell transplantation (HCT) recipients are vulnerable to invasive infection due to Haemophilus influenzae type b (Hib) and Streptococcus pneumoniae (Sp). Evaluation of immune responses after vaccination is important to ensure protection in these immunocompromised hosts. Data on antibody response after vaccination against Hib and Sp are needed in Korean HCT recipients. Materials and Method: Pediatric HCT recipients were prospectively enrolled at Samsung Medical Center, Seoul, Korea during 2009-2011. Patients received vaccines at least one year after HCT according to our study protocol. After blood collection was performed at scheduled time points, serum from each patient was separated within 1 hour and stored at -80°C. ELISA tests to detect anti-PRP IgG antibody and antibodies to Sp serotypes 4, 6B, 9V, 14, 18C, 19F, and 23F were performed at the Center for Vaccine Evaluation and Study, Ewha Medical Research Institute, Seoul, Korea. Results: Ten pediatric HCT recipients were enrolled. Median age was 5.4 years (range 2.7-12.2 years). Five were male and five were female. Two patients were allogeneic HCT recipients and 8 were autologous HCT recipients. Two patients had acute myelogenous leukemia (AML) and eight patients had solid tumors. For Hib, before Hib vaccination, 60% of patients’ anti-PRP IgG titers were below 0.15 μg/mL. After vaccination, 100% of patients’ anti-PRP IgG titers increased above 0.15 μg/mL and 1.0 μg/mL. In each individual patient, significant increase in anti-PRP IgG titer above protective level was observed. For pneumococcus, in patients aged 2-5 years, except serotype 19F, pre-vaccination geometric mean concentrations (GMCs) of IgG for six serotypes (4, 6B, 9V, 14, 18C, and 23F) were below 0.35 μg/mL and post-vaccination 5 month GMCs of IgG for all 7 serotypes increased above 0.35 μg/mL. In patients aged older than 5 years, pre-vaccination GMCs of IgG for 4 serotypes (4, 9V, 14, and 23F) were below 0.35 μg/mL and post-vaccination 3 month GMCs of IgG for all 7 serotypes increased above 0.35 μg/mL. Conclusion: Antibody response measurement after Hib and Sp vaccination in Korean pediatric HCT recipients was performed. It was observed that most HCT recipients have low or no protective antibodies to Hib and Sp before vaccination but good immune responses were observed after vaccination to protective level. More data are needed on detailed immune response to Hib and Sp vaccination in Korean HCT recipients.;조혈모세포 이식 환자들은 b 형 헤모필루스 인플루엔자(Haemophilus influenzae type b, Hib)과 폐렴구균(Streptococcus pneumoniae, Sp)에 의한 침습성 감염에 취약하다. 이들 면역저하 환자에서 백신 접종 후 면역 반응을 평가하는 것은 매우 중요하며 한국 조혈모세포이식 환자에서 Hib와 Sp 백신 접종 후 면역 반응에 대한 데이터가 필요한 실정이다. 삼성서울병원에서 2009-2011년 사이에 조혈모세포 이식 환자들에게 전향적으로 Hib 와 Sp 백신을 접종하였으며 면역 반응을 평가하고자 이화 백신 효능 센터에서 Hib에 대한 anti-PRP IgG 와 Sp 의 4, 6B, 9V, 14, 18C, 19F, 23F 혈청형에 대한 항체가를 측정하였다. 10명의 소아환자가 참가하였고 연령의 중앙값은 5.5세 (범위, 2.7-12.2세). 남녀 비는 1:1 이었다. 8명은 자가이식, 2명은 동종이식을 받았다. Hib 백신 이전에는 60%의 환자에서 anti-PRP IgG가 0.15 μg/mL 보다 낮았으나 백신 접종 후 100%의 환자에서 0.15 μg/mL와 1.0 μg/mL 이상으로 증가하였다. Sp 백신에 대한 면역반응으로 2-5세 환자군은 접종 전 검사한 7개의 혈청형 중 6개의 혈청형(4, 6B, 9V, 14, 18C, 23F)에 대한 기하평균 항체가가 0.35 μg/mL 미만 이었으나 Sp 백신 접종 시작 5개월째 7개 혈청형에 대한 기하평균 항체가가 모두 0.35 μg/mL 이상으로 의미 있게 증가하였다. 5세 초과의 환자 군에서는 접종 전에 4개의 혈청형(4, 9V, 14, 23F)에 대한 기하평균 항체가가 0.35 μg/mL 미만 이었으나 백신 접종 시작 3개월째 검사한 7개 혈청형에 대한 기하평균 항체가가 모두 0.35 μg/mL 이상 증가하였다. 결론적으로 본 연구는 한국 소아 조혈모세포 이식 환자에서 Hib와 Sp 백신 접종 후에 두가지 모두에 대한 면역 반응을 모두 측정한 최초의 연구이며, 대부분의 환자들이 백신 접종 전에 방어 항체가가 낮거나 측정되지 않았으나 백신 접종 후 면역 반응을 보임을 관찰하였다. 한국 소아 조혈모세포 이식 환자에서 이들 백신에 대한 면역 반응에 대한 추가 연구가 필요할 것으로 사료된다.
