근·골격장애 환자의 간호질 평가 도구개발

Abstract

We need to continuously evaluate for nursing quality and the outcome to reflect nursing, to repair and improve of nursing. The first problem for nursing quality improvement is development of evaluation too1 for the quality of nursing care. We need a system for quality assurance evaluation that is, development and framework of evaluation tool for nursing care quality. The purpose of this study was attempted to develop an evaluation tool for the quality of nursing care in musculoskeletal disorder patients. The approach method of nursing quality for the development of the tool in this study was process evaluation framework. The study were devided development process and reliability, validity verification process. The subjects of development process of this tool are three nursing export groups, and the subject of reliability, validity verification process are 20 samples of two university hospital in seoul, who were within discharge 3 months after admission treatment in musculo-skeletal disorder. Data for this study was collected from March 10 to April 13, 1995. The development process of the tool were as follows : 1. Make preliminary list of the tool by focal group were constituted 12 clinical nurses. 2. Modify and add preliminary list by 4 nursing expert panel. 3. Calculate content validity of the tool by 25 nursing expert panel of judge. 4. Verify reliability and validity of the tool. 5. Finalize an evaluation tool for the quality of the nursing care in musculo-skeletal disorder. The results of this study were as follows : 1. Development an evaluation tool for the quality of nursing care in musculo-skeletal disorder. a. The evalution tool of this study was developed 5 standards , 33 criterias and 133 indicators. b. 5 standards were divided according to Nursing Process. from standard 1 to standard 5, involved criterias were each 6(18%), 3(9%), 3(9%), 15(46%), 6(18%). 2. Verify reliability and validity of the tool. a. Score of adequate degree for content validity of 33 criterias and 133 indicators were every average 2.82. b. Inter-rater reliabilities(consentaneity score) of the tool by pearson correlation coefficient between three raters were : r= .7506, r= .8934, r= .6695. and Inter-rater reliabilities by single- facet crossed design were : r= .7464. c. The alpha coefficient relating to internal consistency was .8524 over all 30 items of 33 criterias of developed tool. d. Score of the quality of nursing care following to general characteristics of this study subjects were stastically significant differences according to educational level ( F=2.93, p= .029) and diagnosing classification ( F= 2.50, p= .042). Through this study, I'm sure that the developed tool for the quality of patient care in musculo-skeletal disorder will show the way of more improvement of the quality of nursing care and effective nursing interventions.;질적으로 우수한 양질의 간호서비스를 제공하고 보장하기 위해서는 간호의 질을 계속적으로 평가하고 그 결과를 간호에 반영하여 간호업무를 개선하고 향상시켜 나가야 한다. 간호의 질 향상을 위한 가장 중요하면서도 우선적인 과제는 환자 간호의 질평가를 위한 도구의 개발이다. 환자 간호의 질평가에는 체계가 필요하다. 즉 질평가를 위한 중요한 단계들과 함께 질평가 도구의 개발과 틀이 필요하다. 이에 본 연구는 근·골격장애 환자의 질적 간호를 제공하기 위한 질평가 도구의 개발을 목적으로 시행되었다. 환자 간호의 질 향상을 위해 간호활동의 순서와 방향을 제시해 주고 간호행위에 대한 의사결정을 하고 결과를 예측하고 평가할 때 필요한 간호과정을 통해 환자 간호의 요소를 설명하는 것이 필요하다고 보아 근·골격장애 환자 간호의 질평가 접근법으로 환자에게 행해지는 모든 간호과정의 유용성과 효과를 평가하는 과정평가 방법을 선택하였다. 본 연구는 도구의 개발과정과 도구의 신뢰도, 타당도 검증과정으로 나뉘어진다. 도구 개발과정에서의 대상은 3그룹의 간호전문가 집단이었으며 도구의 신뢰도, 타당도 검증을 위한 대상은 서울에 위치한 2개의 A대학부속병원의 정형외과 병동에서 근·골격장애로 입원 치료를 받고 퇴원 후 3개월 이내의 환자 20명을 대상으로 하였다. 신뢰도, 타당도 검증을 위한 자료수집 기간은 1995. 3. 10부터 4. 13까지 이루어졌다. 본 연구의 도구개발 과정은 다음과 같다. 1. 12명의 임상간호사로 구성된 초점집단을 통한 자료수집 및 문헌고찰을 통한 도구의 예비목록 작성 2. 4명의 전문가 소집단을 통한 예비목록의 수정, 보완 3. 25명의 전문가 평가단을 통한 예비도구의 내용 타당도 측정 4. 개발된 근·골격장애 환자의 간호질 평가 예비도구의 신뢰도 검증 5. 신뢰도, 타당도 검증을 거친 표준, 기준 및 지표를 기초로 근·골격장애 환자의 간호질 평가 도구확정 본 연구의 결과는 다음과 같다. 1. 근·골격장애 환자의 간호질 평가를 위한 도구의 개발 a. 개발된 근·골격장애 환자의 간호질 평가 도구는 5개 표준, 33개 기준, 133개 지표로 구성되어 있다. b. 개발된 도구의 표준은 간호과정에 따라 5개로 나뉘어진다. 표준 1에서 표준 5까지 포함된 기준 수는 각각 6개(총 기준의 18%), 3개(9%), 3개(9%), 15개(46%), 6개(18%)이었다. 2. 개발된 도구의 신뢰도, 타당도 검증 a. 근·골격장애 환자의 간호질 평가 도구로 개발된 5개 표준에 따른 기준 및 지표의 내용 타당도를 측정하기 위한 적합도 점수는 표준에 따른 기준 33개의 경우는 평균 2.82로, 기준에 따른 지표 133개의 경우도 평균 2.82로 나타나 비교적 도구의 내용타당도 점수가 높은 결과를 보였다(3점: 매우 적합하다, 2점: 비교적 적합하다, 1점: 적합하지 않다). b. 개발된 근·골격장애 환자의 간호질 평가 도구의 33개 기준에 대한 3명의 평가자간의 일치도를 보기 위해 pearson correlation coefficient를 본 결과 r= .7506, r= .8934, r= .6695로 나타났으며, 각 대상자가 받은 간호수행 결과에 대한 세 평가자의 평가자료를 가지고 평가자의 영향을 분석하는 단일국면 교차설계(single-facet crossed design)를 이용하여 평가자간 상관계수를 측정한 결과 r= 7.464로 나타났다. c. 개발된 도구의 기준 33개중 모두에게서 3점으로 평가된 "기준 1-5", "기준 1-6"과 0점으로 평가된 "기준 5-6"을 제외한 30개 기준의 내적 일관성 신뢰도는 문항분석 결과 alpha 계수는 .8524로 나타났다. d. 신뢰도, 타당도 검증 대상자의 일반적 특성에 따른 질평가 점수는 교육정도(F=2.93 p= .029)와 진단분류(F=2.50 p= .042)에 따라 통계적으로 유의한 차이가 있는 것으로 나타났다. 본 연구를 통해 연구자는 다양한 방법의 신뢰도, 타당도 검사를 통해 신뢰도, 타당도가 높은 근·골격장애 환자의 간호질 평가 도구를 개발함으로써 보다 간호의 질을 향상시키고 효과적인 간호중재의 방향을 제시해 줄 수 있다고 사료된다.
