임종환자 가족이 지각하는 죽음과 관련된 문제

Abstract

This study is intended to present the basic data so that nurses can perform the efficient nursing intervention to one's family as well as a dying client by understanding their problems facing in the death of the family member. As a sample group, 28 persons of client's families that clients were diagnosed dead within 6 months, were randomly selected in one hospital attached to the university, and one general hospital in D city and one in K city. The period of data collection is 51 days, from february 14, 1992 to april 5, 1992. The client's family is interviewed by unstructured open questions and tape-recorded voluntarily. Data was analyzed using van Kaam's phenomenology method. 189 statemants collected are classified with 34 concepts and 10 themes, and finally with 3 categories. In this process, reliability and validity of the categorized content were tested by 5 doctoral students and a master's student majoring in nursing, and reliability coefficients are respectively .96 and .95. The results of this study represent as follows: 1. The problems related with the death that a dying patient's family perceives represent to be hopelessness, powerlessness, vexation, grief, loss, guilty conscience, lonelyness, burnout, shrinkage, confusion, anxiety, fear, uneasiness, suffocating headache, anorexia, general fatigue, and insomia. Again, the results are categorized with the emotional and the physical problems, and then the theme categorizes with the intra-family problems.. 2. The problems related with the death that a dying patient's family perceives represent to be aches in the pain, concern of medical team, and family support in the comport and support, not to give a dying client with hope feelings of hopelessness in a dying notice, lack of explanation on the client's conditions, and the clinical labaratory. Again, the results are categorized with inter-personal relationship patient's pain, comport and support, role change, a dying notice, a lack of information, and a marriage problems. 3. The problems related with the death that a dying patient's family perceives involve not to do their best efforts due to a short medical cost and family expenditures funeral severce and dying away from home. The results are categorize with economic problems and the funeral service problems, and the theme are targeted with problems of family member. The studies so far regarded the family as a support source, but rarely studied on the problems that the family faced in a patient-oriented nursing care. As shown in this study, however, we are able to know that a death of the family member give rise to several problems to the other family member as well as the dying oneself. These problems influence the dying client due to the interaction among family members, and consequently press the client for death because he or she feels a heavy burden. Nursers therefore should provide an efficient nursing intervention considering problems - that is, the emotional and the physical problems in the intra-family, a patient's pain, comfort and support, role change, a dying notice, a lack of informations, and marriage problems in the inter-personal relationship of family mamber, and the economical and a funeral service problem beyond family member - which family of a dying client perceives.;본 연구는 임종환자를 간호하고 있는 가족들이 한 가족원의 임종으로 인하여 경험하고 있는 문제를 파악하여 임종환자 뿐만 아니라 그 가족에게도 효율적인 간호중재를 할 수 있도록 하기 위한 기초자료를 제시하고자 시도되었다. 연구대상은 임의선정된 D시의 1개 대학부속병원과 1개 종합병원, K시의 1개 종합병원에 입원하고 있으며, 의사로부터 6개월이내에 사망할 것이라는 진단을 받은 환자가족 중 직접 환자를 돌보는 가족원 28명이었다. 자료수집기간은 1992년 3월 14일부터 4월 5일까지 총 51일간이었다. 자료수집방법은 비구조적인 개방식 질문으로 연구자가 직접 면담하였으며, 대상자의 양해하에 녹음 수록하고 나중에 그 내용을 있는 그대로 기술하였다. 이렇게 기술된 자료는 van Kaam의 현상학적 연구방법을 이용하여 분석하였다. 이 과정에서 범주화된 내용의 신뢰도와 타당도는 간호학과 석사과정생 1인과 간호학과 박사과정생 5인에 의해 검정되었으며, 신뢰계수는 각각 0.96, 0.95이었다. 본 연구를 통해 얻어진 연구결과는 다음과 같다. 수집원 189개의 진술에서 34개의 개념과 10개의 주제로 분류하였으며 최종적으로 3범주로 분류하였다. 1. 임종환자 가족이 지각하는 죽음과 관련된 문제로서, 절망감,무력감,안타까움,슬픔,상실감,죄책감,외로움,지침,위축,혼돈,염려,두려움,답답함,불안감,두통,전신통,식욕부진,힘이없음,피로감, 불면증 등의 개념으로 이는 다시 정서적 문제, 신체적 문제의 주제로 그 주제는 가족원내 문제로 범주화 하였다. 2. 임종환자 가족이 지각하는 죽음과 관련된 문제로서, 동통, 의료인의 관심,가족지지,역할변화,책임증가, 희망을 가진 환자에게 절망을 줄수없음,환자상태에 대한 설명 부족,검사설명 부족 등의 개념으로 이는 다시 환자의 고통,위로와 지지,역할변화,임종통고,정보제공 부족,자녀결혼 문제등의 주제로 그 주제는 가족원 대인관계 문제로 범주화 하였다. 3. 임종환자 가족이 지각하는 죽음과 관련된 문제로서, 의료비의 부족으로 최선의 노력을 다하지 못함,입원으로 인한 가족 경비,장례의식,죽음을 맞는 장소 등의 개념으로 이는 다시 경제적인 문제,장례식 문제의 주제로 그 주제는 가족원의 문제로 범주화 하였다. 지금까지는 환자중심의 간호로서 가족을 단순히 환자에 대한 지지원으로만 보았으며,가족이 가질 수 있는 문제에 대해서는 연구가 거의 이루어지지 않았다. 그러나 이상의 연구결과에서와 같이 한 가족원의 죽음은 그 개인의 문제뿐만 아니라 그 가족전체의 문제로서 다른 가족원에게도 여러 가지 문제를 야기시킬 수 있다는 것을 알 수 있다. 또한 이와 같은 문제들은 가족들간의 상호작용으로 환자에게 영향을 주어 환자 자신이 가족들에게 많은 부담감을 끼친다는 느낌으로 즉음을 더 재촉할 수 있는 문제가 대두될 수 있다. 따라서 간호사는 환자 뿐만 아니라 그 가족도 대상자로 보아 임종환자 가족이 가진 가족원내 문제로 정서적 문제,신체적 문제,가족원 대인관계 문제로 환자 고통,위로와 지지,역할변화,임종통고.정보제공 부족,자녀결혼 문제 그리고 가족원외 문제로 경제적인 문제,장례식 문제등에 대하여 효율적인 간호중재를 제공해야 할것이다.
