Abstract
Glomerular mesangial cells are the main component of human glomerulus, and exist as a quiescent state in normal healthy kidney. It is now apparent that mesangial cells do not act as a mere framework within the glomerular tuft, but that these cells can undergo striking phenotypic transformation after an exposure to a variety of stimuli including immune complex, cytokine and mitogens such as platelet derived growth factor (PDGF) and epidermal growth factor (EGF). Proliferation of mesangial cells is a prominent histologic feature of various glomerular diseases in human as well as in experimental animal. Because glomerular mesangial cells have direct relation with glomerular function through their contractility and proliferation, the inhibitory effects on cell proliferation can result in preventing or delaying the progression of renal disease into end stage renal failure. Heparin is a highly sulfated glycosaminoglycan with anticoagulant activity. It has also been shown that heparin exhibits antiproliferactive effects in cultured vascular smooth muscle cells and can ameliorate glomerular injury in some animal model. The present study was undertaken to characterize the inhibitory effects of heparin on PDGF- and EGF-induced proliferation of mesangial cells isolated from male Sprague-Dawley rat in vitro. Also to examine whether this antiproliferative effects of heparin is related with anticoagulant activity, the effect on the PEGF-induced mesangial cell proliferation was assessed using N-desulfated and N-desulfated N-acetylated heparin which showed no anticoagulant effects. The result were as follows: 1) PDGF simulated the DNA sythesis of glomerular mesangial cell assessed by `^3 H-thymidine uptake in a dose-dependent manner. At a concentration of 10ng/ml, `^3 H-thmidine uptake was significantly increased compared to the control value (512.0±38.6 vs. 3300.0±432.5 cpm, p<0.05). 2) EGF also induced a significant increase of DNA synthesis of cultured mesangial cell, and at a concentration of 5 ng/ml or higher, the increases in DNA synthesis assessed by `^3 H-thymidine uptake were statistically significant compared to the control value (428.0±50.4 vs. 1618.0±219.7 cpm, p<0.05). 3) Intact heparin caused a dose-dependent inhibition of PEGF-induced DNA synthesis of mesangial cells. At a concentration of 1μg/ml of heparin, 36.7% of inhibition in DNA synthesis of mesangial cell was found. At a concentration of 25, 100 and 500μg/ml of heparin, PDGF-induced `^3 H-thymidine uptake of mesangial cell was reduced by 44.4 %, 56.2% and 69.8%, respectively. Chondroitin sulfate and N-desulfated N-acetylated heparin also caused a significant inhibition of PDGF-induced DNA synthesis at the concentration of 25μg/ml or higher. However, N-desulfated heparin showed no significant inhibitory effects on PDGF-induced proliferation of mesangial cell at all concentrations. 4) Intact heparin, N-desulfated N-acetylated heparin and chondroitin sulfated caused a significant inhibition of EGF-induced DNA synthesis only at the highest concentration of 500 μg/ml. 5) Cell count assessment by hemocytometer revealed the similar results with `^3 H-thymidine uptake. In conclusion, heparin had an inhibitory effect on the PEGF-induced DNA synthesis and proliferation of mesangial cell. These antiproliferative effects appear to be unrelated to the anticoagulant effects of heparin, and mediated by selective inhibition of protein kinase C-dependent pathway of mesangial cell proliferation. Further studies utilizing in-vivo glomerulonephritis model are warranted to clarify the potential benefit of heparin in the treatment of glomerulonephritis and to document expected effects of heparin in delaying the progression of glomerulonephritis into end stage renal failure. ; 말기 신부전증으로 투석요법이 필요한 환자의 수가 매년 증가됨에 따라 주된 원인질환인 만성사구체 신염과 당뇨병성신증의 병태생리학적 기전 및 병리 소견 등에 관한 관심이 증가되고 있다. 사구체를 구성하는 주된 세포인 메산지움세포의 증식은 대부분의 신장질환에서 관찰되는 공통적인 소견으로서 이들 세포의 증식과 세포외기질의 합성증가를 억제할 수 있을 경우 사구체 신염과 사구체 경화증을 치료 및 예방할수 있을 것이라는 전제하에 세포 증식억제 효과를 나타내는 인자에 관한 연구가 계속 되고 있다. 본 연구는 정상백서에서 추출한 사구체 메산지움세포에서 각기 다른 기전에 의해 세포증식을 유도하는 것으로 알려진 Platelet derived growth factor (이하 PDGF)와 epidernal growth factor(이하 EGF)로 세포증식을 유도하고, 이들 메산지움세포의 증식이 항응고 효과유무에 따른 여러 종류의 heparin에 의해 어떻게 영향을 받는지 그리고 메산지움 세포의 증식억제효과가 있다면 이러한 효과는 heparin이외의 다른 glycosaminoglycan인 chondroitin sulfate에 의해서도 나타나는지의 여부를 조사하여 heparin의 작용기전과 그 양상을 정확히 규명함으로서 향후 사구체 신염에 관한 생체내 연구의 시행과 치료의 기본 자료를 얻고자 시행되었다. 연구방법은 웅성 Sprague-Dawley rat (200-250 gm)의 사구체 메산지움 세포를 분리, 배양하여 PDGF와 EGF로 세포증식을 유도하고 intact heparin과 항응고 효과가 결여된 N-desulfated heparin 및 N-desulfated N-acetylated heparin의 증식 억제효과를 `^3 H-thymidine uptake를 측정하여 조사하였으며 아울러 hemocytometer를 이용한 viable cell의 수를 직접 조사하여 다음과 같은 결과를 얻었다. 1. PDGF 1, 5, 10, 25 및 50 ng/ml의 농도에서 사구체 메산지움 세포의 `^3 H-thymidine uptake는 각각 696.0±26.6, 1682.5±178.8, 3300.0±432.5, 3382.5±478.4 및 3641.0±244.0 cpm으로 10 ng/ml이상의 농도에서는 대조군 (512.0±38.6 cpm)에 의해 통계학적으로 의의있게 증가되었다. 2. EGF 1, 5, 25, 100 및 250 ng/ml의 농도에서 `^3 H-thymidine uptake는 각각 1124.5±361.5, 1618.0±219.7, 1734.0±300.5, 1855.7±207.8, 2084.8±211.5 cpm 으로 5ng/ml이상의 농도에서는 대조군 (428.0±50.4 cpm)에 비해 통계학적으로 의의있게 증가되었다. 3. Intact heparin은 PDGF 10 ng/ml에 의해 유도된 사구체 메산지움 세포의 증식을 용량 비례적으로 억제하였다. 1 μg/ml의 heparin은 PDGF에 의한 사구체 메산지움 세포의 `^3 H-thymidine uptake를 36.5±5.7 % 감소시켰고, 5, 25, 100 및 500 μg/ml의 heparin농도에서는 각각 37.7±6.5, 44.4±9.8, 56.2±8.0 및 69.8±5.1 % 감소시켰다. Chondroitin sulfate와 N-desulfated N-acetylated heparin은 25μg/ml이상의 농도에서 PDGF의 의해 유도된 사구체 메산지움 세포의 증식을 의의있게 감소시켰으나, N-desulfated heparin은 1 μg/ml 와 500 μg/ml 사이의 농도에서 통계학적으로 의의있는 변화를 보이지 않았다. 4. Intact heparin, N-desulfated N-acetylated heparin과 chondroitim sulfated는 500 μg/ml의 고농도에서만 EGF에 의한 세포증식을 의의있게 억제하였다. 5. Hemocytometer에 의한 세포수의 산정도 유사한 결과를 나타내었다. Intact heparin은 PDGF에 의해 유도된 사구체 메산지움세포의 증식을 5μg/ml의 농도에서부터 의의있게 감소시켰으나 EGF에 의한 세포수의 증가는 intact heparin 100μg/ml이상의 농도에서만 의의있게 감소되었다. 이상의 결과로 heparin은 PDGF에 의해 유도된 사구체 메산지움 세포의 증식을 의의있게 억제시킴을 확인할 수 있었으며 항응고 효과가 없는 N-desulfated N-acetylated heparin로 메산지움 세포의 의의 있는 증식억제효과를 나타낸 것으로 미루어 보아, 항증식 효과는 heparin분자의 항응고 효과와는 무관하고 N-terminal의 구조에 의해 영향 받는 것으로 생각된다. Chondrotin sulfate도 의의 있는 메산지움 세포 증식억제 작용을 나타낸 것은 heparin의 항증식 효과가 heparin고유의 작용이라기 보다는 다른 glycosaminoglycan도 공유하는 특성일 가능성을 시사하는 것으로 이에 대해서는 계속적인 연구가 필요할 것으로 생각된다. 대부분의 만성 신장질환의 공통적인 소견인 사구체 메산지움세포의 증식에 관한 heparin의 이러한 억제효과에 관해서는 사구체신염 동물 모델과 환자를 대상으로 한 전향적인 연구가 요구되며 이는 사구체 신염, 당뇨병성신증등으로 인한 말기 신부전증으로의 진행예방과 치료에 기여할 수 있을 것으로 생각된다.
