- Helicobacter pylori (H. pylori) 감염에서 조절T세포(regulatory T cells, Tregs)와 조절 사이토카인은 숙주의 면역반응을 억제하여 균이 제거되지 않고 감염이 지속되도록 하며, 위 점막의 만성 염증을 야기한다. 반면, T helper 17 (Th17) 세포는 친염증 사이토카인 interleukin (IL)-17을 분비하여 염증 반응을 일으키고 균을 제거하는 역할을 한다. 또한 Smad7은 조절 사이토카인 transforming growth factor-β(TGF-β)의 신호전달을 차단하여 위염의 정도에 영향을 미친다.
이에 본 연구에서는 소아 H. pylori 위염에서 조절 면역반응의 표지자인 forkhead box protein 3 (FOXP3), TGF-β1, IL-10 발현, 이와 상반된 작용을 하는 IL-17A 발현, Th17세포/FOXP3+Tregs 균형관계, Smad7 발현을 조사하여 위점막 면역반응을 연구하였다.
2009년 2월부터 2010년 11월까지 이화여자대학교 목동병원 소아청소년과에서 상부위장관 내시경을 시행한 환자 중, H. pylori(+) 위염군 20례(2-16세)와 정상 대조군 20례(3-16세)에서 보관된 날문방 점막조직으로 후향적 연구를 시행하였다. 병리조직의 판독은 개정된 시드니 분류에 따랐다. FOXP3, TGF-β1, IL-10, Smad7의 유전자 발현은 정량적 실시간 중합효소연쇄반응을 시행하였으며, FOXP3+Tregs와 Th17세포는 면역조직화학염색을 시행하여 세포 수를 계산하여 분석하였다.
H. pylori 밀집도, 염증의 활동성을 나타내는 중성구 침윤, 염증의 만성도를 나타내는 단핵세포 침윤 정도는 H. pylori(+) 위염군에서 정상 대조군에 비하여 유의하게 높았다(P<0.01). H. pylori(+) 위염군에서 FOXP3, TGF-β1, IL-10 mRNA 발현은 정상 대조군에 비해 유의하게 높았으나(P<0.01), Smad7 mRNA 발현은 유의한 차이가 없었다. H. pylori(+) 위염군에서 FOXP3+Tregs 수는 정상 대조군에 비해 유의하게 높았다(P<0.01). H. pylori(+) 위염군에서 FOXP3 mRNA 발현은 TGF-β1, IL-10 mRNA 발현과 유의한 양의 상관성이 있었고, 균의 밀집도, 중성구, 단핵세포 침윤 정도와 유의한 양의 상관성이 있었다(P<0.05). H. pylori(+) 위염군에서 Th17세포 수는 정상 대조군에 비해 유의하게 높았다(P<0.05). H. pylori(+) 위염군에서 Th17세포 수는 H. pylori 밀집도와 유의한 음의 상관성이 있었고, 중성구, 단핵세포 침윤 정도와 유의한 양의 상관성이 있었다(P<0.05). H. pylori(+) 위염군에서 Th17세포와 FOXP3+Tregs의 비는 H. pylori 밀집도와 유의한 음의 상관성이 있었으나(P<0.05), 중성구 및 단핵세포 침윤 정도와는 유의한 상관성은 없었다.
H. pylori 위염에서 위점막 조절T세포 및 조절 사이토카인은 상호작용을 하며 함께 발현이 증가함으로써, 균의 면역 회피 기전에 기여하였다. 또한 Th17세포의 반응 역시 높게 나타났으며, 이는 균 제거를 유도하기 위함으로 해석된다. Th17세포/FOXP3+Tregs 비가 낮을수록 H. pylori 밀집도가 높아지는 것으로 보아, 상반된 두 면역반응의 균형관계에서 조절 면역반응이 우세할수록 균이 제거되지 않고 만성 위염을 일으키는데 중요한 역할을 할 것으로 추정할 수 있다. 향후 H. pylori 감염이 지속되는 기전에 있어서 숙주의 점막 면역반응에 대한 더 많은 연구가 이루어져야 할 것이다.
;Purpose: FOXP3+regulatory T cells (Tregs) and immunoregulatory cytokines such as TGF-β, IL-10 downregulate immune responses, which may contribute to the persistence of bacteria in Helicobacter pylori (HP) infection. Whereas, T helper 17 (Th17) cells induce proinflammatory responses in clearing pathogens. And, Smad7, an inhibitory regulator of TGF-β signaling may affect the HP gastritis. The aim of this study was to examine the expression of FOXP3, IL-10, TGF-β1, Smad7, IL-17A and Th17 cells/FOXP3+Tregs balance in the gastric mucosa of children with HP infection, in relation to the gastric histopathology.
Method: Antral mucosal biopsies were obtained from 20 children with HP(+) gastritis (2-16 years of age) and 20 age-matched normal control (3-16 years of age). Gastric histopathology was assessed by the updated Sydney classification. Gene expression levels of FOXP3, IL-10, TGF-β1 and Smad7 were analyzed by quantitative real-time polymerase chain reaction. Also, immunohistochemical stains for FOXP3+Tregs and Th17 cells were performed.
Results: Histopathological scores of the bacterial density, the infiltration of polymorphonucelar neutrophils (PMN) and mononuclear inflammatory cells (MNC) were significantly higher in the HP(+) gastritis group compared to the control group (P<0.01). The gene expression levels of FOXP3, TGF-β1 and IL-10 mRNA in the HP(+) gastritis group were significantly higher than the control group (P<0.01). The levels of Smad7 mRNA were not significantly different from each other. The number of FOXP3+Tregs was also significantly higher in the HP(+) gastritis group compared to the control group (P<0.01). FOXP3 mRNA levels correlated positively with TGF-β1, IL-10 mRNA levels in the HP(+) gastritis group (P<0.05). Furthermore, FOXP3 mRNA levels correlated positively with the bacterial density and the infiltration of PMN, MNC in the HP(+) gastritis group (P<0.05). The number of Th17 cells was significantly higher in the HP(+) gastritis group compared to the control group (P<0.05). And, the number of Th17 cells correlated negatively with the bacterial density, and correlated positively with inflammatory scores of PMN, MNC in the HP(+) gastritis group (P<0.05). A negative correlation between Th17 cells/FOXP3+Tregs ratio and the bacterial density was demonstrated in the HP(+) gastritis group (P<0.05).
Conclusion: This study suggested that increased infiltration of Tregs and immunoregulatory cytokines into the gastric mucosa suppress the immune responses during HP infection. Thereby, Th17/Tregs balance toward a Tregs-biased response favors the persistence of bacteria which causes a chronic active gastritis. Further large scale researches are needed, in order to address the mechanisms of immune reactions in HP infection of children.
