- Abstract
- 배경: 이전에 발표된 다제 내성 결핵 중 초회 내성 환자들이 치료 실패가 많았던 점에 대한 기전을 알아 보고자 한다.
목적: 초회 내성 환자의 단핵구내의 STAT-1, STAT-4의 인산화 및 사이토카인의 분비가 감소됨을 됨을 증명하여 초회 내성을 갖는 내성 결핵 환자들에게서 치료 실패와 관련하여 IL-12 연관 IFN-γ 경로의 이상이 연관됨을 보고자 한다.
연구 방법: 10명의 완치된 약제 감수성 결핵 환자와 12명의 획득 내성 환자, 그리고 10명의 초회 내성 환자들을 연구에 포함시켰다. 환자들의 말초 혈액에서 단핵구를 분리하여 24시간 동안 IFN-γ , IL-12, PPD, 그리고 결핵균 분리물로 자극하고, 유세포 분석기(flow cytometric analysis)를 이용하여 인산화 형태의 STAT-1과 STAT-4을 각각 측정하고, 배양액의 상층액에서는 IFN-γ, IL-12p40, 그리고 TNF-α 분비를 정량적으로 측정하였다.
결과: 인산화 형태의 STAT-1, STAT-4의 형광도 세기의 중앙값 (mean fluorescent intensitie ; MFI)이 세 자극원 모두 초회 내성 환자군 에서 의미 있게 감소됨을 관찰하였다. 또한, 여러 가지 자극원으로 자극한 후 배양된 배지의 상층액에서 정량적으로 측정한 INF-γ, IL-2p40, 그리고 TNF-α도 초회 내성 환자군 에서 감소됨을 관찰하였다.
결론: 초회 내성 환자들은 단핵구 내 인산화 형태의 STAT-1, STAT-4의 발현 및 사이토카인의 분비의 감소가 관찰되며, 이러한 결과는 초회 내성 환자에서 IL-12 연관 IFN-γ 경로의 이상이 연관되어 있음을 보여 준다.;Introduction: I recently showed that treatment failure rate was higher among multi-drug resistant (MDR) tuberculosis (TB) patients without a previous history of TB treatment, or so-called 'primary resistance'. I investigated the phosphorylation levels of signal transducers and activator of transcription (STAT-1) and STAT-4 and the subsequent cytokine release as a possible cause of a poor prognosis in MDR-TB patients with primary resistance.
Methods: Ten patients with successfully treated pulmonary TB without any resistance, 12 MDR-TB patients with acquired resistance, and 10 MDR-TB patients with primary resistance were enrolled. After 24 hour stimulation of peripheral blood mononuclear cells (PBMC) with interferon-gamma (IFN-γ), interleukin-12(IL-12), purified protein derivative of tuberculin (PPD), or lysate of M. tuberculosis, flow cytometric analysis of intracellular pSTAT-1 and pSTAT-4 were performed and secretion of IFN-γ, IL-12p40, tumor necrosis factor-alpha (TNF-α) were measured in culture supernatant.
Results: The mean fluorescent intensities of pSTAT-1 and pSTAT-4 in PBMC of MDR-TB patients with primary resistance decreased on stimulation of IFN-γ, PPD or lysate of M. tuberculosis when compared with patients with acquired resistance. In addition, secretion of IFN-γ, IL-12p40, TNF-α in these patients decreased on various stimuli.
Conclusion: Decreased phosphorylation of STAT-1, STAT-4, and of subsequent cytokine release are associated with the dysfunction of IL-12 dependent IFN-γ pathway in the primary MDR-TB.
