Immune response in infants of the heptavalent pneumococcal conjugate vaccine against vaccine-related serotypes 6A and 19A

Abstract

현재 사용되고 있는 7가 폐구균 단백 결합백신(PCV7)은 백신에 포함된 혈청형에 대해 좋은 면역반응을 유도하는 효과적인 백신이다. 한편, 백신 관련 혈청형에 대한 면역반응은 혈청에 따라 다양하게 나타난다. 저자는 백신 혈청형 및 백신 관련 혈청형에 대한 면역반응을 확인하기 위해 PCV7을 2, 4, 6개월에 스케줄대로 접종 받은 영아 31명을 대상으로 마지막 접종 후 1개월 되는 시점에 혈액검사를 시행하였다. 효소면역 검사법 (enzyme-linked immunosorbent assay, ELISA)로 8개 혈청형 (백신에 포함된 7개 혈청형 및 백신 관련 혈청형 19A)에 대하여 anti-Pn IgG 검사를 시행하였고, 11개 혈청형 (백신 포함된 7개 혈청형, 백신 관련 혈청형 19A, 6A 그리고 백신에 포함되지 않은 혈청형 5와 7F)에 대하여 multiplexed opsonization assay를 시행하였다. ELISA에서 항체 농도는 8 개 혈청형에 대하여 1.84-10.49 μg/mL로 측정되었고, 19A를 포함하여 8개 혈청형 모두에 대해 0.35 μg/mL 이상의 농도를 보였다. 한편, opsonic index는 7개 백신에 포함된 혈청형에 대해 모두에서 ≥8 이상인 것으로 나타난 반면, VRT인 6A에 대해서 81% 그리고 19A에 대해서 19%만이 ≥8 이상의 값을 보였다. 그리하여 PCV7은 기초 접종 후에 영아에서 백신에 포함된 혈청형에 대해 좋은 면역반응을 보이나, 혈청형 19A에 대해서는 옵소닌 작용이 있는 항체를 유도하지 못하는 것으로 확인하였다. ELISA는 영아에서 백신 관련 혈청형에 대한 면역 평가를 하는데 있어 제한점이 있음을 확인하였다.;The currently available 7-valent pneumococcal conjugate vaccine (PCV7) elicits good immune response to and is effective against vaccine-type serotypes. However its effectiveness against vaccine-related serotypes is variable. Serum samples were obtained 1 month after the last vaccination from 31 infants immunized with PCV7 at 2, 4 and 6 months of age. The sera were used to determine IgG antibody levels to 8 serotypes (7 vaccine-type serotypes and one vaccine-related serotype 19A) with ELISA and opsonic capacity against 11 serotypes (7 vaccine-type serotypes, two vaccine-related serotypes 19A and 6A, and non-vaccine serotypes 5 and 7F) using a multiplexed opsonization assay. Antibody concentrations varied between 1.84 and 10.49 μg/mL and all subjects had an antibody concentration ≥0.35 μg/mL for all serotypes including serotype 19A. In contrast, opsonic index was detectable (i.e., opsonic index ≥8) in all children for the 7 vaccine-type serotypes, 81% for serotype 6A, and merely 19% for serotype 19A. PCV7 shows good immunogenicity for vaccine-type serotypes in infants after a primary series. Although PCV7 elicits opsonic antibodies to serotype 6A, it does not elicit opsonic antibodies to serotype 19A. ELISA may be an inadequate surrogate assay for evaluating the response for cross-reactive serotypes in infants.
