Abstract
근골격계 질환이 전체 업무상질병에서 차지하는 비중은 매년 증가 추세에 있다. 근골격계 질환은 생산성 저하뿐 아니라 노사갈등의 원인이 되기도 하여 사회적 관심도 큰 질환이다. 작업관련성 근골격계 질환 예방 및 관리를 효율적으로 하려면 근골격계 증상 호소의 규모 및 관련 위험요인을 파악하는 것이 필요하다. 따라서 본 연구에서는 우리나라 전체 근로자들의 작업관련성 근골격계 질환 증상의 유병률을 파악하고 관련위험요인을 파악하고자 하였으며, 2003년도 한국산업안전공단에서 시행한 근로자 건강실태조사 자료를 이용하였다. 전국 5인 이상 사업장 근로자를 대표할 수 있는 표본 근로자를 대상으로 하여 작업관련성 근골격계 증상이 있는 근로자를 환자군으로 하고, 건강한 근로자와 작업과 관련 없는 일반 근골격계 증상 근로자를 대조군으로 하여 다중 로지스틱 회귀분석으로 작업관련성 근골격계 질환과 관련된 위험요인을 파악하였다. 주요 연구결과는 다음과 같다. 1. 전체 근로자의 10.4%에서 작업관련성 근골격계 증상을 호소하였다. 제조업 근로자(11.0%)에 비해 비제조업 근로자(9.6%)에서 약간 낮은 편이었다. 남성에 비해 여성에서 증상 유병률이 높았다. 제조업 근로자에서 남성은 40대에서 가장 높고 50대에서 감소경향을 보인 반면, 여성은 50대와 30대에서 높은 것으로 나타났다. 비제조업에서는 남성은 50대와 30대, 여성은 10대와 50대 순으로 높은 유병률을 보였다. 2. 작업관련성 근골격계 증상 유병률을 사업장 규모별로 살펴보면, 제조업에서는 500인 이상의 대규모 사업장(5.2%)과 100-299인 규모의 사업장(3.0%)에서 높은 유병률을 나타냈다. 남성의 경우 사업장 규모가 커질수록 유병률이 높아지는 경향을 보였으며 여성의 경우 500인 이상(6.6%)의 사업장이 가장 높고 30인 미만의 사업장(1.2%)이 가장 낮았으며, 30-49인, 50-99인, 100-299인 규모 사업장 간의 차이는 거의 없었다. 비제조업 분야에서는 300-499인(3.8%), 100-299인(3.0%), 500인 이상(2.6%) 사업장 규모 순으로 높았다. 남성은 300-499인(3.4%), 100-299인(2.7%), 50-99인(2.4%) 사업장 규모 순이었고, 여성은 300-499인(4.8%), 100-299인(3.6%), 500인 이상(3.5%) 사업장 순으로 높은 유병률을 나타냈다. 3. 작업관련성 근골격계 증상에 관련 있는 요인을 보기위해 단변량 분석에서 통계적으로 유의하게 나온 변수들의 효과를 다른 변수들의 효과를 통제하고 다중 로지스틱 회귀분석을 실시한 결과, 인구학적 요인에서는 근로자의 성, 소득수준이, 개인건강행태 요인에서는 자기인식 건강상태, 흡연, 음주, 스트레스가, 직업요인에서는 직업분류, 근무시간, 근로자의 직급이, 작업환경요인에서는 작업강도, 작업속도, 작업자세, 반복작업 여부, 작업장 환경이, 사업장 요인에서는 업종, 근로자 수를 기준으로 한 사업장 규모, 산재・공상자 수, 산업보건에 관한 사업주의 관심도 등이 유의한 변수로 선정되었다. 작업관련성 근골격계 증상 유병률은 제조업에서는 기타 운송장비 제조업(4.9%), 통신장비 제조업(4.2%), 조립금속제품 제조업(4.2%) 순이고, 비제조업에서는 숙박 및 음식점업(4.8%), 오락문화 및 운동관련 서비스업(4.4%), 금융 및 보험업(3.7%) 순으로 높았다. 이 연구는 전국 5인 이상 사업장 근로자를 대표할 수 있는 표본 근로자 12,989명을 대상으로 연구가 이루어졌다는 측면과 설문 응답률이 높아 집단에 대한 대표성이 높은 연구다. 또한 그동안 거의 연구가 이루어지지 않았던 여러 다양한 업종에서 작업관련 근골격계 증상 유병률을 파악하고 위험요인을 분석함으로서 근골격계 질환 예방 정책 수립에 기초자료로 제공할 수 있다는 점에서 연구의 의의를 지닌다.;The incidence of the work-related musculoskeletal disorders (WMSDs) is increasing. WMSDs are a significant cause of decreased productivity and labor conflict. Currently, there is increasing concern about this disease. In order to prevent the development of WMSDs, an understanding of both the scale and the risk factors associated with WMSDs is needed. The aim of this study was to determine both the prevalence and risk factors associated with WMSDs among workers in Korea. The data from the Workers Health Survey carried out by the Korea Occupational Safety and Health Agency in 2003 was used. The samples were chosen from the workers of companies with more than five employees. The cases were those with symptoms of WMSDs and the controls were healthy workers with symptoms not related to work. Multivariate logistic regression analysis was carried out to search for the risk factors associated with WMSDs. 1. 10.4% of all workers complained of symptoms of WMSDs. Non-manufacturing workers(9.6%) experienced fewer symptoms than manufacturing worker(11.0%). Women reported more symptoms of WMSDs than men. Among manufacturing workers, males in their 40's had the most frequent complaints of symptoms but females in their 50’s and 30’s had the highest prevalence of symptoms overall. Among non-manufacturing workers, males in their 50’s and 30’s, and females in their 10’s and 50’s showed a high prevalence of symptoms. The highest prevalence of WMSD symptoms was observed in manufacturing workplaces with more than 500 people (5.2%). 2. Univariate analysis was used to examine the effect of the risk factors while adjusting for the effect of the other variables using logistic regression analysis. The demographic factors of workers were gender and income. The health behavioral factors were the health status, smoking, drinking and the stress. The occupational factors were the grouping of the occupation, working hours and the class of the position. The factors of the working conditions were the intensity of the work, the speed of the work, the working posture, the repetition of the work and the physical workplace environment. According to the number of employees, the industrial factors were the type of industry, the number of employees, the number of accidents and injuries, and the concern regarding the industrial health of the business proprietors. The prevalence of symptoms of WMSDs in the manufacturing industry is ranked in the following order: Other transport equipment (4.9%), Electronic components, radio and communication equipment (4.2%) and Fabricated metal products(4.2%). In the non manufacture field, the prevalence of symptoms of WMSDs is ranked as follows: Hotel and restaurants(4.8%), Recreational, cultural and sporting activities(4.4%), Financial institutions and insurance (3.7%). This study is reliable because the data from 12,989 employees were examined and there was a good response from the questionnaire. An understanding of the prevalence of WMSDs and a proper analysis of the associated the risk factors can provide the basic data to establish a plan for preventing WMSDs.
