Abstract
The purpose of this study is to analyze the effects of a melody therapy program on autism children's such behaviors related to development of their sociability a s playing of instruments, following of instructions, smiling, speaking, homologous behaviors and loitering. For this purpose, monovular twins aged 7 who had been diagnosed a s autism by DSM-IV Diagnosis were sampled to be subject to a 12-session melody therapy for 4 months from February to May, 2003. In Korea, various singing therapies were applied to handicapped children, but no melody therapy has been applied. Thus, the researcher composed a melody based on French Anne-Marie & Ferrand-vidal's and the US Juliette Alvin's theories. The melody therapy was applied at three stages each of which consisted of 4 sessions. At the first stage, aconventional children's song familar to the subjects was used in order to adapt them to the therapy and help them form a friendly relationship with the therapist. Then, in order to facilitate the interaction between them, they were treated in group by themusic therapy for two sessions. At the second stage, the melody therapy was applied on a full scale, while some instrumental activities were properly inserted. At the last stage, in order to assess the effects of the melody therapy, a melody containing some instructions was presented to the subjects, while the words used at the second stage were presented repeatedly to encourage the subjects to imitate them. The third stage was divided into former and latter parts, and then, video recordings were analyzed to observe subjects' overall behavioral changes and thereupon, graphed each of the changes. On the other hand, in order to observe subjects' behavioral changes under other circumstances than the melody therapy, subjects' parents were interviewed to check their behavioral changes in their ordinary life. As a result of applying the melody therapy to the two autism children, it was found that the therapy did not affect theirinstrument playing, but had some effects on their following of instructions, smiling and speaking. On the other hand, although not consistent, their loitering and homologous behaviors were reduced at the last stage of the therapy. Despite the two subjects moved during the therapy and the period of the therapy was short, this study may well be significant in that it found that the melody therapy had some positive effects on autism children's following of instructions, smiling and speaking as well as their loitering and homologous behaviors, and that such a therapy was applied first in Korea.;본 연구는 멜로디 치료 프로그램이 자폐아동의 사회성과 언어 발달에 관련된 행동, 즉 악기연주, 지시 따르기, 미소, 발성, 상동행동, 배회행동에 어떤 영향을 미치는 가를 알아 본 것이다. 본 연구는 DSM-IV 진단 기준에 의해 자폐로 판정 받은 7세의 일란성 쌍생아, 남아 2명을 대상으로 2003년 2월부터 2003년 5월까지 4개월에 걸쳐 12회기 동안 진행되었다. 우리나라에서 노래 부르기를 통한 여러 가지 음악치료가 시행된 바 있으나 멜로디 치료의 전례는 전무하여 프랑스의 Anne-Marie, Ferrand-vidal과 미국의 Juliette Alvin의 이론적 배경을 중심으로 멜로디를 직접 작곡하여 사용하였다. 본 연구는 3단계로 나누어 각 단계는 각각 4회기씩 진행되었다. 제 1단계에서는 내담자가 음악치료에 적응하고 음악치료사와 관계를 형성할 수 있도록 내담자에게 익숙한 기존의 동요를 사용하였고, 내담자간의 상호작용을 위해 처음 2회기 동안 2명을 그룹으로 음악치료를 진행하였다. 제 2단계에서는 멜로디 치료가 본격적으로 시행되었으며 이 시기에는 가급적 악기활동을 피하고 멜로디 치료를 우선시 하였으나 적절한 악기활동을 삽입하였다. 마지막 3단계에서는 멜로디 치료를 평가하기 위해 내담자에게 지시어를 사용한 멜로디를 제공하고 내담자의 언어적 모방을 위해 멜로디 치료에서 사용된 가사를 언어로 반복하였다. 3단계를 각각 전기와 후기로 나누어 비디오 분석을 통하여 내담자의 전반적인 행동변화를 서술하고 각각의 행동변화 결과를 그래프로 정리하였다. 한편 멜로디 치료 외의 상황에서의 변화를 위해 부모와의 면담을 통하여 내담자의 일상생활에서의 행동변화를 서술하였다. 멜로디치료를 2명의 자폐아동에게 실시한 결과 내담자의 악기연주에는 영향을 미치지 못하였으며 지시 따르기와 미소, 그리고 발성의 영역에서는 증가된 결과를 나타내었다. 한편 배회행동과 상동행동은 일관성이 부족하지만 첫 회기 때보다 마지막 회기 때에 감소된 결과를 나타내었다. 본 연구는 연구기간 중의 2명의 내담자가 모두 이사를 하는 등의 변인들이 작용되고 연구기간이 짧았다는 제한점에도 불구하고 결론적으로 멜로디 치료가 자폐아동의 지시 따르기와 미소, 발성에 긍정적인 영향을 미치고 배회행동과 상동행동의 감소를 보인 점과 이러한 멜로디 치료가 우리나라에서 처음으로 시도되었다는 데에 의의가 있다.
