Abstract
Joint attention means that the caregiver and the infant pay attention together to the same object or activity, in which the infant intends to share their interests or feelings (Butterworth, 1991; Tomasello, 1995; Todd, 1983; Stern, 1985). In other words, the development of communal attention can be said to be the starting point of the process of social interaction with others using verbal and non-verbal responses. Joint attention formed in parent-infant interaction situations is the result of dynamic mutual cooperation between parents and young children, so it is not determined only by the infant's unique ability. In addition to the presence or absence of disabilities and communication characteristics of infants, the role of parents also plays an important role. Depending on the clues and facilitation parents provide to infants, joint attention between the two can be established more easily, and the duration of joint attention can be lengthened (Bottema‐ Beutel, K., 2016). As discussed above, the classification criteria for the types of cues parents use to establish joint attention in parent-infant interaction differ according to previous studies, and the pattern of parents' use of cues varies 66 according to the listening status of the target infant. Therefore, it is difficult to figure out how mothers use cues and joint attention in the interaction between hearing-impaired infants and young children. In addition, most of the previous studies analyzed the characteristics of joint attention without considering the mother's cue type in the interaction between the mother and the hearing-impaired infant. In order to understand the characteristics of mothers' use of cues and joint attention, mother-infant interaction was evaluated. Various toys were used for interaction, and they were used selectively according to the interests of infants and toddlers. Parents and infants interacted using toys for about 20 minutes in a quiet and independent space. In order to analyze the characteristics of communal attention, the establishment and termination of communal attention were set by referring to the criteria for analysis of communal attention by Tomasello and Farrar (1986). After excluding the first 5 minutes and the second half of the 20-minute mother-infant video, 10 minutes of active interaction were analyzed. Establishment of joint attention was coded with EJA (established joint attention), and after the mother initiated joint attention using cues, (1) the infant made a gesture such as pointing within 3 seconds, or (2) pointed at an object (object). Actions included staring into the mother's gaze, (3) holding the mother's body part, (4) touching an object of interest, (5) waving a hand within the mother's field of vision, and (6) verbally responding. . Failure to establish joint attention was coded as FJA (failed joint attention), and included cases where (1) the infant's response behavior did not occur within 3 seconds of the mother's cue or (2) was not maintained for more than 3 seconds (Bakeman & Adamson, 1984). The summary of the research results is as follows. First, as a result of examining the frequency of joint attention attempts by cue type and group in mother-infant interaction, it was found that the frequency of mothers' attempts in the cochlear implant group was significantly 67 higher than that in the healthy infants group. Second, there was no significant difference between the two groups in the success rate according to the group and cue type in the mother-infant interaction. This means that the cochlear implant success rate and the healthy infant group showed similar patterns in coattention success rates. Third, in mother-infant interaction, the difference between the two groups in the rate of joint attention time was significant, and the rate of joint attention time in the cochlear implant group was found to be significantly lower than that in the healthy infant group. Fourth, the chronological age of infants in the group of cochlear implant infants showed a significant correlation with the success rate of joint attention, time ratio, average duration, receptive language score, and expressive language score. The frequency of joint attention attempts showed no correlation with the receptive language score and expressive language score, and the joint attention success rate, time ratio, and average duration showed a significant correlation with the receptive language score and expressive language score. The chronological age of infants in the healthy infant group showed a significant correlation with the frequency of joint attention attempts, success rate, time rate, average duration, receptive language score, and expressive language score. In addition, the frequency and time ratio of joint attention attempts showed significant correlations with the receptive language score and expressive language score. Through these results, it was confirmed that the mothers of the cochlear implant group used a significantly higher trial frequency than the mothers of the hearing group. There was no significant difference in the coattention success rate between cochlear implant infants and hearing infants, but significant differences were found in the percentage of total interaction time and average duration. The time ratio and average duration of cochlear implant infants were 68 significantly shorter than those of healthy infants, which is thought to be due to the fact that the mothers of the cochlear implant infant group did not closely grasp the communication intentions of their children. Initiation of communal attention using clues was successful, but difficulties in sustaining it for a long time means that it is difficult to create an environment that can promote language development. Through this study, by comparing the common attention characteristics (attempt frequency, success rate) of a group of cochlear implant infants and a group of healthy infants, it was confirmed which type of cue to initiate common attention is effective in eliciting establishment. . In addition, by comparing the temporal characteristics of communal attention between the two groups, the importance of continuing and maintaining communal attention was identified. Find out what kind of relationship they have. Therefore, it is considered that by identifying the characteristics of cochlear attention in the interaction situation of cochlear implant infants and young children in the interaction situation with their mothers, it may have provided important clinical implications to mothers and therapists who deal with hearing-impaired infants and young children.;공동주의는 양육자와 영아가 동일한 사물이나 활동에 함께 주의를 기울이는 것을 의미하며, 이 때 영아는 자신의 관심이나 감정을 공유하려는 의도를 지닌다(Butterworth, 1991; Tomasello, 1995; Todd, 1983; Stern, 1985). 공동주의가 확립되지 않은 상황에서는 상대와 의사소통을 시작하는 것이 어렵고 시작되었다 하더라도 유지되기 어렵기 때문에 공동주의의 발달은 언어적, 비언어적 반응을 이용하여 타인과 사회적 상호작용을 이루는 과정의 출발점이라 할 수 있다(김영태, 2014). 따라서 영아가 타인과 함께 하나의 사물에 주의를 집중하는 행동은 차후 모든 의사소통에서의 필수 요건이며 성공적인 의사소통의 기저를 이룬다고 할 수 있다(Bruner, 1977). 난청 영유아는 건청 부모와 의사소통 모드가 불일치하기 때문에 초기 상호작용에서의 공동주의 확립 양상이 건청 영유아와 다르게 나타날 수 있으며, 이와 관련해 다수의 연구가 진행되었다. 자유놀이 상황에서 난청 및 건청 아동의 공동주의 성공률을 비교한 연구에서는 건청 아동과 건청 어머니 쌍이 난청 아동과 건청 어머니 쌍보다 공동주의 확립에 있어 유의하게 높은 수행률을 보였다(MacGown, 2021). 또한, 공동주의의 개시 주체자를 어머니와 아동으로 나누어 분석한 연구에서는 어머니가 공동주의 주체자인 경우에 건청 아동 집단의 공동주의 성공률이 난청 아동 집단에 비해 유의하게 높게 나타났다(Nowakowski, 2009). 이는 난청 아동이 생애 초기에 겪는 청각 박탈 경험으로 인해 대화 상대자의 말에 적절하게 반응하고 주의를 기울이는 경험이 부족했기 때문으로 해석할 수 있다. 본 연구에서는 생활연령 11~35개월 사이의 인공와우이식 영유아 집단과 연령을 일치시킨 건청 영유아 집단의 어머니-영유아 상호작용을 통해 어머니의 단서 유형에 따른 시도 빈도에 대한 집단 간 차이를 확인하고자 한다. 또한 각 집단에서 나타나는 공동주의 관련 변수(시도 빈도, 성공률, 시간 비율, 평균지속시간)간에 어떤 차이를 가지는지 비교하고자 한다. 이어서, 공동주의 관련 변수와 영유아의 언어능력(수용언어점수, 표현언어점수)간 관련성을 파악하여 공동주의와 언어능력의 상관에 대해 살펴보고자 한다. 본 연구의 집단 간 비교를 통하여 인공와우이식 영유아를 자녀로 둔 어머니의 단서 사용 양상 및 공동주의 특성에 대해 이해할 수 있으며 이로써 난청 영유아와의 상호작용에 유용한 임상적 정보를 제공하고자 한다. 어머니의 단서 사용 및 공동주의 특성을 파악하기 위하여 어머니-영유아 상호작용 평가를 진행하였다. 상호작용 시 다양한 장난감을 활용하였으며 영유아의 흥미에 따라 선택적으로 사용되기도 하였다. 부모와 영유아는 조용하고 독립된 공간에서 약 20분간 장난감을 활용하여 상호작용을 진행하였다. 공동주의 특성을 분석하기 위해 Tomasello & Farrar(1986)의 공동주의 분석 기준을 참고하여 공동주의의 확립 및 종료를 설정하고자 하였다. 총 20분의 어머니-영유아 영상 중 초반 5분과 후반 5분을 제외한 후 상호작용이 활발히 일어난 10분을 분석하였다. 공동주의 확립은 EJA(established joint attention)로 코딩하였다. 공동주의 확립은 (1) 어머니가 공동주의 개시를 위해 단서를 사용한 후 3초 이내에 영유아가 가리키는 등의 제스처를 취하거나, (2) 대상(사물)에 시선을 응시하거나, (3) 어머니 의 신체 부위를 잡거나, (4) 관심 대상(사물)을 만지거나, (5) 어머니의 시야 안에서 손을 흔들거나, (6) 언어로 응답하는 행위를 포함하였다. 공동주의 확립 실패는 FJA(failed joint attention)로 코딩하였다. 공동주의 확립 실패는 (1) 영유아의 응답 행동이 어머니의 단서로부터 3초 이내에 이루어지지 않거나, (2) 3초 이상 유지되지 않는 경우를 포함하였다(Bakeman & Adamson, 1984). 연구 결과를 요약하면 아래와 같다. 첫째, 어머니-영유아 상호작용에서 단서 유형과 집단에 따른 공동주의 시도 빈도를 살펴본 결과, 인공와우이식 영유아 집단에서의 어머니의 시도 빈도가 건청 영유아 집단에서의 어머니의 시도 빈도보다 유의하게 높은 것으로 나타났다. 또한 단서 유형에 따른 주효과가 통계적으로 유의한 것을 확인하였으며, 네 가지의 단서 유형 중 통합적 단서를 사용하여 공동주의를 개시한 시도 빈도가 유의하게 높게 나타났다. 집단과 단서 유형에 따른 이차상호작용이 효과가 유의하였다. 이는 다른 단서에 비해 통합적 단서에서 집단 간에 공동주의 시도 빈도의 차이가 크게 나타난 것이 이차상호작용의 요인으로 해석된다. 둘째, 어머니-영유아 상호작용에서 공동주의 확립 성공률이 두 집단 간 유의한 차이가 나타나지 않았다. 이는 인공와우이식 영유아 집단과 건청 영유아 집단의 공동주의 성공률이 비슷하였음을 의미한다. 단서 유형에 따른 주효과가 유의하였으며, 이를 통해 통합적 단서에 따른 공동주의 성공률이 다른 단서보다 유의하게 높은 것을 확인하였다. 두 집단 간 이차상호작용은 통계적으로 유의하지 않았다. 셋째, 어머니-영유아 상호작용에서 공동주의 시간 비율의 두 집단 간 차이가 유의하였으며, 인공와우이식 영유아 집단의 공동주의 시간 비율이 건청 영유아 집단보다 유의하게 적은 것으로 나타났다. 또한 공동주의 평균지속시간에서 두집단 간에 유의한 차이가 나타났으며, 인공와우이식 영유아 집단의 공동주의 평균 지속시간이 건청 영유아 집단보다 유의하게 짧은 것으로 나타났다. 넷째, 인공와우이식 영유아 집단에서의 영유아의 공동주의 성공률, 시간 비율, 평균지속시간은 수용언어점수, 표현언어점수와 유의상관이 나타났다. 공동주의 시도 빈도는 수용언어점수, 표현언어점수와 상관을 보이지 않는 것으로 나타났다. 건청 영유아 집단에서의 영유아의 공동주의 성공률은 표현언어점수와 유의한 상관을 보였다. 공동주의 시간 비율은 수용언어점수, 표현언어점수와 유의한 상관을 보였으며, 평균지속시간은 수용언어점수와 유의한 상관을 보였다. 공동주의 시도 빈도는 수용언어점수, 표현언어점수와 유의한 상관을 보이는 것으로 나타났다. 이러한 결과를 통해 인공와우이식 영유아 집단의 어머니가 건청 집단의 어머니보다 유의하게 많은 공동주의 시도 빈도를 사용하는 것을 확인하였다. 단서에 따른 두 집단의 공동주의 성공률에 있어 유의한 차이가 나타나지 않은 것을 확인할 수 있는데 이는 생애 초기 청각 박탈을 경험한 인공와우이식 영유아들이 보장구의 도움을 받아 건청 영유아만큼의 수행을 보였음을 의미한다. 보장구의 도움을 받는다고 하여도 인공와우이식 영유아가 건청 영유아만큼의 듣기 능력을 지닌다고 보기는 어려운데 그럼에도 공동주의 성공률에 두 집단 간 차이가 유의하지 않았다는 것은 공동주의집중이 듣기 능력만으로 이루어지는 것이 아닌, 주의조절능력이나 인지능력 등 다양한 영역과 밀접하게 연관된 과제라는 것을 알 수 있다. 인공와우이식 영유아와 건청 영유아의 공동주의 성공률에는 유의한 차이가 나타나지 않았지만 상호작용 전체 시간 당 차지하는 시간 비율과 평균지속시간에서는 유의한 차이가 나타났다. 단서를 이용해 공동주의의 개시에는 성공하였으나 이를 길게 지속하는 데에는 어려움을 보인다는 것은 언어발달을 촉진할 수 있는 환경 조성이 어렵다는 것을 의미한다. 각 집단의 영유아의 수용 및 표현언어 점수와 공동주의 변수들의 상관을 살펴본 결과, 대부분의 공동주의 특성과 언어점수가 관련성이 있는 것으로 나타났다. 본 연구를 통해 인공와우이식 영유아 집단과 건청 영유아 집단의 공통주의 특성을 비교함에 따라 어떤 유형의 단서를 통해 공동주의를 개시하는 것이 확립을 이끌어내는 데에 효과적인지 확인하였다. 또한, 두 집단 간 공동주의의 시간적 특성을 비교하여 공동주의 확립에서 그치지 않고 이를 지속하고 유지하는 것의 중요성에 대해서도 파악하였으며, 마지막으로 언어점수와의 관련성을 살펴봄으로써 공동주의 행동이 영유아의 언어발달과 어떤 관계를 가지는지 알아보았다. 인공와우이식 영유아 집단이 어머니와의 상호작용 상황에서 나타나는 공동주의 특성에 대해 분석함으로써 난청 영유아를 대하는 어머니 및 치료사에게 유용한 임상적 시사점을 제공할 것으로 사료된다.
