Abstract
지방간질환은 전세계적으로 가장 흔한 만성 간질환 중의 하나이며, 대사증후군을 동반하는 경우가 많고 유병률이 점차 늘어나면서 중요성이 강조되고 있다. 본 연구는 대사(이상)관련 지방간질환을 진단 기준으로 한 대규모 종단연구로 근육량과 체지방률에 따른 질환의 발생 및 소실과의 관련성에 대해 알아보고, 기계학습의 분석 방법론을 적용하여 신체 조성과 그 변화가 대사(이상)관련 지방간질환의 발생과 소실에 미치는 영향에 다른 임상적 요인이 어떻게 작용하는지 추가로 알아보고자 한다. 한국의학연구소 건강검진센터에서 2014년 1월부터 2019년 12월까지 3차례 이상 직장 검진 혹은 개인 검진을 받은 19세 이상 성인 85,226명을 대상으로 하였다. 첫번째와 두번째 검진 사이에 평균 추적관찰 기간은 14.0 ± 5.1 개월에 해당하였으며, 체성분의 측정은 생체 전기 임피던스 분석(BIA, Inbody, Seoul, Korea)을 이용하였다. 신경망 모델인 DeepSurv를 이용하여 신체 조성에 따른 지방간의 발생과 소실과 관련한 예측 모델에 대해 추가로 분석하였다. 기저 시점에 MAFLD가 없었던 집단에서 근육량이 낮은 경우 모든 체질량지수군에서 새로운 MAFLD 발생 위험이 유의하게 높았고, 높은 체지방률은 정상체중, 과체중, 비만군에서 새로운 MAFLD 발생의 독립적인 위험인자였다. 추적 기간 중 근육량 감소와 체지방률의 증가는 정상체중, 과체중, 비만군에서 모두 MAFLD 발생의 위험을 높였다. 또한 근육량이 낮고 체지방률이 높은 경우 비만군에서 MAFLD 소실 가능성이 유의하게 낮아졌다. 첫번째와 두번째 검진 사이에 체지방률이 증가하는 경우에는 과체중 및 비만군에서 MAFLD 소실 가능성을 유의하게 낮추고, 근육량이 증가하는 경우 높은 삼분위수 군에서 체질량지수에 상관없이 MAFLD 소실 가능성을 유의하게 높였다. 또한, 최초 검진에서 MAFLD가 있던 군은 지방간이 소실된 경우 그렇지 않은 경우에 비해 심혈관계 위험도(CRI, AIP)와 인슐린 저항성(TyG)지표들이 낮은 수치를 보였고, 신기능은 차이를 보이지 않았다. 추적 검진에서 지속적으로 MAFLD가 관찰된 경우에는 간섬유화지표(NFS, FIB-4)는 악화되는 방향으로 변화함을 알 수 있었다. MAFLD가 없더라도 근육량이 낮거나 체지방률이 높은 경우, 추적 중 근육량이 증가하지 않고 체지방률이 증가하는 경우에는 체질량지수와 무관하게 MAFLD 발생 위험을 높였으며, MAFLD가 있더라도 근육량이 높거나 체지방률이 낮은 경우, 추적 중 근육량이 증가하거나 체지방률이 증가하지 않은 경우에는 비만군에서 MAFLD 소실 가능성을 높였다. 기저 시점의 근육량과 체지방률이 미치는 효과 뿐 아니라 1년 내외의 비교적 짧은 평균 추적 관찰 기간 동안 근육량과 체지방률의 변화가 지방간의 발생과 호전에 중요한 영향을 미칠 수 있으므로, 단순한 체중이나 체질량지수가 아닌 지방간의 발생위험이 높은 신체 조성을 가진 경우에 지방간을 조기에 발견하고 치료에 활용할 수 있으며 추적 치료 중 신체 조성의 변화에 주목하여 지방간을 예방하는데 도움이 될 수 있다. ;We investigated the effects of body components and their subsequent changes on the development of incident metabolic associated fatty liver disease (MAFLD) or the resolution of baseline MAFLD in a large, longitudinal, population-based cohort. In addition, by using machine learning, we further investigated the effect of body composition and other clinical factors on the development and resolution of MAFLD. This study included 85,226 who underwent three or more health examinations at the Korea Medical Institute between January 2014 and June 2019. The interval between the first exam and the second exam is 14.0 ± 5.1 months. Body composition was measured using bioelectrical impedance analysis. Machine learning algorithms using neural network (DeepSurv) was applied to develop prediction model for incident MALFD and its resolution. Mean follow-up duration was about 35.4 months. Among the entire study population, baseline MAFLD was identified in 31,005 subjects (36.4%). Of the 54,221 subjects without baseline MAFLD, 13,494 (24.9%) developed incident MAFLD. Compared to the lowest tertile of weight-adjusted skeletal muscle index (SMI_wt) at baseline, the middle and highest tertile were inversely associated with incident MAFLD (middle tertile: adjust hazard ratio [AHR]=0.740, 95% confidence interval [CI] 0.708-0.773, P<0.001; highest tertile: AHR=0.408, 95% CI 0.383-0.434, P<0.001). Compared to the lowest tertile of baseline fat percentage, the middle and highest tertiles were significantly associated with incident MAFLD (middle tertile: AHR=1.779, 95% CI 1.681-1.883, P<0.001; highest tertile: AHR=2.424, 95% CI 2.28-2.577, P<0.001). Decreased SMI_wt and increased fat percentage were independent risk factors for incident MAFLD in all weight categories (all P values <0.001). Effects of both baseline values and their changes of higher fat mass and lower SMI on the development of MAFLD were dose-dependent relationship as well as more robust in women than in men. Resolution of MAFLD was observed in 8,898 (28.7%) of 31,005 subjects with baseline MAFLD. Compared to the lowest tertile of SMI_wt at baseline, the middle and highest tertiles were significantly associated with MAFLD resolution (middle tertile: AHR=1.219, 95% CI 1.147-1.296, P<0.001; highest tertile: AHR=1.417, 95% CI 1.327-1.514, P<0.001). Compared to the lowest tertile of baseline fat percentage, the middle and highest tertiles were inversely associated with MAFLD resolution (middle tertile: AHR=0.855, 95% CI 0.811-0.902, P<0.001; highest tertile: AHR=0.697, 95% CI 0.653-0.745, P<0.001). Increased SMI_wt and decreased fat percentage were independent risk factors for MAFLD resolution in obese group (all P values <0.001). Effects of both baseline values and their changes of higher fat mass and lower SMI on the resolution of MAFLD were dose-dependent. MAFLD prediction and resolution model using machine learning showed sufficient predicting performance (C-index: 0.79 for incident MAFLD, and 0.69 for resolution of MALFD in the test set). Relative skeletal muscle and fat mass at baseline and their early changes were independently associated with the development of new MAFLD and the resolution of preexisting MAFLD, respectively.
