Abstract
WHO는 환자의 안전과 의료의 질 향상을 위해 환자가 진료과정에 적극적으로 참여할 것을 강조해 왔다. 고령 및 만성질환자의 증가와 함께 COVID-19 대유행에 따른 비대면 의료서비스 제공과 다양한 IT 기반 의료시스템 적용 등 보건의료환경의 급속한 변화는 진료과정에 환자참여를 더욱 요구하고 있다. 본 연구는 국내의 사회 문화적 맥락내에서 환자참여의 속성을 명확히 하고, 질병치료 및 건강관리과정에 환자참여 정도를 측정할 수 있는 도구를 개발하고자 시행되었다. 도구개발 단계에서는 환자참여와 관련된 선행문헌을 고찰하고 환자, 간호사 및 의사를 대상으로 수집한 질적자료를 분석하였다. 분석한 결과를 토대로 32문항의 환자참여 측정을 위한 예비문항을 개발하였고, 총 2회의 전문가 내용타당도 검증을 실시하여 문항을 수정, 보완하였다. 이후 국문학자의 검토와 예비조사를 통해 본조사를 위한 34문항이 확정되었다. 도구검증 단계에서는 서울시 소재 상급종합병원 4곳의 순환기내과와 호흡기내과 병동 또는 외래에서 치료 중인 성인환자 330명을 대상으로 자료수집을 진행하였으며, 이 중 312명의 자료를 이용하여 타당도와 신뢰도를 검증하였다. 구성타당도는 탐색적 요인분석과 확인적 요인분석으로 검증하였다. 탐색적 요인분석을 시행한 결과, 환자참여 측정도구는 ‘정보와 지식의 공유’, ‘의사결정과정에 참여, ‘주도적 자기관리활동 수행’, ‘상호 신뢰관계 수립’의 4개 요인, 21개 문항이 도출되었으며 도구의 설명력은 61.9%로 나타났다. 이후 21개 문항에 대한 확인적 요인분석이 실시되었으며 모형 적합도 확인결과 CMIN/DF, RMR, IFI, CFI는 기준에 적합하였고 GFI는 비교적 양호한 수준의 적합도를 나타냈으며, 수렴타당도가 확보되었다. 준거타당도 검증을 위해 준거가 되는 강세정과 박정윤(2019)의 도구에 대한 탐색적 및 확인적 요인분석을 실시하여 타당도와 신뢰도를 확인한 후 본 연구 도구와의 상관관계를 확인하였다. 그 결과, 상관계수는 .794(p<.001)로 개발된 도구의 준거타당도 근거가 확보되었다. 신뢰도 검증을 위해 내적일관성 신뢰도를 확인한 결과, Cronbach's α 계수 .92로 나타나 도구의 안정성과 일관성이 확보되었음을 확인하였다. 이상과 같이 본 연구를 통해 개발된 환자참여 측정도구는 국내 사회 문화적 특성이 반영된 도구로, 환자가 자신의 질병치료 및 건강관리과정에서 경험하는 환자참여 정도를 타당하고 신뢰성 있게 측정할 수 있다. 따라서 측정결과는 환자와 의료진의 환자참여에 대한 인식을 고취시키며 환자참여 활성화 전략을 마련하는데 기여할 것이다. ;WHO has encouraged patients to actively participate in their care to improve quality of care and patient safety. The changes in the healthcare environment, such as the increase in the number of elderly and chronically ill patients, the provision of contactless healthcare services due to the COVID-19 pandemic, and the introduction of various IT-based healthcare systems, require more patient participation in their care. The purpose of the study is to develop a scale to measure patient participation in the Korean socio-cultural context and to investigate measurement properties of the scale. In order to develop the scale, the literature data related to patient participation and the qualitative data were collected from patients, nurses, and doctors. Based on the results, 32 preliminary items were developed to measure patient participation, and they were modified and supplemented after being validated by experts in two rounds. Then 34 items were finalized through validation by Korean language scholars and preliminary surveys. During the scale validation stage, data were collected from 330 adult patients(≥18 years) who being treated in cardiology or pulmonology ward or outpatient clinic at the four tertiary hospitals in Seoul. Of these, data of 312 patients were used to verify the validity and reliability. The construct validity was verified by exploratory factor analysis and confirmatory factor analysis. Using the exploratory factor analysis, 21 items of four factors were derived: “sharing of information and knowledge”, “partaking in decision-making process”, “performing autonomous self-management activities”, and “establishing a mutual trust relationship”. The variance of four factors was 61.9%. In the confirmatory factor analysis, the goodness-of-fit test showed that the CMIN/DF, RMR, IFI, and CFI all satisfied the criteria, and the GFI was relatively decent, indicating that convergent validity has been achieved. In order to verify the criterion validity, an exploratory and confirmatory factor analysis of the reference validity scale was conducted to confirm its validity and reliability. The criterion validity of the scale was verified by analyzing the correlation between the score of the developed scale and that of the reference validity scale. The reliability was measured with the internal consistency reliability, and Cronbach’s α was .92. The patient participation scale developed in this study reflects the social- cultural characteristics of Korea and can evaluate the degree of patient participation with validity and reliability. Therefore, the results of the measurement will enhance the awareness of patient participation for the healthcare provider and contribute to developing strategies for promoting patient participation.
