Abstract
본 연구는 중국 내 성인 뇌손상 환자를 대상으로 음악을 활용한 중재 연구들을 고찰하여 전반적인 특징과 음악 중재 및 음악 특성에 대해 알아보고, 이를 통해 중국 음악치료 재활 연구의 임상적 확장과 적용에 필요한 자료를 제공하기 위해 시행되었다. 연구 내 자료 선정기준에 따라 총 83편의 연구를 수집하였으며, 이를 분석한 결과는 다음과 같다. 먼저, 연구의 전반적 특성을 살펴보면, 2012년 이후 음악을 활용한 중재연구의 수가 증가하였고, 연구 방법에 따른 분류에서는 대조군을 포함한 무작위 실험설계 연구가 53(63.8%)편으로 가장 많았다. 또한 연구의 대상자로는 뇌졸중 환자가 전체 중 79.5%로 가장 높은 비중을 차지하였고, 외상성 뇌손상(13.2%)과 파킨슨씨병(7.2%) 등이 그 다음 순으로 나타났다. 중재 영역별로 분류한 결과 신체 영역은 39.7%, 정서 영역은 38.5%로 두 영역이 유사한 비중을 차지하였다. 연구가 발표된 학문 및 학술지의 분야에는 재활의학, 의약학, 중의학, 간호학 등이 있었으며, 연구 내 중재를 음악치료사가 시행한 연구는 14편(16.8%)으로, 타 분야의 연구자에 의해 시행된 43편(51.8%)보다 적은 것으로 확인되었다. 이러한 중재 제공자의 전문 분야에 따라 연구 내 중재의 특성에도 차이가 있었다. 음악치료사가 아닌 연구자가 중재한 연구들은 많은 경우 정서 영역을 목표로 하여 기존의 녹음 음악을 사용한 감상 활동을 주로 진행한 반면, 음악치료 전공자가 중재한 연구들은 신체 영역의 목표를 위해 치료사가 재구성한 음악을 라이브로 제공하여 활동을 진행한 경우가 많았다. 이 외에 중재 내용의 기술 수준 및 기법 적절성을 평가한 결과, 음악치료사에 의해 진행된 중재 연구가 다른 연구에 비해 상위 점수를 부여받은 비중이 높게 나타났다. 이와 같은 결과는 중국 내 음악치료 연구에서 음악의 치료적 원리를 규명하기 위해서는 음악이 독립변인으로 사용된 중재연구가 진행되어야 할 뿐 아니라 이를 진행하는 음악치료사 역시 체계적 교육과정을 통해 연구자로서도 준비되어야 함을 시사한다. 또한 중국 음악치료의 효과를 보다 명확하게 설명하기 위해서는 연구 내 중재 기법을 구성하는 치료적 근거가 명확하게 기술되어야 함을 시사한다. 본 연구는 중국 내 성인 뇌손상 환자를 대상으로 진행된 음악치료 연구에 대한 고찰을 시도하여 연구에 대한 정보의 제공과 함께 관련된 시사점을 함께 기술함으로써 중국 음악치료의 연구와 임상 현장에 모두 적용 가능한 자료를 제공했다는 점에 의의가 있다.;This study aims to provide data for clinical expansion and application of more advanced music therapy rehabilitation research in the future by exploring music intervention studies for adult brain injury patients in China. In order to achieve the purpose, a total of 83 cases that conformed to the selection criteria were analyzed. The overall characteristics of the analysis and research, also the analysis results of the musical intervention and musical characteristics, are as follows. It reveals that the number of intervention studies using music has been on the rise since 2012, and 53 cases (63.8%) are conducted by using random control research design methods have been dominant. In addition, among the subjects of the study to be analyzed, the stroke subjects (79.5%) are the group with the most number in progress, followed by traumatic brain injury subjects (13.2%) and Parkinson's subjects (7.2%). The study of the body domain (39.7%) and emotional domain (38.5%) together occupied the highest proportion. Rehabilitation medicine, medicine and pharmacy, Chinese traditional medicine, nursing, and other fields manifested as specialized fields of degree majors and academic journals in published research. Researchers in other fields conducted 43 cases (51.8%), and music therapists carried on 14 cases (16.8%) as intervention providers. The characteristics of musical intervention in analytical research vary depending on the provider's professional field. Most cases mediated by experts in other fields use existing recorded music to perform interventions that focus on the emotional field of listening activities. In contrast, most studies mediated by music therapists adopt reconstructed music to provide live music. The intervention proceeded with physical goals. It was found that the proportion of research conducted by music therapists in evaluating the description of the intervention content and the level of technical adequacy was higher than that of experts in other fields. The above indicates that music therapists who have completed professional education courses should carry out intervention studies with music as an independent variable, and technology-based techniques should be embodied in the application of music therapy. The significance of this research is that it provides information applicable to clinical settings by presenting necessary data for the research of Chinese music therapy on adult brain injury patients.
