Abstract
사회경제적 수준에 따른 거주지의 분리로 경제적 취약 지역 인구집단 기인 건강격차가 심화되고 있다. 경제적 수준에 따른 지역별 불균형은 지역의 건강수준과 연결되어 암, 심혈관 및 뇌혈관 질환 등의 유병률 및 사망률에 영향을 미치는데, 특히 흡연은 사회경제적 불평등도가 높은 건강행태 요인으로 지역 간 건강격차를 유발하는 대표적인 원인이 되고 있다. 그러나 동시에 건강증진서비스를 통해 개선의 가능성이 매우 큰 건강결정요인이기 때문에 흡연을 중심으로 건강격차의 원인을 탐색하고 해결방안을 찾는 것은 지역 간 건강불평등을 해소하기 위해 매우 중요하다. 이에 본 연구에서는 WHO 건강의 사회적 결정요인 개념 틀을 기반으로 대도시인 서울시를 중심으로 지역 간 흡연행태 격차에 영향을 미치는 개인수준 및 지역수준 건강결정요인을 탐색함으로써, 사회경제적 계층에 따른 흡연행태 격차와 건강불평등 해소를 위한 전략 수립의 근거자료를 마련하고자 하였다. 이를 위해 본 연구는 2018년도 서울시 지역사회건강조사 데이터와 주민등록인구통계, 인구주택총조사, 서울시 건강격차모니터링 등의 자료를 이용하여 흡연행태인 현재흡연여부에 영향을 미치는 건강결정요인들을 분석하였다. 우선 SPSS 20.0을 이용하여 복합표본을 통해 서울시 전체 및 지역별 개인수준과 지역수준 독립변수들의 분포 및 차이를 비교분석하였으며, 로지스틱 회귀분석을 통해 개인수준 변수들의 현재흡연여부에 대한 영향력을 검증하였다. 이후 HLM 8.0 프로그램을 이용하여 개인수준 및 지역수준 건강결정요인들이 현재흡연여부에 미치는 영향과 상호작용효과를 검증하였다. 본 연구의 주요 결과를 요약하면 다음과 같다. 첫째, 지역에 따라 흡연행태와 개인수준 및 지역수준 건강결정요인에 유의미한 차이가 있었다. 즉 개인수준 요인 중 소득 및 교육수준, 기초생활수급여부, 직장실내 간접흡연 노출경험, 흡연시작 연령, 음주, 비만, 규칙적 운동실천, 스트레스 인지, 주관적 건강수준의 개인수준 모든 변수에 지역 간 차이가 있었다. 또한 금연클리닉 운영 및 금연환경 추진 실적, 지역의 성인 및 남성흡연율, 재정자주도, 지역박탈지수, 비주택거주 가구비율, 담배소매점 개소수의 지역수준 변수에 있어서도 지역 간 차이가 있었다. 둘째, 흡연행태에 개인수준 요인뿐만 아니라 지역수준 요인의 효과가 존재하였다. 흡연행태에 대한 지역 간 분산 비율은 2.0%로 흡연행태를 설명함에 있어 지역 수준 특성의 영향력이 존재하고 있음을 확인할 수 있었으며, 지역수준 보다는 개인수준 특성들의 영향력이 보다 크게 영향을 미치고 있음을 알 수 있었다. 셋째, 개인수준 건강결정요인들이 흡연행태에 영향을 미치고 있었는데, 남성일수록, 연령이 낮을수록, 교육 및 소득수준이 낮을수록, 직장실내 간접흡연 노출이 많고 흡연시작 연령이 늦을수록, 월 1회 이상 음주를 할수록, 비만하지 않을수록, 규칙적 운동 실천을 하지 않을수록, 스트레스를 많이 느낄수록 흡연할 확률이 높아지는 것을 확인할 수 있었다. 넷째, 지역수준 건강결정요인들도 흡연행태에 영향을 미치고 있었는데, 특히 실내 금연구역 과태료 부과건수가 많을수록, 지역의 남성흡연율이 높을수록, 비주택거주 가구비율이 높을수록, 담배소매점 개소수가 적을수록 흡연할 확률이 높아지고 있었다. 다섯째, 개인수준과 지역수준 요인들의 상호작용 또한 흡연행태에 영향을 주고 있었는데, 개인이 규칙적인 운동 실천을 하는 경우, 지역의 남성 흡연율이 높거나 지역 내 비주택거주 가구비율이 높더라도 흡연을 할 확률이 줄어드는 것으로 나타났다. 본 연구는 지역 간 흡연행태 격차를 유발하고 있는 개인수준 및 지역수준 건강결정요인들의 영향을 검증함으로써 흡연행태 개선을 위한 다수준적 접근 전략을 제시하였다는데 의의가 있다. 또한 서울시 각 지역별 구성원의 인구사회학적 특성 및 건강관련 특성, 금연관련 정책 등을 비교·분석한 본 연구의 결과는 각 지역별 금연정책 수립에도 유용한 자료로 활용될 수 있을 것이다.;The separation of residential areas according to the socioeconomic level is widening the health disparities resulted from the population group in economically vulnerable areas. The regional imbalance due to the economic levels interacts with the regional health levels and affects the prevalence and mortality rates of cancer, cardiovascular and cerebrovascular diseases. In particular, smoking is one of the biggest causes of health disparities between regions as a health behavior factor with high socioeconomic inequality. At the same time, since it is the major determinant health factor with lots of possibility of improvement through health promotion services, it is very important to explore the causes of health disparities centering on smoking and find solutions to resolve health inequality between regions. For these reasons this study explores health determinants at individual and regional levels that effect smoking behaviors based on the WHO's concept of social determinants of health in order to prepare the basis for establishing strategies to tackle disparity in smoking behaviors and health according to the socioeconomic class. To this end, this study analyzed the health determinants that affect the current smoking behavior using data such as 2018 community health survey data, resident registration population statistics, population and housing census, and Seoul city health disparities monitoring. First, it compared and analyzed the distribution and difference of independent variables at the individual and regional levels for the entire city and each region through a composite sample using SPSS 20.0, and verified the influence of the individual level variables on the current smoking status through logistic regression analysis. Then, the HLM 8.0 program was used to verify the effects of individual and regional health determinants on current smoking status and interaction effects. The main result of this study was summarized as follows. First, there were significant differences in smoking behavior and health determinants at the individual & regional levels depending on the region. In other words, there were differences between regions in all individual variables such as income and education level, basic livelihood security recipients, exposure to secondhand smoking in the workplace, age of smoking starts, alcohol, obesity, regular exercise, stress perception, and subjective health level. In addition, there was another regional difference in terms of the number of smoking cessation clinics and promotion of smoking cessation environments, regional adult and male smoking rates, financial independence, regional deprivation index, ratio of non-residential households, and the number of tobacco retail stores in the area. Second, smoking behaviors were not only affected by individual-level factors but also by regional-level factors. The regional distribution ratio of smoking behavior was 2.0%, and it confirmed that the regional-level factors in when developing smoking behavior. It also showed that the individual-level elements had a greater impact than the regional level factors. Third, individual-level health determinants shaped the smoking behavior. It was confirmed that more men than female, the younger the age, the lower the education and income level, the more exposure to secondhand smoking in the workplace, and the later in the life the smoking starts, when drinking alcohol more than once a month, the less obese, the less regular exercise, the more stressed, the probability of smoking is increasing. Fourth, the regional health determinants were also influencing smoking behavior. In particular, the higher the number of fines imposed for indoor smoking violaiton, the higher the male smoking rate in the region, the higher the proportion of non-residential households, and the smaller the number of tobacco retail stores. The probability of smoking was increasing. Fifth, the interaction of the individual and regional level factors also affected smoking behavior. When individuals exercised on a regular basis, their smoking probability decreased even though the male smoking rate of the area was high or the proportion of non-residential households in the area was high. This study was meaningful in that it suggested a multi-level approach to improving smoking behavior by verifying the effects of health determinants at the individual and the regional level that were causing the smoking behavior disparities between regions. In addition, the results of this study, which compared and analyzed the demographic and health-related characteristics of residents of each borough in Seoul, and policies related to smoking cessation, can be used as useful data for the establishment of smoking cessation policies for each area.
