발치와 치조제 보존술 후 식립한 임플란트 주위 골 변화

Abstract

본 연구에서는 구치부 임플란트 식립을 위하여 발치와 치조제 보존술을 시행한 환자를 대상으로 임플란트 식립 직 후, 최종 보철물 장착 후 및 유지관리 기간에 따른 치조제 높이 변화량을 방사선학적으로 측정하여 발치와 치조제 보존술 후 식립한 임플란트 주위 골 변화를 평가하고자 하였다. 2016년 1월부터 2019년 12월까지 발치 후 임플란트 식립을 주소로 이화여자대학교 목동병원 치과 치주과에 내원하여 구치부 발치와 동시에 치조제 보존술을 시행한 후 임플란트 식립을 한 32명을 연구대상으로 선정하였다. 총 36개의 임플란트가 식립되었고, 임플란트 식립 후 추적관찰기간은 최소 6개월에서 최대 2년 6개월로 평균 1년 5개월이었다. 발치와 치조제 보존술을 시행한 모든 부위는 임플란트 식립 직 후, 최종 보철물 장착 후, 유지관리 기간에 촬영된 파노라마 이미지를 이용하여 근심측, 원심측의 치조제 높이(Crestal bone level : CBL)를 각각 수직적 수치로 측정하여 평가하였다. 연구대상으로 선정된 환자의 기록과 방사선 자료에 대한 후향적 연구를 수행하였다. 환자의 연령 및 성별, 전신질환, 임플란트 식립 위치(즉, 치아 번호), 발치와 치조제 보존술에 사용된 골 이식재 유형, 차폐막 적용 방법, 술자의 숙련도, 임플란트 식립 직 후, 최종 보철물 장착 후, 유지관리 기간별 측정 시기간의 치조제 높이 변화량은 평균과 표준편차를 구하였고, 치조제 높이 변화량과 각각의 요소들 간의 연관성은 독립표본 t검정을 시행하였다. 임플란트 식립 직 후, 최종 보철물 장착 후, 유지관리 기간별 측정한 치조제 높이 간의 연관성은 대응표본 t검정을 시행하였다(p<0.05). 본 연구를 통해 다음과 같은 결과를 얻었다. 1. 각 시기별 임플란트 주위 치조제 높이의 평균값은 임플란트 식립 직 후 근심 0.27mm, 원심 0.27mm, 최종 보철물 장착 후 근심 0.30mm, 원심 0.30mm, 유지관리 기간에 근심 0.35mm, 원심 0.37mm을 나타냈다. 2. 각 시기별 임플란트 주위 치조제 높이 변화량은 임플란트 식립 직 후와 최종 보철물 장착 후 사이, 최종 보철물 장착 후와 유지관리 기간 사이, 임플란트 식립 직 후와 유지관리 기간 사이에 모두 통계적으로 유의한 차이를 보였다(p<0.05). 3. 임플란트 식립 직 후, 최종 보철물 장착 후, 유지관리 기간 시기 간의 치조제 높이의 변화량과 환자의 연령 및 성별, 전신질환, 임플란트 식립 위치, 발치와 치조제 보존술에 사용된 골 이식재 유형, 차폐막의 적용 방법, 술자의 숙련도 등은 통계적으로 유의한 연관성을 보이지 않았다. 본 연구 결과 발치와 치조제 보존술 후 임플란트 주위 골조직은 환자 및 수술적 요소에 영향을 받지 않고 유지기간동안 안정적으로 유지되었으며, 발치와 치조제 보존술은 임상적으로 사용 가능한 술식이라고 사료된다. ;The aim of this study is to evaluate bone change around the implant in patients who underwent alveolar ridge preservation for implantation in the posterior teeth using radiographic data measuring changes of mesial, distal crestal bone level according to post-implantation, post-final prosthesis delivery and follow-up periods. A retrospective study was performed of the records and radiologic data of patients who underwent alveolar ridge preservation for implantation in the posterior teeth from January 2016 to December 2019 in Ewha Womans University Mokdong Hospital. In total, 36 implants were placed in 32 patients. The follow-up period after implant placement was an average of 1 year and 5 months, from a minimum of 6 months to a maximum of 2 years and 6 months. The mesial and distal crestal bone level of all the areas where alveolar ridge preservation was performed uses panoramic images taken post-implantation, post-final prosthesis delivery, and follow-up period each was measured and evaluated as a vertical value. The following factors were analyzed: associations between changes of crestal bone level and factors(e.g., age, sex, systemic diseases, dentist, implant location, tooth, bone type, membrane). The statistical analysis was performed using the mean, standard deviation and Independent T-test, Paired T-test(p<0.05). The following results were obtained through this study. 1. The average value of the crestal bone level for each period was mesial 0.27mm, distal 0.27mm when post-implantation, and mesial 0.30mm, distal 0.30mm when post-final prosthesis delivery, and mesial 0.35mm, distal 0.37mm when follow-up periods. 2. Analysis of crestal bone level differences between periods shows statistically significant differences(p<0.05). 3. There was no statistically significant difference when the changes of crestal bone level between post-implantation, post-final prosthesis delivery and follow-up periods were correlated with each factors. In conclusion, after alveolar ridge preservation, bone around the implant remained stable during the maintenance period without being affected by the patient and surgical factors, and alveolar ridge preservation is considered a clinically usable procedure.
