혈액투석 남성 환자의 성기능, 우울, 수면의 질

Abstract

혈액투석 환자는 말기 만성 신부전 외에도 만성질환을 앓고 있으므로 질환이 진행됨에 따라 다양한 증상을 호소한다. 혈액투석 남성 환자는 전체 혈액투석 환자의 60%를 차지하며, 신체적, 심리적 증상뿐만 아니라 성기능 장애를 경험하지만, 한국 문화적 특성상 쉽게 호소할 수 없어 삶의 질 저하에 영향을 미친다. 본 연구는 혈액투석 남성 환자의 성기능, 우울, 수면의 질 간의 관계를 파악하기 위한 서술적 조사연구이다. 연구 대상자는 서울특별시 소재 E대학병원 인공신장실에서 혈액투석을 받는 남성 환자 134명을 대상으로 하였다. 자료 수집 기간은 임상연구 심의위원회의 승인을 받은 후 2019년 6월 10일부터 2019년 10월 30일까지 진행하였다. 성기능은 Rosen 등(1997)이 개발한 International Index of Erectile Function (IIEF)를 정태규 등(1999)이 번안한 한국어판을 사용하였고, 우울은 Radloff(1977)가 Center for Epidemiologic Studies–Depression Scale (CES-D)를 전겸구, 최상진, 양병창(2001)이 번안한 한국어판을 사용하였으며, 수면의 질은 Buysse 등(1989)이 피츠버그 수면센터에서 개발한 Pittsburgh Sleep Quality Index (PSQI)를 손승일, 김도형, 이미영, 조용원(2012)이 번안한 한국어판을 사용했다. 수집된 자료는 SPSS 21.0 program을 이용하여 각 변수의 실수와 백분율, 평균과 표준편차를 산출하였다. 대상자의 일반적 특성 및 질병관련 특성에 따른 성기능, 우울, 수면의 질 정도의 차이는 t-test, ANOVA로 분석하고, 사후 검정은 Scheffe test로 분석하였다. 성기능, 우울, 수면의 질 간의 관계는 Pearson’s correlation coefficients로 분석하였다. 본 연구의 결과는 다음과 같다. 1. 대상자의 성기능 점수는 평균 8.69±7.65점으로 나타났고, 우울 점수는 평균 20.69±7.16점으로 나타났으며. 수면의 질 점수는 평균 6.58±4.29점으로 나타났다. 2. 성기능은 대상자의 연령(F=10.927, p<.001), 직업 유무(t=2.582, p=.011), 월평균 수입(F=6.157, p=.003), 도움을 주는 사람이 누구인지에 따라(F=2.783, p=.044), 운동 유무(t=3.233, p=.002), 흡연 여부(F=3.775, p=.025), 앞으로의 성상담 기회가 있을 시 상담 의향 여부(t=3.103, p=.002)에 따라 유의한 차이를 보였다. 우울은 대상자의 직업 유무(t=-3.278, p=.001), 월평균 수입(F=3.737, p=.026), 현재 앓고 있는 질환 중 암의 유무(t=2.849, p=.005), 수면제 복용 유무(t=3.520, p=.003), 당뇨병약 복용 유무(t=-2.175, p=.031), 우울증약 복용 유무(t=2.824, p=.025), 한 달 동안 힘들었던 증상 수(F=3.953, p=.005)에 따라 유의한 차이를 보였다. 수면의 질은 대상자의 도움을 주는 사람이 누구인지에 따라(F=5.603, p=.001), 현재 앓고 있는 질환 중 심혈관질환의 유무(t=2.066, p=.042), 수면제 복용 유무(t=10.711, p<.001)에 따라 유의한 차이를 보였다. 3. 대상자의 성기능은 수면의 질과 유의한 상관관계가 없는 것으로 나타났지만, 성기능의 하부영역인 전반적인 만족도는 우울과 유의한 음의 상관관계가 있는 것으로 나타났다(r=.-19, p=.028). 대상자의 우울은 수면의 질과 유의한 음의 상관관계가 있는 것으로 나타났다(r=.60, p<.001). 본 연구 결과를 종합해 보면, 혈액투석 남성 환자의 성기능과 수면의 질이 낮았으며, 우울 정도가 높은 것으로 나타났다. 또한 혈액투석 남성 환자의 우울 정도가 높을수록 수면의 질이 낮게 나타났으므로 우울과 수면의 질은 상관관계가 있는 것으로 나타났다. 따라서 혈액투석 남성 환자의 성기능, 우울, 수면의 질을 개선하기 위해 혈액투석 남성 환자에게 더욱 적극적인 관심을 가지고 사정함으로써, 혈액투석 남성 환자의 우울을 낮추고, 성기능 및 수면의 질을 향상하기 위한 중재 개발이 필요하다고 사료된다. ;Hemodialysis patients have chronic diseases in addition to end stage chronic renal failure, and as the disease progresses, they complain of various symptoms. Hemodialysis male patients account for 60% of the total hemodialysis patients and experience sexual dysfunction as well as physical and psychological symptoms, but due to the cultural characteristics of Korea, it can not easily appeal, affecting the quality of life. This study is a descriptive study to investigate the relationship between sexual function, depression and sleep quality in male patients with hemodialysis. The participants of this study consisted of 134 patients from outpatients who were diagnosed with end stage chronic kidney disease and have undergone the hemodialysis. The data was collected from June 10th, 2019 to Octobel 30th, 2019. The structured questionnaires include the International Index of Erectile Function (IIEF), Center for Epidemiologic Studies–Depression Scale (CES-D), and the Pittsburgh Sleep Quality Index (PSQI). The collected data was then analyzed using descriptive statistics, t-test, ANOVA, Scheffe test and Pearson’s correlation coefficient using the SPSS 21.0 program. The findings of this study are as follows: 1. The mean score was 8.69±7.65 for sexual function, 20.69±7.16 for depression, and 6.58±4.29 for sleep quality. 2. The sexual function of the participants varied widely according to the age(F=10.927, p<.001), occupation(t=2.582, p=.011), income level(F=6.157, p=.003), helper(F=2.783, p=.044), exercise(t=3.233, p=.002), smoking(F=3.775, p=.025) and future sexual consultation(t=3.103, p=.002). Depression of participants varied widely according to occupation(t=-3.278, p=.001), income level(F=3.737, p=.026), cancer(t=2.849, p=.005), taking sleeping pill(t=3.520, p=.003), taking diabetes mellitus medication(t=-2.175, p=.031), taking depression medicaion(t=2.824, p=.025), and number of disease related symptoms during one month(t=3.953, p=.005). Sleep quality of the participants varied widely according to helper(F=5.603, p=.001), cardiovascular disease(t=2.066, p=.042), and taking sleeping pill(t=10.711, p<.001). 3. The sexual function of the subjects was not statistically correlated with the quality of sleep, but the overall satisfaction level, which is a subdomain of sexual function, was statistically correlated with depression(r=.-19, p=.028). Subjects' depression showed negative correlation with sleep quality(r=.60, p<.001). These results suggest that hemodialysis male patients had low sexual function and sleep quality, and high levels of depression. In addition, the higher degree of depression in hemodialysis male patients, the lower the quality of sleep. Therefore, it is necessary to develop intervention to improve sexual function and sleep quality and lower the depression of male hemodialysis patients.
