간호대학생의 독일간호실습 경험

Abstract

글로벌 시대를 맞이하여 대학교육 변화에 대한 요구가 그 어느 때보다 중요한 시점에 간호학 분야에서도 국제적으로 간호대학과의 교류협력을 증진하여 학생들의 국제적인 역량을 키워 나가는데 노력을 기울이고 있다. 본 연구는 간호대학생의 독일간호실습 경험의 의미를 밝히고 자료로부터 패턴화된 의미를 귀납적으로 이끌어내어 상세하고 풍부한 설명을 제공하고자 질적주제분석방법을 적용한 질적 연구이다. 본 연구의 대상자는 서울시 소재 일개 간호대학에서 3학년 학생 중 독일간호실습을 경험한 간호대학생으로 편의표집과 눈덩이표집 방식을 통해 최종 8명을 대상으로 하였다. 자료는 면대면 인터뷰를 통해 수집하였으며, 2018년 11월 18일부터 2018년 12월 14일까지 자료의 포화가 이루어질 때까지 진행하였다. 간호대학생의 독일간호실습 경험에 대한 면담내용은 Braun과 Clarke이 제시한 6단계의 질적주제분석방법을 적용하여 분석한 결과, 6개의 주제와 15개의 하위주제가 도출되었다. 첫 번째 주제는‘해외실습을 통해 새롭게 배운다는 기대감을 가짐’이었고, 하위주제로는‘선진간호를 배운다는 기대감이 있음’, '독일이라는 특수한 장소에 대한 호기심이 있음’이었다. 두 번째 주제는‘언어적⋅비언어적 의사소통을 적극 활용하여 언어문제에 대처해 감’이고, 하위주제는 ‘독일어 포켓북을 제작하여 학습함’,‘비언어적인 몸짓을 이용함’,‘번역기를 최대한 활용함’이었다. 세 번째 주제는‘실습에 대한 자신감을 가지도록 지지를 받음’이며, 하위주제는‘학생을 대하는 환자들이 호의적인 반응을 보임’,‘간호사가 간호를 제공할 때마다 상세한 설명을 제시해 줌’,‘간호사로부터 학생을 신뢰한다는 인상을 받음’이었다. 네 번째 주제는‘환자에게 수행하는 기본간호의 중요성을 깨달음’이며,‘간호사는 환자들의 기본간호에 모두 참여함’,‘환자와의 라포형성에 기본간호가 바탕이 된다고 느낌’이었다. 다섯 번째 주제는‘병동 분위기가 여유롭고 편안한 분위기여서 부러움’이며, 하위주제는‘의료진, 환자, 보호자들 간에 대화를 나누는 모습이 편안해 보임’,‘실습 동안 몸의 여유로움을 느꼈음’이었다. 여섯 번째 주제는‘한국의 실습 상황을 되돌아보는 계기가 됨’이며, 하위주제는‘실습에 직접 참여하면서 그동안 느꼈던 실습에서의 두려움이 없어짐’,‘ 환자에게 간호 제공 이전의 편안한 관계의 의미를 새겨봄’,‘한국간호실습과의 차이점을 통해 배울 수 있었음’이었다. 본 연구의 결과, 간호대학생들은 독일간호실습에 참여하면서 실습에 대한 자신감을 가지도록 지지를 받았다. 이러한 지지에는 간호대학생이 직접적으로 실습에 참여하도록 기회를 제공하고 독일간호사의 친절한 지지와 환자들의 호응을 들 수 있다. 또한 모든 간호사가 참여하는 기본간호가 환자와의 신뢰 관계를 유지하는 바탕을 이룬다는 것을 깨닫고 그 중요성을 인식하였다. 무엇보다 독일간호 실습에 참여한 간호대학생들은 여유롭고 편안한 병원 분위기를 통해 의료진과 환자 및 보호자 간의 편안한 대화가 한국의 간호실습 환경과 상당히 다른 경험이었음을 인식하였다. 이러한 경험은 독일간호를 이해할 수 있는 지평을 넓히고 간호대학생의 지속적인 해외실습경험의 폭을 넓힐 수 있는 기초자료로 기여하였다는데 의의가 있다. ;The purpose of this study was to explore by a qualitative thematic analysis on how nursing students' practical training experiences in Germany are being expressed. With patterned meaning drawn inductively form the data, this research has applied the qualitative subject analysis towards nursing university students' practical training in Germany to understand its meaning in-depth and to provide detailed and profound information. The subjects of this research are 8 individuals who agreed to participate to the research in-depth interview amongst 16 nursing students who had a one semester practical training experience in Korea and have practical training experience in Germany amongst nursing university students whose institute are located in S District in Seoul Metropolitan City. The data collection period is from November 18, 2018 to December 14, 2018, and the analysis method for the data analysis regarding interview was analyzed with the application of 6-step Qualitative subject analysis that Braun and Clarke(2006) has suggested. The articles were analyzed by Braun and Clarke(2006)'s qualitative thematic analysis. Through the analysis of data, 6 themes and 15 subthemes were extracted as shown below: As a result of this research, the practical training experiences in Germany of nursing university students were divided into 6 themes and 15 subthemes. The first theme was‘expectations for new teachings through overseas practice, with subthemes of‘expectations for advanced nursing’,‘curiosity about a special place, Germany’. Second theme was 'resolving linguistic problems through active utilization of linguistic․non-linguistic communication’,with subthemes of‘make and learn from German pocket-book’,‘understand non-linguistic gestures’ and‘make full use of translator’. Third theme was‘gain support to become confident in practice’, with subthemes of ‘favorable response from patients faced by students’, 'nurses provided detailed explanations to students upon nursing’ and‘nurses gave impression that they trusted students’. Fourth theme was‘realization of basic nursing provided to patients’, with subthemes of 'nurses attended to every basic nursing of patients’and‘basic nursing provided basis to form rapport with patients’. Fifth theme was‘Envious of relaxed and comfortable ambience of ward’, and relevant subthemes were‘conversations between medical staff, patients and guardians seemed comfortable’ and‘felt comfortable during practice period’. Sixth theme was ‘opportunity to review practice conditions of Korea’, and following subthemes were‘overcame the fear of practice by participating in practice’,‘acknowledged the meaning of comfortable relationship rather than nursing the patients’and‘learnt much through differences from Korean nursing practice’. Therefore, the results of this research brought significance together the diverse society and cultural meaning of the overseas experience that will bring nursing students in the future, and as a starting data to improve the continuous overseas experience for the nursing university students in the long-term. By escaping the fear of practical training, and receive healing.
