- 위암은 우리나라에서 두번째로 흔하게 발생하는 암으로, 근치적 수술에도 불구하고 흔히 재발한다. 최근 보조 항암화학치료가 발전하고 분자생물학적 이해가 향상되었지만, 2-4병기에 해당하는 환자들의 재발률이 25-40%로 여전히 높고 세포 독성 항암화학치료 외에는 다른 뚜렷한 보조 요법이 없어서, 위암의 재발과 전이를 막을 수 있는 새로운 약제에 대한 연구가 필요하다. Epithelial-mesenchymal transition (EMT)는 상피 세포가 극성과 세포간 결합 등, 그 고유의 성질을 잃고 간엽 세포화되어 이동성을 획득하게는 과정으로 암의 전이 및 진행에 중요하게 작용하며, Wnt/β-catenin, PI3K/Akt/mTOR, Ras, TGF-β 같은 다양한 세포내 신호전달체계들에 의해 조절된다. 이중 Wnt/β-catenin 신호전달체계는 세포의 성장, 분화, 발달 과정에 중요한 역할을 하며, 여러 암의 발생과 진행에 관련되어 있다. 위암에서도 약 30-50%에서 Wnt/β-catenin 신호전달체계의 비정상적인 활성화가 보고되고 있고, 림프절 전이 및 나쁜 예후와 연관이 있다고 알려져 있다. 이 Wnt/β-catenin 신호전달체계는 다른 신호전달체계들과의 상호 작용에 의해 영향을 받는데, 특히 PI3K/Akt/mTOR 신호전달체계는 GSK3β를 인산화시켜 그 기능을 억제함으로서 세포내에 β-catenin을 축적시키고 Wnt/β-catenin 신호전달체계의 활성화를 가중시킬 수 있다. 본 연구에서는 Wnt/β-catenin 신호전달체계가 활성화되어 있는 위암 세포주에서 Wnt/β-catenin과 PI3K/Akt/mTOR 두 신호전달체계를 동시에 억제시 부가적인 항종양 효과가 있는지 알아보고 그 기전을 살펴보고자 하였다.
AGS와 MKN-28 위암 세포주에서 Western blot을 이용한 단백질 발현 정도와 luciferase assay를 이용한 전사 활성화 측정을 통하여 β-catenin 활성이 증가되어 있는 것을 확인하였고, 두 신호전달체계를 억제하기 위하여 β-catenin 저해제인 ICG-001과 mTOR 저해제인 rapamycin을 사용하였다. 두 세포주 모두에서 ICG-001 처리시 세포 생존율이 용량 의존적으로 감소하였고, AGS 세포주의 경우 rapamycin을 함께 처리시 세포 생존율이 다소 더 감소하는 결과를 보였으나, MKN-28 세포주에서는 차이가 없었다. Cell invasion assay에서 ICG-001 단독 처리시 AGS 세포의 침습이 47% 억제되었고, rapamycin 추가시 83% 억제되었으며, Cell scratch assay에서도 ICG-001 단독 처리시 AGS 세포의 이동이 33% 억제되었으나, rapamycin 추가시에는 82% 억제되어, ICG-001과 rapamycin을 함께 처리한 경우 위암 세포주의 침습과 이동을 억제하는데 부가적인 첨가 효과가 있음을 확인할 수 있었다. Western blot을 이용하여 EMT에 관련된 여러 인자들의 단백질 발현 정도를 검사하였고, 두 약물을 함께 처리한 경우 Snail의 발현이 가장 많이 감소하는 것을 확인하였다. 또한, Wnt/β-catenin과 PI3K/Akt/mTOR 두 신호전달체계 모두에서 중요한 조절자로 알려져 있는 GSK3β의 인산화가 두 약물을 함께 처리시 가장 많이 증가되었다.
결론적으로, 본 연구를 통해 Wnt/β-catenin 신호전달체계가 활성화되어 있는 위암 세포주에서 Wnt/β-catenin과 PI3K/Akt/mTOR 두 신호전달체계를 동시에 억제하는 것이 Snail의 발현을 감소시킴으로써 세포의 침습과 이동을 억제하는데 부가적인 항종양 효과가 있음을 확인하였다. 또한, 두 신호전달체계를 동시에 억제하는 치료적 접근이 Wnt/β-catenin 신호전달체계의 비정상적인 활성화를 보이는 위암에서 침습과 전이를 줄이는 새로운 보조 치료 전략이 될 수 있음을 제시하였다.;Gastric cancer, a highly aggressive malignancy, frequently recurs despite curative surgery and metastasizes to lymph node and distant sites. Epithelial-mesenchymal transition (EMT) is the main phenomenon of cancer progression, including invasion and metastasis. It is regulated by crosstalks between diverse intracellular signaling pathways such as Wnt/β-catenin and phosphatidylinositol-3-kinase(PI3K)/Akt/ mammalian target of rapamycin (mTOR) pathways. Here, this study elucidated the effects of co-inhibition of Wnt/β-catenin and PI3K/Akt/mTOR pathways on gastric cancer.
It is known that Wnt/β-catenin signaling pathways are constitutively activated in two of gastric cancer cell lines, AGS and MKN-28. ICG-001 (β-catenin transcription inhibitor) and rapamycin (mTOR inhibitor) were used for dual blockade, and then the combinatory effects were examined.
As a result, both cell lines were significantly affected by the β-catenin transcription inhibitor, ICG-001, and addition of the mTOR inhibitor, rapamycin induced marginal inhibitory effects on cancer cell proliferation and target gene expression. In addition, invasive activity was 62.3 ± 15.7% and 53 ± 14.4% inhibited by single treatment, but it was 79 ± 13.3% inhibited by co-treatment. Moreover, migratory activity was 33 ± 13%, 42 ± 9% inhibited by single treatment, but it was 82 ± 4% by co-treatment. Interestingly, Snail, one of the important regulatory protein during EMT process, was significantly decreased by co-treatment.
Taken together, these results suggest that co-inhibition of Wnt/β-catenin and PI3K/Akt/mTOR pathways could effectively enhance antitumor effects on gastric cancer via the inhibition of invasion and migration as well as decrease of Snail expression.
