Abstract
본 연구는 중소병원의 간호근무환경 개선 및 간호인력 수급의 문제해결의 기초자료를 제공하기 위한 서술적 조사연구이다. 연구대상자는 2017년 10월 28일부터 11월 13일까지 서울, 경기, 인천의 수도권에 위치한 9개 병원의 3개월 이상 5년 미만 근무한 일반간호사 346명을 대상으로 자료를 수집하였다. 9개 병원은 병상수별로 100-199병상 3개 병원, 200-299병상 3개 병원, 300-399병상 3개 병원이었다. 연구도구는 구조화된 설문지를 사용하였고, 한국어판 간호근무환경측정 도구(Korea Version of the Practice Environment Scale of Nursing Work Index, K-PES-NWI), 직무만족도 측정도구(Minnesota Satisfaction Questionnaire, K-MSQ), 간호사의 이직의도 도구(Korean Nurse Turnover Intention Scale: K-NTIS)를 사용하였다. 수집된 자료는 SPSS v. 23.0 프로그램을 이용하여 각 변수의 실수와 백분율, 평균과 표준편차의 기술통계와 빈도분석, 카이제곱 검정, ANOVA, Scheff'e Test, 다중선형회귀분석을 이용하여 분석하였다. 본 연구의 결과는 다음과 같다. 1. 병동 근무자의 평균 담당 환자 수는 20.71±10.70명이었다. 월 평균 급여는 평균 247.17±32.12만원이었으며, 희망 급여는 298.79±42.52만원으로 나타났다. 신규간호사 교육기간은 평균 31.74±26.39일이었으며, 평균 66.74±55.78일이 적절하다고 생각하는 것으로 나타났다. 2. 대상자 중 51.9%가 1년 이내에 이직할 의사가 있는 것으로 나타났으며, 가장 큰 이유는 낮은 임금수준(37.0%)으로, 중소병원 간호인력 수급문제의 최우선 해결과제는 급여인상(44.6%)으로 나타났다. 직업으로서 간호사를 추천할 의향은 67.5%가 없는 것으로 나타났다. 3. 대상자의 간호근무환경, 직무만족도 및 이직의도를 측정한 결과, 간호근무환경은 4점 척도에서 평균 2.51±0.39점, 직무만족도는 5점 척도에서 평균 3.06±0.45점, 이직의도는 5점 척도에서 평균 3.92±0.58점으로 나타났다. 4. 중소병원 간호사의 간호근무환경과 직무만족도, 이직의도에 영향을 미치는 요인을 파악하기 위하여 다중선형회귀분석을 실시한 결과, 간호근무환경에 가장 큰 영향을 미치는 요인은 신규간호사 교육기간의 적절성(B=-.224, P<.001)이었으며 직무만족도에 가장 큰 영향을 미치는 요인도 신규간호사 교육기간의 적절성 (B=-.187, P<.001)이었다. 이직의도에 영향을 미치는 요인 중 가장 큰 영향을 미치는 요인은 외부교육에 대한 지원(B=.151, P=.033)이었다. 5. 대상자를 병상수별로 구분하여 분석한 결과, 100-199병상의 병원에 근무하는 대상자는 다른 두 그룹에 비하여 연령(36.04±11.95세), 임상실무경력(8.49±7.93년), 이직경험(75.4%)이 높게 나타났으며, 신규간호사 교육기간이 적절하지 않다고 응답한 대상자가 64.2%로 나타났다. 300-399병상의 병원에 근무하는 대상자는 근속연수(2.34±1.37년), 신규간호사 교육기간(37.62±28.90일)이 가장 길게 나타났으며, 1년 이내 이직할 의사가 64.5%로 가장 높게 나타났다. 1년 이내 이직 이유는 100-199병상과 300-399병상의 병원에 근무하는 대상자는 낮은 임금수준이 각각 42.1%, 49.1%로 가장 높게 나타났으나, 200-299병상의 병원에 근무하는 대상자는 불규칙한 근무시간이 26.9%로 가장 높았다. 대상자의 병상수별 구분에 따른 간호근무환경과 직무만족도는 통계적으로 유의한 차이가 없었으나, 이직의도는 통계적으로 유의한 차이가 있는 것으로 나타났으며(F=4.64, p=.010), 병상수가 작을수록 이직의도가 높게 나타났다. 대상자의 간호근무환경, 직무만족도 및 이직의도에 영향을 미치는 요인은 병상수에 따른 유의한 차이는 없었다. 이상의 연구결과를 종합해 볼 때, 중소병원의 병상수가 작을수록 이직의도가 높게 나타났으며 간호근무환경과 직무만족도, 이직의도에 영향을 미치는 요인이 신규간호사의 교육기간, 외부 교육지원과 관련이 있는 것으로 나타났다. 따라서 신규간호사들이 적정수준의 교육을 받을 수 있도록 교육기간에 대한 일관된 업무지침 개발 및 실무중심의 교육, 프리셉터 제도 도입이 필요하다고 사료된다. 또한 표준임금제 도입, 다양한 수당제공 등으로 적절한 보상체계 확립과 적정수준의 담당 환자수 배정, 다양한 근무형태, 법적 간호인력 기준을 지키지 않는 병원에 대해서는 기준을 지킬 수 있는 유인방안을 강구하여, 간호근무환경 개선을 위한 보다 구체적이고 장기적인 관점에서의 대책 마련이 필요하며, 이에 본 연구결과는 중소병원의 효율적인 인적자원 관리를 위한 기초자료가 될 것이다. ;The purpose of this descriptive study is to provide basic data to improve nurses’ work environment and to solve nursing workforce supply and demand problems in small-medium sized hospitals. Data were collected from October 28th to November 13th, 2017 in 346 nurses who have been working for more than 3 months but less than 5 years in total 9 small-medium sized hospitals within those capital areas of Seoul, Gyeonggi and Incheon, Korea. Among these 9 hospitals, three hospitals had 100-199 beds, another 3 had 200-299 beds, and the rest three hospitals had 300-399 beds. A structured questionnaire was handed out. A Korean version of Practice Environment Scale of Nursing Work Index (K-PES-NWI) was used to assess nursing work environment, Modified Minnesota Satisfaction Questionnaire (Korean-MSQ) was included for job satisfaction. Adapted Korean version of Nurse Turnover Intention Scale (K-NTIS) was applied to survey nurses’ turnover intention. Collected data were analyzed with SPSS v. 23.0 program, performed by fact numbers and percentages, descriptive statistics, chi-square test, ANOVA, scheff'e test and multiple linear regression analysis. The results of this study are as follows. 1. The average number of patients in charge was 20.71 ± 10.70 in ward nurses. Average monthly salary was 247.17 ± 32.12 million won, and hope salary was 298.79 ± 42.52 million won. The mean new nurse training duration was 31.74 ± 26.39 days, while the expected duration average was 66.74 ± 55.78 days. 2. 51.9% of the respondents were planning turnover within one year, with a most common reason inadequate salary (37.0%). The top priority of solving supply-demand problems of nursing workforce in small-medium sized hospitals was to increase nurses’ salary (44.6%). 3. Assessment on nursing work environment, job satisfaction and turnover intention shows that the average score of work environment was perceived to be 2.51 ± 0.39 points out of 4, job satisfaction 3.06 ± 0.45 points and turnover intention 3.92 ± 0.58 points out of 5. Results of correlation analysis among nursing work environment, job satisfaction, and turnover intention revealed that nursing work environment and job satisfaction were positively correlated with statistical significance (r=.650, p<.001) while turnover intention had statistically significant negative correlations with both work environment (r=-.174, p<.001) and job satisfaction (r=-.160, p=.003). 4. To filter factors affecting nursing work environment, job satisfaction and turnover intention in small-medium sized hospitals, a multiple linear regression analysis was applied. As a result, adequacy of new nurse training duration was the most effective factor (B=-.224, p<.001) which affects nursing work environment, as well as the most significant effect factor (B=-.187, p<.001) affects job satisfaction. Turnover intention was significantly affected by support for external education (=.151, p=.033). 5. In another analysis, respondents were categorized into 3 different groups according to number of beds that their hospital owns. Nurses from hospitals with 100-199 beds were older (36.04 ± 11.95 years old), with longer clinical working experience (8.49 ± 7.93 years) and more turnover experience (75.4%). A large amount of nurses considered that training duration for new nurses is not appropriate (64.2%). Nurses who were working at hospitals with 300-399 beds had longer working duration for the current hospital (2.34 ± 1.37 years), longer new nurse training duration (37.62 ± 28.90 days), and higher intention of turnover within one year (64.5%). In the case of nurses who have turnover plan within one year, the main reason for turnover intention in both 100-199 beds group and 300-399 beds group was low salary with a respective rate of 42.1% and 49.1%, while the 200-299 beds group had irregular working hours (26.9%) as the most common reason. Though there was no statistically significant difference between nursing work environment and job satisfaction in these 3 groups divided by number of beds, work environment and turnover intention was significantly different (F=4.64, p=.010) and the less beds hospital owned, the higher turnover intention nurses had. There were no significant differences when it came to the factors affecting nursing work environment, job satisfaction, and turnover intention among these 3 groups. The results of this study suggest that in small-medium sized hospitals, the less beds hospital owns, the higher turnover intention nurses have. The factors affecting turnover intention, nursing work environment and job satisfaction is new nurse training duration and support for external education. Therefore, development of standardized new nurse training guide, application of practice-centered and preceptor training regime are required to assure that new nurses can receive training in need. In addition, it is necessary to apply standard wage system, develop variety compensation systems, assure appropriate nurse-to-patient ratio, emphasize hospitals’ responsibility with attractions and rewards as well as construct more specific and long-term countermeasures to improve nursing work environment in small-medium sized hospitals. This study provides basic data for efficient nurse resource management in small-medium sized hospitals.
