Abstract
난소암은 우리나라를 포함하여 전세계적으로 여성의 암 발병률에서 10권 안에 속하고 부인과의 암 중에서는 사망률이 높은 암으로 분류된다. 난소암 환자의 사망률을 높이는 요인으로는 환자들이 비교적 말기에 암 진단을 받고 그로 인한 암 재발 증가로 초기 항암제인 시스플라틴에 대한 저항성을 지니게 된다는 점에 있다. 시스플라틴에 저항성을 지니는 난소암을 치료하기 위해 저항성을 극복하고 나소암을 치료할 수 있는 대체 치료제를 찾기 위한 연구가 진행되고 있다. 본 연구에서는 시스플라틴의 저항성을 극복할 수 있는 치료제로 백화사설초 추출물을 사용하여 난소암 세포에 적용해 보았다. 백화사설초는 중국에서 쓰이던 약초로 염증성 질환과 암의 치료제로 기록되어 있다. 최근에는 유방암과 대장암 등의 암에서의 항암작용이 나타나는 것으로 밝혀졌다. 난소암 세포모델로는 총 6가지 세포를 사용하였고 기전 연구를 위해서는 대표적으로 시스플라틴 민감성 세포인 A2780과 저항성 세포인 A2780cis를 사용하였다. 백화사설초 추출물 중 특히 메탄올로 추출한 메탄올 추출물이 물 추출물에 비해 효과적으로 시스플라틴 민감성과 저항성 난소암 세포 모두를 효과적으로 세포 생존률을 감소시켰고 세포사멸을 야기시켰다. 메탄올 추출물 자체의 처리는 백화사설초의 표준물질인 우르솔릭산과 올레아놀릭산의 처리보다 효과적으로 난소암 세포의 생존률을 감소시켰다. 메탄올 추출물을 시스플라틴과 함께 A2780cis에 처리했을 때 효과적으로 세포 생존률이 감소하였고 A2780과의 세포 생존률 격차도 감소하였다. 시스플라틴 민감성 세포군과 저항성 세포군을 마이크로 어레이 분석하여 후성유전 변화에 관여하는 유전자 중 달리 발현되는 차별발현유전자를 추축출한 결과 히스톤 단백질 조절인자인 KDM1B의 mRNA 발현이 저항성 세포군에서 현저히 높게 나타남을 알 수 있었다. siRNA를 통해 유전자 발현을 감소시킨 후 시스플라틴을 처리한 결과 A2780cis의 세포 생존률이 유의적으로 감소하였다. 메탄올 추출물을 처리한 A2780cis에서도 KDM1B의 유전자 발현이 감소하여 세포 사멸에 KDM1B 유전자의 발현 감소가 관여된다는 점을 도출해냈다. 백화사설초 처리가 시스플라틴 저항성을 지니는 난소암 세포에 가져오는 변화 기전을 알아보기 위해 마이크로 어레이 분석을 진행했다. 그 결과 백화사설초 처리한 A2780cis에서 DNA 회복 유전자인 DCLRE1B의 유전자 발현이 낮게 나타남을 알 수 있었다. 실제로 siRNA를 통해 DCLRE1B의 유전자 발현을 감소시킨 후 시스플라틴을 처리했을 때 A2780cis의 세포 생존률이 유의적으로 감소하였다. 또한 메탄올 추출물을 처리했을 때 DCLRE1B의 유전자 발현이 감소한 것을 확인하여 백화사설초 처리가 A2780cis의 세포 사멸을 유도하는 데 DCLRE1B가 관여됨을 밝혔다. KDM1B와 DCLRE1B 유전자의 발현은 메탄올 추출물을 시스플라틴과 함께 처리한 A2780cis에서 모두 감소하였고 흥미롭게도 siRNA를 통해 KDM1B의 유전자 발현을 방해했을 때 DCLRE1B의 유전자 발현이 감소하였다. 이를 통해 백화사설초의 처리가 효과적으로 시스플라틴 저항성 난소암 세포를 세포 사멸로 이끄는 데 작용하며 그 기전으로는 후생유전학적 지표인 KDM1B와 DNA 회복 유전자인 DCLRE1B의 유전자 발현 감소가 관여된다는 결과를 도출할 수 있다. 이와 같은 결과는 향후 KDM1B의 후생유전학적 조절이 DCLRE1B 유전자에 미치는 직접적인 영향에 대한 기능적 연구를 통해 규명되어야 할 것으로 사료된다. ;Ovarian cancer is the most fatal gynecologic malignancy worldwide with its high risk of late diagnosis and recurrence. Cisplatin is widely used against ovarian cancer, however, recurrent tumors after the treatment frequently demonstrate acquired chemo-resistance resulting in high mortality. Oldenlandia diffusa Roxb (OD), containing ursolic acid and oleanolic acid as standard substances, has been known as a medicinal herb to have effects on lung and stomach cancer. This study was aimed to discover the anti-tumor effects of OD extracts on ovarian cancer cells, in particular carrying cisplatin-resistance. A2780, CAOV3, TOV-112D for cisplatin sensitive cells (CSC) and A2780cis, SKOV3, OVCAR3 for the counterpart cisplatin resistant cells (CRC) were used. The methanol extract of OD (MOD) effectively reduced cell viability than water extract of OD (WOD) in both CRC and CSC and also its effect was greater than that of standard substances. Cell viability was confirmed through flow cytometry, analyzing apoptosis markers in MOD treated A2780 and A2780cis. MOD treatment in combination with cisplatin substantially reduced cell viability of A2780cis. The combination treatment lessens the gap between A2780 and A2780cis upon a single treatment of cisplatin. Integrative analysis of genome-wide gene expression revealed multiple candidate genes that are differentially expressed in CRC compared to CSC. One of epigenetic markers lysine demethylase 1B (KDM1B) was expressed significantly higher than in CSC. siRNA-mediated knock-down of KDM1B made A2780cis to be more sensitive to cisplatin treatment. KDM1B mRNA expression was downregulated upon MOD treatment on A2780cis in a dose-dependent manner. The MOD treated A2780cis showed to significantly repressed expression of DNA Cross-Link Repair 1B (DCLRE1B) according to microarray data analysis. It was validated by quantitative RT-PCR, showing downregulation of DCLRE1B in a MOD-dose dependent manner. Similar to silencing of KDM1B, A2780cis became sensitive to cisplatin treatment upon siRNA-mediated knock-down of DCLRE1B. KDM1B and DCLRE1B both mRNA expression was downregulated in combination treatment of MOD and cisplatin. The co-expression pattern was confirmed by correlation of DCLRE1B and KDM1B expression at cohort data through R2 platform. Next, it was examined whether KDM1B is involved in DCLRE1B-mediated cisplatin response. Using siRNA-mediated knock-down of KDM1B under cisplatin treatment reduced cell viability of A2780cis as well as reduced DCLRE1B mRNA expression. The results showed that the OD extract may effectively promote cell death in the resistance ovarian cancer cells under cisplatin through modulating KDM1B and its target DCLRE1B.
