Abstract
성폭력 피해자를 지원함에 있어서 성폭력 피해 사건의 경중에 따른 차등이 있어서는 안 된다. 지원되어야 할 서비스 항목에는 차이가 있을 수 있지만, 도움이 필요한 대상이라는 피해자로서의 규정과 민감성은 피해 사건의 경중과 상관없이 같은 강도로 유지되어야 한다. 이와 같은 주장을 가지는 이유는 사건의 심각성과 피해자의 고통에 대해서 타인이 판단하거나 결론지을 수 없기 때문이다. 폭력이 동반된 성폭력 피해를 경험한 사람과 일회성 신체접촉의 성폭력 피해를 경험한 사람 중 어떤 사람의 심리적 외상이 더 극심할지에 대해서는 누구도 장담할 수 없다. 폭력이 동반된 성폭력 피해는 사법기관에서 피해자로 인정받을 확률이 높아지며, 주변의 사람들도 시각화된 피해자의 피해를 더 신뢰하고 회복을 지지해 줄 수 있다. 반면 일회성 신체접촉의 경우 증거가 없어 사법기관에서 피해자로 인정받거나 어떠한 안전조치를 신청할 수 있는 자격이 주어지기 어려우며, 주변의 사람들도 대수롭지 않은 피해로 여길 위험이 있다. 이 중 누구의 심리적 외상이 더 심각할지에 대해서 우리는 예측할 수 없으며, 이처럼 성폭력 사건은 심리적 손상을 예측하는 데에 고려해야 할 많은 요인이 존재한다. 여기에서 무엇보다 중요한 것은 피해자가 이 사건에 대해서 어떤 생각을 가지고 있고, 어떻게 의미를 부여하고 있는지에 대한 피해자의 인지와 시각이라고 볼 수 있다. 특히 피해자가 아동‧청소년일 경우 사건의 의미에 대해서 충분히 이해하지 못하거나, 왜곡된 인지를 가지고 있는 경우가 많기 때문에 더욱 세심하고 개별적으로 접근하여야 한다. 이에 본 연구에서는 이와 같은 성폭력 피해 아동‧청소년의 귀인 및 지각의 인지적 요인이 외상 후 스트레스 증상에 유의미한 영향을 미치는지 검증하고자 하였고, 많은 연구자들에 의해 고려되는 성폭력 사건특성과 대처특성이 미치는 영향도 함께 검증하고자 하였다. 조사대상은 만 8세 이상 16세 이하의 아동‧청소년으로 경기지역 성폭력 피해자지원 전담센터에 2014년 12월부터 2016년 8월까지 수집된 2차 자료를 사용하였다. 독립변수는 성폭력 사건특성(피해유형, 피해지속성, 가해자와의 관계, 물리적 폭력 동반여부, 영상 및 사진촬영 동반여부), 대처 특성(폭로까지의 기간, 도움지연), 피해 귀인 및 지각특성(또래와이 이질감, 자기비난, 피불신감, 대인불신), 통제변수는 인구통계학적 특성(성별, 피해연령)으로 구성하였으며, 기술통계, t-test 및 ANOVA, 다중회귀분석을 이용하여 검증하였다. 연구 결과 성폭력 사건특성 중에서는 피해지속성, 피해 귀인 및 지각특성 중에서는 자기비난, 피불신감, 대인불신이 외상 후 스트레스 증상에 영향을 미치는 것으로 나타났다. 이는 피해가 장기간 지속될수록, 성폭력 피해 이후 자기 스스로를 비난하는 자기 귀인과 자신이 다른 사람들로부터 신뢰받지 못하고 있다는 지각, 다른 사람들을 신뢰하지 못하게 되는 지각을 가질수록 외상 후 스트레스 증상의 심각성이 높아질 수 있다는 결과로 해석할 수 있다. 이와 같은 연구결과를 통하여 다음과 같은 실천적 함의를 제시할 수 있다. 첫째, 성폭력 피해 아동·청소년에게 성폭력 통념 및 사회적 인식을 고려한 더 실제적인 전문 치료 프로그램이 개발되고 시행되어야 한다. 더불어 전문 치료 프로그램을 제공하는 치료자 또한 자신의 성폭력 통념과 시각에 대한 통찰을 계속해 나가야 할 것이며, 제공 인력에 대한 교육 또한 병행되어야 하겠다. 둘째, 협의의 범주로는 성폭력 피해 아동‧청소년의 집단 활동 프로그램의 확대가 필요하고, 광의의 범주로는 피해 아동‧청소년의 사회적 네트워크를 강화하고자 하는 개입이 필요하다. 성폭력 피해 아동‧청소년을 지원하는 기관에서는 다양한 실천형태와 개입을 제공하여야 하며, 성폭력 피해는 개별적인 신체‧정신적 외상치료뿐만 아니라 사회적 체계 내에서 피해자가 고립되지 않고 안전한 사회적 관계를 확장시켜나갈 수 있도록 돕는 역할 또한 수행하여야 함을 염두에 두어야 할 것이다. 셋째. 아동‧청소년이 성폭력 피해임을 올바르게 규정하고 자기 비난적 귀인이 없이 인지할 수 있도록 돕는 성폭력 예방교육이 필요하다. 성폭력 피해 아동‧청소년이 잘못 습득된 통념과 인지로 인하여 피해사실을 노출하지 못하는 일이 없도록, 다양한 성폭력의 유형과 발생원인의 이해가 주요하게 다뤄질 수 있는 성폭력 예방교육을 시행해야 한다. 본 연구는 피해자지원 전담센터에 내방한 피해 아동‧청소년의 실제적 자료를 이용하여 성폭력 피해 아동‧청소년의 외상 후 스트레스 증상에 피해 귀인 및 지각이 영향을 미치고 있음을 검증하였다는 것에 의미가 있다. 피해 귀인 및 지각 특성은 안전한 대상과의 상호작용을 통해 얼마든지 수정될 수 있는 개입이 가능한 영역이므로, 위와 같은 실천적 함의가 성폭력 피해 아동‧청소년의 회복을 위한 개입에 활용될 수 있기를 바란다.;In supporting the victims of sexual violence, there should not be any differentiation according to the relative gravity of the incidents of damage from sexual violence. There may be differences in the service items to be supported, but the rule on them and sensitivity to those in need must be maintained at the same intensity regardless of the relative gravity of damage incidents. The reason for this rule is that others cannot judge or conclude the severity of the incidents and the pain of the victims. No one can assure which one’s psychological trauma would be severer, a person who experiences damage from sexual violence accompanied by violence or a person who experienced sexual violence by one-time physical contact. A person who was victimized by sexual violence accompanied by violence is more likely to get recognized as a victim by the judicial authority, and the people around the person, too, can trust visualized victim’s damage more and support the person’s recovery more. In contrast, for one-time physical contact, there is no proof, so it is difficult for the victim to be qualified as a victim by the judicial authority or for application for any safety measures, and the people around the person, too, may be at risk for treating that as a trivial damage. We cannot assure whose psychological trauma would be more serious, and like this, in an incident of sexual violence, there are many factors to be considered in predicting psychological damage. What is most important here is the victim’s perception and perspective, concerning what thought victims have and how they attach meaning. Especially, since most children and adolescent victims did not recognize the meaning of the incident sufficiently or have a distorted perception, they should be dealt more carefully and individually. Thus, this study would verify if these cognitive factors of the attribution and perception of children and adolescent victims of sexual violence have significant impacts on Post-Traumatic Stress Disorder (PTSD) symptoms and verify the characteristics of a sexual violence incident and coping with it considered by many researchers. The subjects of the survey were children and adolescents aged between 8 and 16 and secondary data were used, which had been collected at the Center of Support exclusive for Victims of Sexual Violence in Gyeonggi from December 2014 through August 2016. Independent variables were the characteristics of sexual violence incident (Type of damage, durability of damage, relationship with the perpetrator, accompanying physical violence, accompanying filming and photographing), characteristics of coping (time to exposure, delay of help), characteristics of the attribution and perception of damage(Sense of difference from peers, self-condemnation, sense of being distrusted and distrust of others), control variables were composed of demographic characteristics (age and sex of victims), and they were verified, using descriptive statistics, t-test and ANOVA and multiple regression. As a result of the study, it turned out that the durability of damage, of the characteristics of a sexual violence incident and self-condemnation, the sense of being distrusted and distrust of others, of the characteristics of the attribution and perception of damage affect PTSD symptoms. It can be interpreted that, as the damage lasts for a long time, the more the perception of self-attribution of condemning oneself after damage from sexual violence and the more perceptions of being distrusted by others and distrusting other people, the more the severity of PTSD symptoms may become. Through the above research results, the following practical implications can be suggested. First, it is necessary to develop and implement a more practical, specialized treatment program for children and adolescent victims of sexual violence sexual violence considering common notions and social perceptions. In addition, the therapists who provide the specialized treatment program, too, should keep discerning their own conventional idea and view of sexual violence, and an education for the providers, too, should be provided in parallel. Second, as a category in the narrow sense, it is necessary to expand the group activity program for the children and adolescent victims of sexual violence while as a category in a wide sense, it is necessary to intervene to enhance the social network of children and adolescent victims. The organizations supporting the children and adolescent victims of sexual violence should provide various forms of practice and interventions, and for damage from sexual violence, in addition to individual physical and trauma therapy, the role should be performed in helping expand a safe social relationship, while the victims are not isolated. Third, an education for preventing sexual violence is needed, which can help children and adolescent victims define themselves as the victims of sexual violence correctly and recognize the damage without self-condemning attribution. It is necessary to conduct an education for preventing sexual violence that can primarily deal with the understanding of various types and causes of sexual violence, so that child and adolescent victims of sexual violence may not be able to expose the fact of damage due to the common notion and recognition acquired wrongly. This study has a significance that it verified that the attribution and perception of damage affect PTSD symptoms in children and adolescent victims of sexual violence using the practical data on the children and adolescent victims who visited the Center of Support exclusive for Victims. Since the characteristic of the attribution and perception of damage is an area, which can be corrected in any degree through interactions with safe objects, that is, can be intervened, it is desirable that the above practical implications could be utilized to enhance the support for children and adolescent victims of sexual violence.
