교합인기재로 채득한 교합접촉면적과 덴탈 프리스케일의 교합접촉면적 비교

Abstract

The purpose of the study is to investigate the actual discrepancy between the real occlusion pattern and the date acquired from Dental prescale system by comparing the date from Dental prescale system and occlusal information attained from Polyvinyl siloxane (product name, blu-mousse). The cases in this study include patients of the department of orthodontics in Ehwa Women's University Mokdong Hospital who visited the clinics for the chief complaint of malocclusion from 2012 Feb to 2016 July.The occlusal information attained from Polyvinyl siloxane were digitally scanned and modulated into 3D-images using 7 Series (Impression scanner, dental wings, Montreal, Canada). In addition to the digital modulation, each patient were subsequently subjected to Dental prescale system for the scaling of maximal intercuspation. The date from maximal intercuspation acquired from Dental prescale system was then processed CCD camera (occluzer FDP-709) for the calculation of the total area of occlusion. SPSS(version 23.0, IBM, New York) was used for the statistical calculation. The two data were then compared in terms of the total area of occlusion and classified into anterior, premolar, and molar region. The control group for the comparison was the occlusal information from Polyvinyl siloxane and the experimental group was the occlusal information obtained from Prescale system. The data shows a statistical significance with a result of the experimental group being lesser than the control group in anterior and premolar region. (p<0.05). On contrary, no significant difference has been found from molar region and the regression analysis shows linear relationship; R=0.85, R2=0.72. Dental prescale system shows occlusion pattern in a quantitative way and is a reliable method to evaluate the changes in occlusion pattern during the orthodontic treatment or inter-patient comparison of occlusion pattern. However, the system alone has its limitation in evaluating the entire occlusion pattern and may further require extra method to confirm the occlusion pattern in anterior region.;본 연구의 목적은 치과 치료 시 악간 교합관계를 기록하기 위한 부가 중합형 실리콘 인상재, 폴리비닐실록산 Blu-mousse®(Parkell Inc., Edgewood, NY, USA)를 이용하여 채득한 악간 교합인기와 동일한 환자에게서 채득한 덴탈 프리스케일의 교합접촉점의 분포 및 교합접촉면적을 비교하여 덴탈 프리스케일 시스템이 과연 실제 교합접촉을 얼마나 재현하고 있는가를 보고자 한 실험이다. 본 연구는 2012년 1월부터 2016년 7월까지 이화여자대학교 목동병원 치과진료부에 부정교합을 주소로 내원하여 교정 진단을 받은 환자를 대상으로 하였으며, 부가 중합형 실리콘 인상재를 이용하여 환자가 최대교두감합위로 교합한 상태에서 악간 교합인기를 채득하였다. 채득된 교합 인기는 디지털 스캐너인 7 Series (dental wings, Montreal, Canada)를 이용하여 스캔하여 3차원 이미지로 변환하였다. 또한 동일한 환자에게서 덴탈 프리스케일 시스템을 이용하여 환자에게 최대 교합력으로 압력감지필름을 물도록 지시하여 채득하였다. 환자에게서 채득한 압력감지필름을 CCD camera (occluzer FDP-709, Fuji Film Corp., Tokyo, Japan)에 넣어서 계산되는 면적을 측정하였다. 동일한 환자에서 두 가지 방법으로 채득한 교합접촉면적을 비교하였다. 교합 접촉면적을 전치부, 소구치부, 대구치부로 나누어서 각각 비교하였으며, SPSS(version 23.0, IBM, New York)통계 프로그램을 이용하였다. 폴리비닐실록산 교합인기를 대조군으로 덴탈 프리스케일을 비교군으로 하여 두 군 간의 교합접촉면적을 비교해보았다. 전치부와 소구치부에서는 비교군이 대조군보다 작은값을 나타내었으며, 이는 통계적으로 유의한 차이가 있었다. (p<0.05) 구치부에서는 두 군간 통계적으로 유의한 차이가 없었으며, 회귀분석 결과 R값 0,85, R2 값 0.72의 선형 관계를 보였다.(p>0.05) 덴탈 프리스케일 시스템은 교합을 정량적으로 표시하여 주는 좋은 도구이므로 덴탈 프리스케일을 여러 환자의 교합 상태를 비교하거나 한 환자 내에서의 치료 과정 중 변화하는 교합을 평가하는 수단으로 사용하는 것은 좋은 방법이다. 그러나 이 방법만으로 환자의 교합을 온전히 평가하는 것에는 한계가 있다. 그러므로 덴탈 프리스케일 시스템을 이용하여 교합을 평가하기 위해서는 전방부의 교합을 확인해 줄 부가적인 수단이 필요할 것이다.
