Abstract
ADHD 아동에게는 주의력결핍, 과잉행동, 충동성으로 대표되는 행동적인 증상 외에도 내재화된 우울, 낮은 자존감, 불안, 분노 등의 정서 문제들이 있으며, 이러한 문제들이 지속될 경우 학습장애, 반항장애, 품행장애 등의 공존장애로 이어질 가능성이 높다. 따라서 ADHD를 진단 받은 시점부터 치료적 개입이 중요하며, 이를 위해 약물치료 뿐 아니라 ADHD 아동의 내재화된 정서문제를 다루기 위한 심리 치료적 측면의 통합적 접근이 필요하다. 본 연구는 정서 문제가 있는 ADHD 아동의 미술치료 사례연구로 ADHD 아동의 정서에 미술치료가 어떤 영향을 미치는지 살펴보는데 목적을 두고 있다. 연구 참여자는 정서 문제를 보이는 만 9세의 초등학교 3학년 ADHD 남자 아동이며, 연구자는 2015년 12월 11일부터 2016년 4월 23일까지 주 2회, 60분씩 총 30회기동안 개인 미술치료로 진행했다. 연구를 위한 방법은 질적 사례 연구로서 자료 수집은 미술치료 회기를 진행하여 매 회기 녹음 및 전사한 축어록, 녹화 자료, 치료 일지, 미술작업 및 작품 사진, 연구자의 반성적 노트, 보호자와의 면담을 참고했다. 자료 분석은 축어록과 수퍼비전 자료, 매 회기 아동의 작품, 언어 표현, 행동 반응과 정서 변화를 관찰하여 기록한 자료들을 통해 아동의 정서 변화를 평가 및 분석하였다. 본 연구를 통해 나타난 결론은 다음과 같다. 첫째, 미술치료가 ADHD 아동 내면의 억압된 욕구와 충동성을 건강하게 표출할 수 있도록 도와 아동 스스로 정서를 조절하고 통제하는 힘을 갖게 하였다. 아동은 초기에 자신의 불안이나 긴장을 억압하려는 방어적, 공격적인 태도를 보였지만 문제 해결의 다양한 방법들을 탐색해가며 자신의 정서를 조절할 수 있게 되었다. 둘째, 미술치료가 ADHD 아동의 낮은 자존감에 긍정적인 영향을 미쳤다. 미술활동을 통한 성공 경험으로 아동은 자발성과 잠재력을 발휘하여 자신에 대한 긍정적인 인식을 할 수 있게 되었다. 셋째, 그림이야기 검사(DAS)를 통해 사전, 사후 검사에서 아동의 정서문제와 자아상, 유머의 변화가 나타났다. 사전 검사에서는 수동적이고 소극적인 자아상과 적대적인 관계, 불안과 두려움의 정서를 나타냈다면 사후검사에서는 문제를 극복하고 소망을 성취하는 강하고 긍정적이며 유머러스한 자아상의 모습으로 변화하였다. 이처럼 본 연구는 미술치료가 ADHD 아동의 정서 문제에 긍정적인 영향을 미치는 것을 살펴보아 ADHD 아동을 이해할 수 있는 기초자료를 제공했다는 데 그 의의가 있다.;Children with ADHD shows characteristic behavioral symptoms such as attention deficit, hyperactivity, and impulsiveness, but also have internalized emotional problems like depression, low self-esteem, anxiety, and anger and such problems has high risk of leading to comorbidity such as learning disorder, oppositional defiant disorder, and conduct disorder. Therefore, as therapeutic intervention from the time of being diagnosed ADHD is crucial and for the therapy, , integrative approach of therapy as well as drug therapy and cognitive-behavioral therapy are necessary to handle internalized emotional problem in ADHD children. This study has its purpose on studying the influence of art therapy in alleviation of emotional problem in ADHD children and reviews cases of art therapy that provides opportunity for ADHD children to express their emotions. Participants in the study were 9-year-old ADHD boys in 3rd grade of elementary school and experimenter conducted 30 single art therapy sessions of 60 minutes, twice a week from December 11th, 2015 to April 23rd, 2016. The method of this study was qualitative case study and for collection of data, recording of each session and its transcript, video, therapy diary, supervision material, photos of art activity and artwork, retrospective note by researcher, and interview with guardian were used for reference. Analysis of data was collective analysis on emotional changes and evaluation of the children based on recording of consult transcript, supervision material, children’s work in each session, verbal expression, behavioral response and emotional changes. Results of this study is as following. First, at therapy has helped ADHD children to relieve desires and negative emotions inside then and experience positive emotions. In initial stage of therapy, the children attempted to control their anxiety and tensions, but as the therapy progressed, they explored and found various methods of coping with their problems. Also, completing their art works over several sessions had them experience sense of accomplishment. Second, art therapy showed positive influence in interaction and conversation of ADHD children. With interaction with therapist, he children could relieve tension on new media and have confidence. Positive feedback on the children allowed children to overcome renunciation and frustration and conduct the art work consistently. Stable environment of therapy with consistent support and attention helped overcome insecure emotions and foster emotional stability. Third, positive change has been observed with pre-and post- Draw-A-Story (DAS) test. In the preliminary test, the children showed negative ego, hostile relations, frustration, and unrealism, whereas the children changed into an ego that apprehended reality, overcame obstacles, and fulfilled their desires. Therefore, this study concludes that art therapy instigated positive influence in alleviation of emotional problems in ADHD children and has its significance in that it has proposed a constructed plan for treatment of ADHD children with emotional problems by providing a detailed and live reference on interaction between therapist and the children of the study.
