dc.description.abstractPropofol is an agonist of the gamma-aminobutyric acid A (GABAA) receptor and an antagonist of the N-methyl-D-aspartate receptor (NMDAR), and is widely used as an intravenous anesthetic. The action of propofol at the NMDAR modifies long-term potentiation (LTP), which is a long-lasting increase in the strength of the synapse that plays an important role in learning and memory function in the hippocampus. Propofol also affects synaptic plasticity via the depression of postsynaptic proteins such as calcium/calmodulin-dependent protein kinase II (CaMKII) and postsynaptic density protein 95 (PSD-95). Recent studies have reported that propofol is neurotoxic in developing brains that are still undergoing synaptogenesis, whereas other studies have shown that propofol protects brain and cognitive function under conditions of ischemia and may be related to changes in autophagic action on neuronal cells. Autophagy is a process that removes cytoplasmic substrates and protects cells from stressors such as ischemia or starving. Impairments in autophagy can cause neurodegenerative diseases such as Alzheimer’s disease and Parkinson’s disease. Thus, this study utilized behavioral experiments and protein analyses to investigate the effects of propofol on the hippocampus and memory function in adult mice that had completed synaptogenesis. In addition, changes in autophagy were evaluated in adult mice and Amyloid β-protein fragment 25–35 (Aβ25–35) treated organotypic rat hippocampal slice cultures (OHSCs). This study included 75 GFP-LC3 and C57BL/6 adult mice that were 2–11.4 months in age, weighed between 20 g and 30 g, and received intraperitoneal injections of either intralipid (sham control) or propofol. Propofol was administered via three methods: a single injection of an induction dose (150 mg/kg; n = 24), a single injection of a double induction dose (300 mg/kg; n = 6), or a dose for long-duration anesthesia over 2 h (375 mg/kg in total; n = 22). The sham control (n = 23) received intralipid injections that were the same volume as the corresponding propofol doses. For behavioral testing, the mice completed a Y-maze on days 1, 3, and 7 after being anesthetized, and the percentages of alternating arm entries were compared among groups. Following the Y-maze tests, the mice were sacrificed, their hippocampi were harvested, and Western blot, immunofluorescence staining, and transmission electron microscopic (TEM) analyses were performed. In addition, OHSCs were used to investigate the effects of propofol on Aβ25–35-treated cultures. For this procedure, the hippocampi of 8-day-old Sprague-Dawley rats were isolated, cut into transverse slices (400 μm), and cultured for 2 weeks. Aβ25–35 was prepared at a final concentration of 2.5 μM and applied to the culture medium for 3 days. Then, the cultures were treated with propofol at a final concentration of 20 μM for 6 h with or without Aβ25–35 in the culture medium. Finally, Western blot analyses and immunofluorescence staining procedures were performed. The mice that received a single induction dose of propofol did not significantly differ from the control in either the Y-maze test r protein analyses. After injection of a double induction dose, all of the mice died without recovering from the anesthesia; therefore, the long-duration anesthesia dose was used for all of the further experiments. Compared to the control, there was a significantly lower percentage of alternation on day 3 after long-duration propofol anesthesia but this deficit recovered on day 7. Western blot analyses showed that the levels of p-CaMKIIα and PSD-95, which are memory-related proteins, significantly decreased compared to the control on day 3 but recovered on day 7. Immunofluorescence staining revealed similar results. In terms of autophagy-related proteins, there were higher intensities of microtubule-associated protein light chain 3 (LC3), phosphatidylethanolamine (LC3-II), and p62 on day 3 and these immunoreactivities were highly co-localized. However, on day 7, the increase in LC3-II had recovered but the increase in p62 was sustained, and there was less co-localization with p62 as determined by immunofluorescence staining, which was indicative of autophagosome degradation. Quantitative analyses of the lysosomal proteins LAMP2 and cathepsin D revealed that they had higher intensities on day 3, and that there was a sustained increase in LAMP2 on day 7. In addition, the ultrastructure of the long-duration group showed cellular autophagic vacuoles (AVs) on day 3 but no specific AVs on day 7. The enhancement of autophagic induction was confirmed in the OHSCs based on increased levels of LC3-II and p62 following treatment with Aβ25–35 alone, and decreases in LC3-II and p62 expression were evident when propofol was administered in conjunction with Aβ25–35. These results indicate that propofol resulted in the degradation of autophagosomes. Cathepsin D increased in all of the slice cultures compared to the control. There was a trend towards a significant increase in LAMP2 expression compared to the control with propofol, and propofol plus Aβ25–35 treatments, while there was only slight increase without significance in treatment with Aβ25-35 alone. This implies that propofol increased the amount of autolysosomes. Immunofluorescence staining revealed less LC3-positive puncta and more cathepsin D-positive and LAMP2-positive puncta, with high levels of co-localization in OHSCs treated with Aβ25–35 and propofol compared to treatment with Aβ25–35 alone. These results suggest that propofol treatment enhanced autophagic flux. The results of this study demonstrated that, although transient memory deficits occurred, adult mice recovered memory function after long-duration propofol anesthesia. The molecular mechanisms underlying these memory deficits may involve changes in NMDAR-dependent signaling pathways regulated by p-CaMKIIα and PSD-95. In terms of recovery, propofol-induced enhancement in autophagic flux, which occurred simultaneously, may have compensated for the induction of autophagy by propofol. The present findings may also support the clinical safety of propofol anesthesia in adults.;Propofol은 gamma-aminobutyric acid A (GABAA) receptor agonist이자 N-methyl-D-aspartate receptor (NMDAR) antagonist로서, 전신마취제로 널리 사용되는 약제이다. Propofol의 NMDAR에 대한 작용은 해마에서 long-term potentiation (LTP)이라 불리는 학습과 기억에 관계하는 지속적인 시냅스 연결의 강화 작용을 변화시키는 것으로 알려져 있다. 또한, CaMKII, PSD-95와 같은 시냅스 후 신경세포에 있는 단백질을 감소시켜 신경전달에 영향을 준다. 때문에 propofol이 시냅스 형성이 완전하지 않은 발달단계의 뇌에서 신경독성을 나타낸다는 최근 연구들이 있다. 그러나 허혈성 조건에서 뇌를 보호하고 인지기능을 보호한다는 연구결과도 있어 이는 propofol이 신경세포의 자가포식 과정에 미치는 영향이 관계가 있는 것으로 보인다. 세포내의 불필요한 단백질을 세포자체 내에서 분해시키는 자가포식 작용은 세포의 허혈이나 굶주림과 같은 스트레스 상황에서 세포를 보호하는 과정으로서, 알츠하이머나 파킨슨과 같은 신경퇴행성 질환에서 자가포식 과정에 이상이 있다는 연구결과들이 있다. 이에 우리는 이러한 propofol이 신경발달이 끝난 성체(adult)에서는 어떤 작용을 보이는지 알아보고자, 성체 생쥐에서 기억력에 관한 행동실험을 시행하였고 이들 쥐의 해마를 채취하여 분석하였다. 동시에 해마에서 일어나는 자가포식의 변화를 성체 생쥐와 Aβ25-35를 처리한 흰쥐의 해마조직절편배양에서 살펴보았다. 실험대상으로 75 마리의 월령 2 ~ 11.4 개월(무게 20 ~ 30 g)에 해당하는 GFP-LC3 transgenic (TG) 성체 생쥐를 사용하였으며 이들에게 propofol 또는 대조군으로서 intralipid를 각각 복강내 주사하였다. Propofol은 세가지 용량으로 투여하였으며 마취유도용량(total n = 24), 마취유도용량의 2배(n = 6), 장시간마취를 위한 용량(total n = 22)으로 나누어 투여하였다. 대조군에는 해당 propofol 용량과 동일한 부피의 intralipid를 투여하였다 (n = 23). 마취후 1, 3, 7일에 Y-maze 행동실험을 하여 교체행동비율(percentage of alternation)을 비교하였으며 실험 후 희생시켜 해마를 채취한 뒤에 단백질 분석, 면역형광염색 및 전자현미경 분석을 시행하였다. 또다른 실험으로서, Aβ25-35를 처리한 해마조직절편배양에서의 propofol의 영향을 보기위해, 생후 8일된 Sprague-Dawley 흰쥐의 해마를 채취하여 400 µm  두께로 자른 절편을 2주간 배양한 뒤, 2.5 µM의 Aβ25-35를 배양액에 첨가하였다. 이를 3일간 더 배양한 후, 최종 propofol의 농도가 20 µM이 되도록 propofol을 배양액에 추가하여 6시간 추가배양한 뒤 단백질 분석, 면역형광염색을 시행하고 대조군과 비교하였다. GFP-LC3 TG 생쥐에서 마취유도용량을 투여했을 때, 대조군과 행동실험 및 단백질 분석에서 유의한 차이를 보이지 않았다. 마취유도용량의 2배를 투여한 쥐는 마취 후 회복되지 않고 모두 사망하였다. 이에 이후 실험에서 장시간 마취용량을 사용하여 실험하였다. 장시간 마취를 위한 용량을 투여하였을 때, 마취 후 3일째에 Y-maze 행동실험에서 교체행동비율이 유의하게 감소하였으나 7일째에 시행한 실험에서 회복되었다. 이에 따라 기억력과 관계된 단백질인 PSD 95, p-CaMKIIα를 분석하였을 때 Western blot 단백질 정량분석에서 3일째에는 감소하였지만 7일째에는 다시 회복되는 결과를 보였다. 면역형광염색에서도 비슷한 결과를 보였다. 자가포식의 유도와 관련된 단백질 분석검사에서는 3일째에 LC3 II와 p62가 증가하고 면역형광염색에서도 함께 겹쳐보이는 양상을 보였다. 7일째에 LC3 II 증가는 회복되었으나 p62 증가는 유지되었다. 하지만 면역형광염색에서 두 단백질이 겹치지 않고 따로 보여 자가포식체의 분해를 추측할 수 있었다. 자가포식에서 마지막에 단백질이 분해되는 과정과 관련된 단백질인 LAMP-2, cathepsin D의 경우, 3일째에 증가하였고 7일째에는 감소하는 추세였으나 cathepsin D는 대조군과 같은 수준까지 감소함에 반해 LAMP-2는 대조군보다는 7일째에도 유의하게 증가한 수준이었다. 전자현미경 검사를 통한 세포의 미세 구조를 보았을 때 3일째에는 자가포식 공포(autophagic vacuole, AV)들을 관찰할 수 있었으나 7일째에는 대조군과 같이 이들 AV가 보이지 않았다. 해마조직절편배양 실험에서 LC3 II와 p62는 Aβ25-35만 사용한 배양에서 증가하여 자가포식 유도를 확인할 수 있었고, Aβ25-35와 propofol을 함께 투여한 배양에서는 Aβ25-35만 처리한 배양에서보다 감소하여 자가포식체의 분해를 추측하였다. Cathepsin D는 모든 배양에서 증가하였고 LAMP2는 Aβ25-35만 사용한 배양에서는 대조군과 차이가 없었고 propofol을 단독 처리한 배양과 Aβ25-35와 propofol을 함께 투여한 배양에서 대조군보다 증가한 양상을 보여 autolysosome의 증가를 생각할 수 있었다. 면역형광염색에서도 Aβ25-35만 사용한 배양에서는 LC3 양성 입자가 많이 보인 반면 propoofol이 첨가된 배양에서는 cathepsin D와 LAMP2의 색이 더 진하게 겹쳐보였다. 이에 따라 propofol에 의해 자가포식의 흐름이 활성화 되었음을 알 수 있었다. 결론적으로 성체 생쥐는 propofol을 이용한 장시간의 마취 후의 일시적 기억력 감소를 회복할 수 있는 능력이 있다. NMDAR에 위치한 PSD 95와 p-CaMKIIα를 통한 신호 전달 체계의 변화가 일시적 기억력 감소와 관련이 있고 자가포식 작용의 유도가 증가하지만 단백질 분해까지 원활하게 이루어짐으로써 유도의 증가를 보상할 수 있어 기억력이 회복되는 것으로 보인다. 이와 같은 결과는 성인에서 propofol의 임상적인 안전성을 뒷받침할 수 있을 것으로 생각된다.-
