Abstract
Sedation is commonly used for endoscopy, simple surgeries and medical imaging. However, no research has been done on how risky each of the general population and physicians perceive sedation as, bringing difficulty to decision-making. A survey was conducted on adult residing in Seoul and Gyeonggi province and physicians working in hospitals of Seoul and Gyeonggi province. A 5 point scale was used to check overall sedation risk. A total of 1738 members of the general population and 649 physicians completed the survey. Among the surveyed general population, 58.4% had received sedation before. There was significant difference in the overall sedation risk perception between the two groups (13.8% versus 12.6%). It was more likely for the general population to perceive sedation as risky when they were age < 40 years (OR 2.27, P < 0.001), female (OR 1.62, P = 0.002), married (OR 1.62, P = 0.004), self-reported unhealthy (OR 1.47, P = 0.019), had previously experienced sedation with adverse events (OR 1.79, P = 0.049), had discomfort experience during the target procedure (OR 2.08, P = 0.008), and did not have sufficient explanation about the sedation (OR 2.05, P < 0.001). In case of physicians, the probability to perceive sedation as risky was high for anesthesiologists (OR 4.90, P < 0.001), and for physicians who encountered adverse events during the procedure (OR 1.73, P = 0.031). The general population was more likely to worry about awakening or inadequate sedation, delayed emergence, hypoxic brain damage and death than physicians (P < 0.001), and to perceive these adverse events as more frequent as compared to physicians perceiving them (P < 0.001). Understanding general population's risk perceptions of sedation and identifying factors affecting risk perception will help physicians to manage the patients in accordance with individual patient's characteristics, beliefs and concerns.;서론 수면마취는 내시경이나 간단한 수술, 영상 촬영에 흔히 이용되는 마취 방법이다. 하지만 일반인이나 의사 집단이 각각 이를 얼마나 위험하게 인식하는지 조사가 이루어진 적이 없어, 환자의 의사 결정 과정에 어려움이 존재한다. 대상 및 방법 서울, 경기 지역에 거주하는 성인 남녀와 서울, 경기 지역의 병원에 근무하는 의사를 대상으로 설문 조사를 하였다. 개인의 특성과 이전 수면마취 경험에 대해 물은 뒤, 5점 척도를 이용하여 전반적으로 수면마취를 얼마나 위험하게 생각하는지, 각각의 부작용에 대해 얼마나 위험하게 생각하는지 등을 조사하여 각 집단의 위험도 인식 정도와 집단 간 위험도 인식의 차이를 평가하였다. 결과 1738명의 일반인과 649명의 의사가 설문에 답하였다. 일반인의 58.3%가 이전에 수면마취를 받아본 적이 있었으며, 이 가운데 7.8%가 부작용을 경험했다고 답했다. 일반인의 13.8%, 의사의 12.6%가 수면마취를 ‘매우 자주 위험' 또는 '항상 위험' 으로 인식하였다. 일반인에서 40세 이하(OR 2.27, P < 0.001), 여성(OR 1.62, P = 0.002), 기혼(OR 1.62, P = 0.004), 스스로 건강하지 않다고 생각할 때(OR 1.47, P = 0.019), 이전 수면마취 중 부작용을 경험한 경우에(OR 1.79, P = 0.049) 수면마취를 더 위험하게 인식할 가능성이 높았다. 또 수면마취를 하고 한 시술이 불편했던 경우(OR 2.08, P = 0.008), 시술을 받기 전 시술에 대한 설명을 충분하게 듣지 않았다고 생각할 경우(OR 2.05, P < 0.001), 위험하게 인식할 가능성이 더 높았다. 의사에서는 마취통증의학과 의사일 경우(OR 4.90, P < 0.001), 시술을 시행하는 도중 환자에서 부작용이 나타났을 때 (OR 1.73, P = 0.031) 더 위험하다고 인식했다. 일반인은 의사에 비해 부작용 항목 가운데 마취 중 각성, 의식 회복 지연, 뇌손상, 사망을 더 걱정하였으며(P < 0.001), 이러한 부작용이 더 자주 일어난다고 답했다(P < 0.001). 결론 각 개인의 특성과 이전 수면마취 경험에 따라 수면마취 위험도 인식 정도가 달랐다. 또 일반인 집단과 의사 집단에서도 위험도 인식에 차이가 나타났다. 의사는 환자 개인의 특성과 경험에 맞추어 수면마취에 대한 설명을 함으로써 환자와 의사 모두에게 도움이 될 것이다.
