Abstract
Background Trastuzumab (Herceptin○R) is a monoclonal antibody used for perioperative anti-cancer therapies of patients having HER2 (Human Epidermal Growth Factor Receptor 2, ErbB-2)-positive breast cancer and used for anti-cancer therapies of patients having metastatic breast cancer prior to breast cancer therapy. Like most of monoclonal antibodies, it is reported that trastuzumab has infusion-related reactions including fever, chills, headache, and nausea. Even though trastuzumab has few side effects and these infusion-related reactions can be easily treated, fatal infusion-related reactions such as anaphylaxis and bronchospasm are also reported rarely resulting from trastuzumab. Moreover, it is difficult to estimate seriousness of reactions unless it starts. However, current medical treatment guidelines as well as paper insert do not require premedication drugs for trastuzumab infusion. As a result, premedication drugs are prescribed under clinician’s judgement. In Korea, there is no research addressing risk factors and side effects of infusion-related reactions of trastuzumab. Objectives Although premedication drugs is not required for trastuzumab uses, Samsung Seoul Hospital hemato-oncology outpatient department infuses 4mg of chlorpheniramine as premedication drugs in order to minimize infusion-related reactions of initial trastuzumab uses. By studying infusion-related reaction incidence for first infusion of trastuzumab with antihistamin premedication drugs, chlorpheniramine, to adult breast cancer patients, this study analyzes infusion-related reaction incidence and analysis risk factors, finally find appropriate treatment methodologies. Methods This study investigates patients of Samsung Seoul Hospital hemato-oncology outpatient department over 18 years old who have breast cancer and are infused trastuzumab for the first time between January 1st, 2013 and February 28th, 2014. After collecting and investigating treatment information such as clinical information of patient group, prescribed capacity of trastuzumab, infusion frequency, infusion time, concurrent anti-cancer therapy drugs in combination, premedication drugs in combination, and trastuzumab treatments, it is analyzed statistically addressing significance with infusion-related reactions. Symptoms resulting from infusion-related reactions and treatment methodologies are investigated and developed using retrospective studies based on the given electronic medical record. Results Infusion-related reaction developments are observed 33 times out of 2963 infusions (1.11%). Specifically, most of developments, 32 times which is 17.1% of patients who receiving trastuzumab, are observed when initial infusion. From the univariate regression analysis and the multi-variable logistic regression analysis by varyng clinical information of patients, anti-cancer drugs in combination with trastuzumab, and premedication drugs, a meaningful staistical variable is not found. After investigating symptoms and strengths of side effects based on Common Terminology Criteria for Adverse Events (CTCAE) Version 4.03 of National Cancer Institute, developed side effects are considered as grade 1 (8 patients, 24.2%) and grade 2 (25 patients, 75.8%) without severe side effect developments. Conclusion The proposed approach is effective compared to infusion-related reactions of the product manual(paper insert). The uses of an antihistamine medicine(chlorpheniramine) prior to trastuzumab result to reduction of infusion-related reactions. In addition, no severe side effect of infusion-related reaction is developed. Furthermore, when analyzing risk factors associated with infusion-related reactions for trastuzumab treatment there is no statistically meaningful variables. As a result, infusion-related reactions are not correlated with risk factors and they develop depending on immunological uniqueness of patients. Therefore, to confirm medical history and allergic reactions to drugs of patients is required in order to prevent potential infusion-related reactions while the uses of premedications is required to patients who have infusion-related reactions in the past.;배경 Trastuzumab (Herceptin○R)은 HER2 (Human Epidermal Growth Factor Receptor 2, ErbB-2) 과발현 유방암 환자의 수술 전후 항암요법, 전이성 유방암의 항암요법에 사용되는 유방암 치료의 전 단계에서 사용되는 단일클론항체(monoclonal antibody)이다. 대부분의 단일클론 항체가 주입 관련 반응(infusion related reaction)과 관련이 있는 것처럼 trastuzumab 역시 발열, 오한, 두통, 오심 등의 주입 관련 반응이 보고 되고 있다. Trastuzumab의 주입 관련 반응은 대개는 부작용의 발현이 적고 비교적 치료가 쉽지만 드물게 아나필락시스, 기관지 경련 등 치명적인 주입 관련 반응이 보고되었고 실제 주입 관련 반응의 심각도는 증상이 시작되기 전까지는 알 수 없다. 그러나 약물 설명서를 포함한 의료지침에는 단독으로 trastuzumab을 사용하는 경우 전처치 약물이 필수적이지 않아 현재 임상의의 판단에 따라 전처치 약물을 처방하고 있다. 또한 국내에서는 trastuzumab의 주입 관련 반응의 부작용 현황과 위험 인자 분석에 대해 연구된 바가 없다. 목적 현재 단독으로 trastuzumab을 사용하는 경우 전처치 약물이 필수적이지 않으나 삼성서울병원 혈액종양내과 외래에서는 trastuzumab 초회 투여시 주입 관련 반응을 줄이기 위해 chlorpheniramine 4mg을 전처치 약물로 정맥 투여하고 있다. 따라서 본 연구는 성인 유방암 환자에게 trastuzumab 초회 투여시 전처치 약물로 항히스타민제제인 chlorpheniramine을 투여했을 경우의 주입 관련 반응의 발현을 분석하고 위험 인자를 파악하여 적절한 관리 방안을 모색하고자 한다. 연구 방법 삼성서울병원 혈액종양내과에 내원하는 18세 이상의 성인 암환자로 유방암 진단을 받고 2013년 1월 1일부터 2014년 2월 28일까지 처음 으로 trastuzumab을 정맥 투여 받은 환자를 대상으로 환자군의 임상 정보와 trastuzumab처방 정보, 병용 항암제와 전처치 약물, trastuzumab의 치료 방법 등에 대해 조사한 후 주입 관련 반응과의 유의성을 통계적으로 분석하였다. 주입 관련 반응의 증상과 관리 방법 등에 대해서 전자의무기록을 통해 후향적으로 조사하였다. 결과 대상환자 187명의 전체 투여 건수는 2,963회였으며 주입 관련 반응은 33건(1.11%)이었다. 이 중 초회 투여시 주입 관련 반응의 발생률이 32건으로 trastuzumab 투여의 17.1%에 해당하는 환자에게 주입 관련 반응이 발생하였다. 단변량 분석과 다변량 로지스틱 회귀분석으로 환자의 임상정보, trastuzumab과 병용한 항암제, 전처치한 약제와 주입관련 반응의 유의성을 분석하였을 때 통계적으로 유의하지 않았다. National Cancer Institute의 Common Terminology Criteria for Adverse Events (CTCAE) version 4.03에 의한 부작용의 증상과 강도를 분석하였을 때 부작용은 grade 1인 환자가8명(24.2%), grade 2인 환자가25명(75.8%)이 발생되었고 심각한 부작용은 없었다. 고찰 및 결론 Trastuzumab 초회 투여시 항히스타민제(chlorpheniramine)로 전처치한 후 발생한 주입 관련 반응은 제품 설명서에 안내된 주입 관련 반응에 비해 효과적으로 감소되어 전처치 투여가 효과적이었으며 주입 관련 반응의 부작용도 강도 높은 심각한 부작용의 발현은 없었다. 또한 trastuzumab의 주입 관련 반응은 위험 인자를 분석하였을 때 통계적으로 유의한 인자가 없었으므로 위험 인자와의 관련성이 적고 환자 고유의 면역학적인 특성으로 인해 주입 관련 반응이 발생할 것으로 고려되며 이에 따라 환자의 병력과 약물에 대한 알러지 반응 등을 확인하여 향후 발생할 수 있는 주입 관련 반응을 적극적으로 예방하고 과거 병력이 있는 환자에게는 적극적인 전처치 약물의 사용으로 주입 관련 반응을 예방하도록 하여야 할 것이다.
