Abstract
목적: 궤양성 대장염과 크론병으로 대표되는 염증성 장질환은 만성적인 질환으로 재발과 악화를 반복하여 환자의 삶의 질을 악화시킨다. 줄기 세포 치료는 최근 염증성 장질환을 비롯한 다양한 난치성 질환의 잠재적인 치료법으로 각광받고 있다. 새롭게 개발된 편도 유래 중간엽 줄기 세포(tonsil-derived mesenchymal stem cells, T-MSC)는 줄기 세포 고유 기능을 모두 가지며 비교적 채취가 용이하고 여러 기증자로부터 받은 세포가 잘 융화되어 자란다는 장점이 있다. 본 연구에서는 이러한 T-MSC를 dextran sulfate sodium (DSS)로 유도된 만성 장염 마우스 모델에 적용하여 그 효과를 알아보고자 하였다. 방법: C57BL/6 마우스를 대상으로 급성 및 만성 장염을 유도하였다. 편도 절제술을 받은 환자들에서 채취된 T-MSC는 유도된 장염 모델에 복강 내 투여 또는 정맥 투여 방법으로 주입하여 그 효과를 정상 대조군 및 DSS 장염 대조군과 비교하였다. 육안적 장염의 평가는 disease activity index (DAI)와 장길이를 측정하여 비교하였으며 조직학적인 분석은 histologic colitis scoring system을 따랐다. 대장 조직의 IL-6, TNF-α, IL-10을 측정하여 비교하였다. 투여 방법을 정맥 내 투여 방식과 복강 내 투여 방식으로 비교하였으며 투여 용량은 1x106, 2x106, 5x106 세가지 용량으로 투여하여 비교하였다. 투여 간격에 따른 효과를 비교하기 위하여 실험 6일, 16일에 투여하는 군과 투여 간격을 짧게 하여 실험 6일, 9일, 12일, 16일에 투여하는 군을 비교하였다. T-MSC의 효과를 높이기 위하여 Interleukin-1β (IL-1β)로 전처치를 시행하여 차이가 있는지 비교하였다. 결과: HEK 장염 대조군은 장염 유도한 30일 동안의 평균 DAI가 4.4 ± 1.7 mm로 증가되어 있었으며 평균 장 길이가 64.6 ± 7.13 mm 로 정상 대조군에 비하여 짧아져 있었다. T-MSC 투여에 의하여 평균 DAI는 2.9 ± 1.0 로 유의하게 감소된 소견을 보였으며(p < 0.01) 장 길이 단축도 유의하게 완화되었다(60.4 ± 5.1 vs. 67.5 ± 6.2 mm, P < 0.05). 장염 유도 30일 동안 DAI가 T-MSC 투여군에서 HEK 장염 대조군에 비하여 지속적으로 높았으며 이 차이는 실험 18일경에 가장 극대화되었다. 장염 유도 마지막 날인 30일 째에 채취한 조직의 조직학적 점수는 HEK 장염 대조군에 비하여 T-MSC 투여군에서 낮았으나 통계적인 유의성은 없었다. 염증성 사이토카인을 분석하였을 때 IL-6가 T-MSC 투여군에서 유의하게 낮았으며(36.2 vs. 1.72, p < 0.05). TNF-α, IL-10은 유의한 차이를 보이지 않았다. T-MSC의 용량에 따른 효과 차이는 없었으며 오히려 1x106 T-MSC를 투여한 군에서 DAI가 가장 낮았다. 투여 간격이 긴 군에 비하여 투여 간격을 짧게 하였을 때 DAI도 더 낮고 장 길이도 더 긴 경향을 보였으나 통계적 유의성은 없었다. IL-1β로 전처치를 시행한 T-MSC의 효과 증가도 명확하지 않았다. 결론: T-MSC는 DSS로 유도된 급성과 만성 장염 마우스 모델에서 DAI와 장 길이 단축의 육안적 장염을 호전 시키고 IL-6의 염증성 사이토카인 수치를 낮춰 항염증 효과를 보여주었다. T-MSC는 염증성 장질환의 새로운 치료 물질로서의 가능성이 있으며 추후 T-MSC 만성 장염 모델에서의 작용 기전에 대한 연구를 포함하여 더 많은 개체 수에서 발전된 연구가 필요할 것으로 보인다.;Background/Aims: Stem cell therapy has come into the limelight as a potential therapeutic approach for various diseases, including inflammatory bowel disease (IBD). Tonsil-derived stem cells (T-MSCs), a newly developed source of MSCs, have many advantages including retention of stemness, high differentiation capacity and shorter doubling time. Here, we examined the therapeutic effect of T-MSCs in a mouse model of IBD. Methods: Acute and chronic colitis was induced in 7-8-week-old C57BL/6 mice. For the induction of acute colitis, the mice were treated with 3% DSS (dextran sulfate sodium) for 7 days. T-MSCs were administered to the mice via the intraperitoneal or intravenous route on day 3 of DSS treatment. For the induction of chronic colitis, the mice were treated with 1.5% DSS for 5 days, followed by 5 days of drinking water continuously for three cycles (total of 30 days). T-MSCs were administered on days 6 and 16 of DSS treatment. Colitis severity was assessed daily by determining the disease activity index (DAI), colon length, histologic grading and levels of pro-inflammatory cytokines (IL-6, TNF- α, and IL-10). We compared various options for optimizing the T-MSC treatment. We evaluated the dose-dependent effect of T-MSCs by administering 1x106, 2x106 and 5x106 cells to the mice. T-MSCs were injected on days 6 and 16 of DSS treatment (long interval) and on days 6, 9, 12 and 16 of DSS treatment (short interval). Pretreatment with IL-1β was also evaluated. Results: Mice with DSS-induced chronic colitis (HEK colitis group) showed an elevated mean DAI (4.4 ± 1.7 mm) and shortened mean colon length compared to the normal control group. After T-MSC treatment, DAI was significantly reduced (4.4 ± 1.7 vs. 2.9 ± 1.0; p < 0.01) and colon shortening was lessened (60.4 ± 5.1 vs. 67.5 ± 6.2 mm; p < 0.05) compared to the HEK colitis group. The DAI of the T-MSC-treated group was continuously lower than that of the HEK colitis group throughout the 30 days of colitis induction; the difference was maximal at day 16 of colitis induction. The histopathologic score was not significantly different. The IL-6 level was significantly lower in the T-MSC-treated group compared to the HEK colitis control group (36.2 vs. 1.72; p < 0.05). TNF-α and IL-10 levels were not different between the two groups. No dose-dependent effect of T-MSCs was evident. In the group treated with T-MSC at a short interval, the mean DAI was lower and the mean colon length was greater than those in the group that received T-MSC injection at a long interval, albeit not significantly so. IL-1β pretreatment had no significant effect on the DAI or colon length. Conclusions: In our study, intraperitoneal administration of T-MSC reduced gross inflammation of the colon, as evidenced by reduced shortening of the colon and a lower disease activity index in a murine colitis model. However, the histopathologic improvement of the T-MSC-treated group was not significant. T-MSCs may be a possible option for treatment for inflammatory bowel disease. Whether T-MSCs have therapeutic potential for chronic colitis warrants further evaluation in a larger-scale study.
