dc.description.abstractThe objective of this study is to classify related variables of disaster victims’ posttraumatic stress response systematically using an ecological model, and to analyze their effect sizes comprehensively. To this end, this study is a meta-analysis of 87 studies published in Korea and other countries between 2005 and 2015 on a topic of disaster victims’ posttraumatic stress response. First of all, variables associated with posttraumatic stress responses were derived and classified into sublevels of an ecological model: individualsystem, microsystem, and exosystem. Then, in order to investigate what predictors are more strongly related with the posttraumatic stress responses, and how the relationship between variables and the posttraumatic stress responses change according to characteristics of disaster and sociocultural characteristics surrounding the disaster, effect size analysis and meta regression analysis were performed using the meta-analytic method. The results are summarized as follows. First, among variables associated with posttraumatic responses derived based on the review of previous studies, variables associated with ‘demographic characteristics, exposure level, secondary stressors, individual physical and psychological function prior to disaster, experience of negative events prior to disaster, individual psychological characteristics’were classified as individual system, ‘variables associated with functions within family, stressors caused in family and social relationships after disaster, social support’were classified as microsystem, and ‘community capacity’ were classified as exo system. Second, according to the results of analysis based on the ecological model using all variables, variables present in the microsystem, which has a direct impact on individuals, such as family, friends, and relatives, related posttraumatic stress responses best, followed by individual system and exosystem in order of effect size. Then, the results on effect sizes of each factor group showed that posttraumatic stress responses were better related with variables caused by disaster than existing vulnerabilities prior to disaster such as ‘exposure level, variables associated with structure and functions within family, stressors caused in family and social relationships after disaster, secondary stressors’. Third, according to results on effect sizes by ecological system and factor group separately for different types of disaster, posttraumatic stress responses for natural disasters were best related with the order of micro, exo, then individual system, and posttraumatic stress responses for man-made disaster were better related with micro and individual systems than the exo system. The results of comparison of effect sizes between factor groups showed that effect size of ‘variables associated with structure and functions within family’ was greatest for natural disasters, whereas the effect size of ‘exposure level’ factor group was greatest for man-made disasters. In addition, in both natural and man-made disasters, ‘stressors caused in family and social relationships after disaster’ and ‘secondary stressor’ factor groups were major predictors of victims’ stress responses. Forth, according to results on how effect sizes of factor groups vary according to disaster characteristics such as size of damage and time lapse since disaster in each disaster type, in the relationship between factor groups and posttraumatic stress responses, moderating effect of size of disaster damage was not significant, whereas the moderating effect of time lapse since disaster was. Specifically, in natural disasters, the effect size of the ‘exposure level’, ‘experience of negative events prior to disaster’ factor group, and in man-made disasters, the ‘individual psychological characteristics’ factor group, were higher in the “chronicization period” (i.e., after 24 months since disaster) than immediately after disaster. In other words, was ‘exposure level’ and ‘experience of negative events prior to disaster’ was an important indicator of chronicization of stress responses in the case of natural disasters, and how ‘individual’s psychological characteristics’ were dysfunctional in the case of man-made disasters. Fifth, according to the results on how effect sizes of factor groups vary according to characteristics of macro systems such as national economic power, level of inequality, and social trust in each disaster type, the moderating effect of national economic power was not found to be significant, whereas moderating effects of level of inequality and social trust were found to be significant. Specifically, in natural disasters, variables such as ‘demographic characteristics’, ‘exposure level’, ‘secondary stressors’, and ‘community capacity’ related posttraumatic stress responses better in a society with high levels of inequality. In man-made disasters, the effect size of‘exposure level’ and ‘community capacity’ factor group was smaller in a society with greater levels of inequality. According to results on the moderating effect of personal trust in social trust, in natural disasters, intra-family structural and functional vulnerability maintained from prior to disaster and the stressor caused by disaster in family and social relationships related with posttraumatic stress responses better in a society with high personal trust. However, in man-made disasters, vulnerability associated with individual psychological characteristics related with posttraumatic stress responses better in the society with low personal trust. In addition, in natural disasters, the variables associated with structure and functions within family related with posttraumatic stress responses better, while in man-made disasters, secondary stressors and the individual psychological characteristics factor group related with posttraumatic stress responses better, in the society with lower level of trust in government. In other words, characteristics associated with quality of society such as personal and public trust rather than materialistic basis such as national economic power were found to change vulnerabilities of individuals and groups that can cause posttraumatic stress responses. By determining indicators to focus on in disaster psychological support process, at a clinical level, this study assisted with planning of interventions for disaster victims’ psychological recovery. At a policy level, this study provided basic materials for national-level disaster psychological support measures by considering variables retained by those who are vulnerable to mental health problems after disaster. This study is expected to contribute to establishing effective support measures and policies for assisting with recovery of disaster victims.;본 연구 목표는 재난피해자들의 외상 후 스트레스 반응의 관련 변인들을 생태이론을 사용하여 체계적으로 분류하고 그 효과크기를 종합적으로 분석하는 것이다. 이를 위해 2005년부터 2015년까지 국내외에서 발간된 학술지논문 중 재난피해자의 외상 후 스트레스 반응을 주제로 한 87개의 연구물을 메타분석 하였다. 우선, 외상 후 스트레스 반응과 관련한 요인들을 추출하여 생태이론에 기반한 모델(이하 생태모델)의 하위체계인 개인체계, 미시체계, 외체계로 분류하였다. 이후 외상 후 스트레스 반응과 보다 관련이 있는 관련변인들은 무엇인지, 그리고 재난의 특성, 재난이 발생한 사회문화적 조건에 따라 관련변인들과 외상 후 스트레스 반응 간 관계가 어떻게 변화하는지를 살펴보기 위해 메타분석방법을 활용하여 효과크기 비교분석과 메타회귀분석을 실시하였다. 그 결과를 요약하면 다음과 같다. 첫째, 선행연구 검토를 통해 추출된 외상 후 스트레스 반응 관련변인들 중 인구통계학적 요인, 노출수준, 이차 스트레스 원, 재난 전 기능수준, 재난 전 부정적 사건경험, 개인의 심리적 특성 변인군과 관련된 요인들은 개인체계로, 가족 내 기능, 가족 및 사회적 관계 관련 스트레스 원, 사회적 지지 변인군과 관련된 요인들은 미시체계로, 지역사회의 낮은 회복력 변인군에 해당되는 요인들은 외체계로 분류하였다. 둘째, 전체 관련변인들을 대상으로 생태이론에 따른 분석 결과, 가족이나 친구, 친인척 등과 같이 개인에게 직접적인 영향을 주는 환경인 미시체계에 존재하는 관련변인들이 외상 후 스트레스 반응을 가장 잘 예측하였으며, 그 다음으로 개인체계, 외체계 순으로 효과크기가 컸다. 이후 변인군에 따른 효과크기를 분석한 결과, ‘노출수준’, ‘가족 및 사회적 관계관련 스트레스 원’, ‘이차 스트레스 원’처럼 재난 전부터 지속되었던 취약성보다는 재난으로 인해 야기된 요인들이 재난피해자들의 외상 후 스트레스 반응을 보다 잘 예측하고 있음을 확인할 수 있었다. 셋째, 재난 유형을 구분하여 셍태이론의 체계별, 변인군별 효과크기를 분석한 결과, 자연재해에서는 미시체계, 외체계, 개인체계 순으로 외상 후 스트레스 반응을 예측하고 있었으며, 인재에서는 미시체계와 개인체계의 예측력이 가장 높았다. 또한 변인군별로 효과크기를 비교한 결과, 자연재해에서는‘가족 내 특성’이 인재에서는 ‘노출수준’변인군의 효과크기가 가장 컸다. 또한 자연재해와 인재 모두에서‘가족이나 가까운 관계로부터 발생하는 스트레스 원’, ‘이차 스트레스 원’변인군들이 피해자들의 외상 후 스트레스 반응에 대한 중요한 관련변인이었다. 넷째, 재난 유형을 구분하여 재난 피해규모, 재난 후 경과기간과 같은 재난 특성에 따라 관련변인들의 효과크기가 변화하는지를 검토한 결과, 관련변인들과 외상 후 스트레스 반응 간 관계에 있어서 재난 피해 규모의 조절효과는 유의하지 않았으며, 재난 후 경과기간의 조절효과는 유의한 것으로 확인되었다. 구체적으로 자연재해에 있어서 ‘노출수준’, ‘재난 전 부정적 사건경험’변인군의 효과크기가, 인재에서는‘개인의 심리적 특성’변인군의 효과크기가 재난 발생 직후보다 재난 발생 후 24개월이 지난 만성화기를 경과한 이후에 보다 컸다. 즉, 재난피해자의 외상 후 스트레스 반응이 만성화되는데 있어서 중요한 지표는 자연재해의 경우 ‘노출수준’과 ‘재난 전 부정적 사건경험’여부였으며, 인재의 경우에는 ‘개인의 심리 특성’이 얼마나 역기능적인가 하는 것이었다. 다섯째, 재난 유형을 구분하여 국가 경제력, 불평등 수준, 사회적 신뢰와 같은 거시체계 특성에 따라 관련변인들의 효과크기가 변화하는지를 검토한 결과, 국가 경제력의 조절효과는 유의하지 않은 반면, 불평등 수준과 사회적 신뢰의 조절효과는 유의한 것으로 확인되었다. 구체적으로, 자연재해의 경우 불평등 수준이 높은 사회에서 인구통계학적 요인, 노출수준, 이차 스트레스 원, 지역사회의 낮은 회복력과 같은 요인들이 외상 후 스트레스 반응을 보다 잘 예측하고 있었다. 인재의 경우에는 불평등 수준이 큰 사회일수록 노출수준, 지역사회의 낮은 역량 변인군의 효과크기가 보다 더 작았다. 사회적 신뢰 중 사적신뢰의 조절효과검증 결과, 자연재해의 경우, 사적신뢰가 높은 사회에서는 재난 전부터 지속되어 온 가족 내 구조적, 기능적 취약성, 가족 및 사회적 관계에서 재난으로 인해 야기되는 스트레스 원들이 외상 후 스트레스 반응을 보다 잘 예측하였다. 반면 인재의 경우, 사적신뢰가 낮은 사회에서는 개인의 심리적 특성과 관련된 취약성이 외상 후 스트레스 반응을 보다 잘 예측하고 있었다. 또한 정부에 대한 신뢰가 낮은 사회일수록 자연재해에서는 가족 내 특성 변인군이, 인재의 경우에는 이차 스트레스 원, 개인의 심리적 특성 변인군이 외상 후 스트레스 반응을 보다 잘 예측하였다. 즉, 국가 경제력과 같은 물적 기반보다 사적 및 공적 신뢰와 같은 사회의 질과 관련된 특성이 재난 후 스트레스 반응들을 야기할 수 있는 개인이나 집단이 가진 취약성을 변화시키고 있었다. 본 연구는 재난 심리지원 과정에서 보다 주목해야 할 지표들을 밝힘으로서 임상적으로는 재난피해자들의 심리적 회복을 돕는 개입을 계획하도록 돕고, 정책적으로는 재난 후 정신건강 문제에 보다 취약한 이들이 가진 요소들을 고려하여 국가 차원에서 재난심리지원 방안을 마련하기 위한 기초자료를 마련할 수 있었다. 이는 재난피해자들의 회복을 돕는 효과적인 지원 방안 및 정책을 세우는 데 기여할 것으로 보인다.-
