Abstract
본 연구의 목적은 조현병 진단을 받은 환자의 발테그 그림검사(Wartegg-Zeichentest: WZT) 결과물을 토대로 조현병 환자가 외부자극에 집중하는 정도와 이를 수용하고, 인지 사고하는 과정의 특성을 이해하는데 도움을 주는 하나의 시각화 자료로 유의미한지 살펴보는데 있다. 연구대상은 서울에 소재한 정신건강의학과 전문 병원인 E병원의 폐쇄병동에 입원중인 조현병 환자와 낮병원에서 통원치료중인 조현병 환자를 대상으로 하였다. 검사 기간은 2015년 6월부터 2015년 9월까지 4개월간 실시되었으며 총 80명의 그림을 수집하였고 그 중 자신의 그림이 연구 자료로 사용되는 것에 동의한 환자는 34명이었다. 연구결과 조현병 환자가 WZT에 제시된 8개의 자극도식에 집중하는 정도에 따라‘충분한 유념’, ‘산만한 유념’, ‘고착된 유념’, ‘전면적 경시’로 분류할 수 있었다. 이를 통해 8개 자극도식에 대한 집중정도에 따른 차이와 특성을 관찰할 수 있었으며 이를 토대로 조현병 환자의 외부자극에 대한 집중정도와 그에 대한 반응양상을 추측해볼 수 있었다. 다음으로 WZT에 제시된 각 자극도식의 수용과 배제에 따라 ‘단순한 수용 및 배제’, ‘포괄적 수용 및 배제’, ‘그림과 자극도식 간 의미 연계성이 의심스러운 경우’, ‘완전한 배제’로 구분하고 더 나아가 완전한 배제를 ‘자극도식에 대한 독립적 해석’, ‘상징적 이해’, ‘자극도식에 대한 거부’로 분류하여 살펴보았다. 그리하여 조현병 환자가 자극도식을 인지하고 수용 또는 배제하는 과정을 통해 조현병 환자의 인지, 사고하는 과정의 특성을 유추해볼 수 있었다. 더 나아가 이는 자극도식에 대한 공감 부족으로 이해할 수 있고 이는 넓은 의미에서 조현병 환자의 외부현실에 존재하는 자극에 공감하는 능력의 결여와 이를 수용한 사고와 사고 흐름의 단절을 추측해볼 수 있는 하나의 자료로 이해할 수 있었다. 또한 그림 제목부여에 있어 혼란스러운 단어들의 조합으로 이루어진 표현의 중의성이 나타나는 경우 사고장애를 추측해 볼 수 있었다. 그리고 WZT 결과물에 대한 조현병 환자의 그림에 대한 부정확하고 불분명한 내용진술은 넓은 의미에서 인지와 사고기능에 대한 역기능을 암시함을 확보할 수 있었다. 따라서 본 연구는 WZT의 8개 자극도식에 집중하는 정도와 이를 수용하고 배제하는 양상을 토대로 조현병 환자가 외부자극에 반응하고 이를 수용하여 인지하고 사고하는 과정을 이해하는데 도움을 주는 하나의 보조적 자료로서 의의가 있다고 할 수 있다.;The purpose of the study was to examine if the Wartegg-Zeichen test (WZT) can be considered as one of the visualized materials that can be used to understand the degree of concentration of patients with schizophrenia on external stimulus and to determine the characteristics of their cognition and thinking processes involved in recognition and acceptance of the external stimulus based on results. The study subjects were patients with schizophrenia who were hospitalized at the closed ward at the E Hospital that specializes in psychiatry and those who were getting outpatient treatment during the daytime. The test period was 4 months from June 2015 to September 2015. A total of 80 pictures were collected, but only 34 patients agreed on the use of their test results for the study. The study results revealed that patients with schizophrenia could be categorized into ‘full attention,’ ‘diffuse attention,’ ‘fixated attention’ and ‘full neglect’, depending on their degree of concentration on the 8 simple signs. Based on this, we were able to speculate the differences and characteristics of the degree of concentration of patients with schizophrenia on 8 simple signs. We were also able to suppose the response pattern and concentration degree of patients with schizophrenia on external stimulus. Furthermore, simple signs acceptance and exclusion were categorized into ‘simple acceptance and exclusion,’ ‘comprehensive acceptance and exclusion,’ ‘suspicious connectivity between the picture and simple signs,’ and ‘complete exclusion(independent analysis on simple signs, symbolic understanding, and rejection of simple signs)’ according to the degree. Thus, we were able to infer the characteristics of the cognition and thinking processes of patients with schizophrenia. In a broad sense, Their lack of sympathy and exclusion revealed in simple signs could be related to their lack of sympathy and block of thought from external stimuli. Moreover, the ambiguous expressions shown in confusing combination of words in creating titles for the pictures suggested thinking disorders as well as unclear and uncertain statements of content that suggested fading thinking dysfunction. Therefore, the study is significant in providing supporting data for understanding the characteristics of the cognition and thinking processes of patients with schizophrenia and how much they focus on and accept the external reality, based on their response to simple signs.
