초기 알츠하이머 환자의 노래와 자서전적 기억

Abstract

본 연구에서는 초기 알츠하이머 치매환자를 대상으로 환자 본인이 선곡한 노래 조건과 치료사가 선곡한 노래 조건에서 자서전적 기억 회상의 내용과 회상하는데 걸린 시간과 기분변화를 측정하여 자서전적 기억회상에 효과적인 노래조건을 알아보고자 한다. 연구 대상은 서울시 S구 소재의 치매지원센터, 데이케어센터를 이용하는 평균 78세, 교육정도가 평균 8.5년인 초기 알츠하이머 치매환자 19명으로 구성되었다. 이들은 한국형 간이 정신 상태 검사(MMSE-K) 평균 20.4점, 노인 우울척도(GDS) 점수가 평균 4.2점으로 우울감이 없는 초기 알츠하이머 환자이다. 19명의 초기 알츠하이머 치매환자에게 실험 전에 기분 자가 평가도구로 기분을 평가하고 일반적인 피험자 정보를 작성하도록 했다. 실험은 각각 자신이 선곡한 친숙한 노래와 치료사가 제공한 노래를 들으며 자신이 직접 경험한 기억이 떠오르면 손을 들게 하여 시간을 측정했다. 노래가 끝나면 기억을 이야기하도록 했으며, 이야기 내용은 녹음 전사하여 연구자가 TEMPau척도로 구체성을 평가했다. 실험 후에 피험자에게 기억 평가지를 제공하여 작성하도록 하였다. 본 연구 결과 피험자 본인이 선곡한 노래 조건이 치료사가 선곡한 노래 조건보다 친숙도와 선호도가 높았으며, 회상한 기억과 노래의 밀접함의 정도가 높았다. 또한 구체적으로 기억을 회상할 수 있었으며, 회상하는데 걸린 시간도 짧았다. 또한 실험 전후의 기분변화 폭이 컸으며, 회상한 기억과 기분의 관련성에도 영향을 미쳤다. 이는 초기 알츠하이머 환자 본인이 선곡한 노래가 자서전적 기억을 회상하는 단서가 되어 구체적이고, 긍정적인 기억을 촉진하며 회상시간이 단축됨을 보여주어 임상에서 초기 알츠하이머 환자의 자서전적 기억을 회상할 때, 본인이 선곡한 친숙한 노래가 효과적으로 사용될 수 있음을 보여줄 수 있음을 시사한다.;The purpose of this study is to examines effective song for recalling autobiographical memory between the early AD patients chosen and music therapist chosen based on examining recalling memory time and mood changes. The subjects consisted of 19 early AD patients who using Dementia Care, Day Care Center in Seoul with the average 78-year-old, and an average 8.5-year education. These people averaged 20.4points in MMSE-K, and 4.2points in GDS and had no depression. Before the test, 19 patients with early AD patients evaluated their mood with self-test tools and wrote their general information. Experiment was measuring the time until they remember their own experience when they heard familiar song they chosen and song therapist chosen. After listening song, they told about their memory and wrote memory score paper. It was evaluated for materiality based on TEMPau. A major finding was that the condition with patient chosen song had higher familarity and preference and subjective relation with memory than song therapist chosen condition. Also, person who patient chosen song condition can remember detailed memory in shorter time. Furthermore it had higher changes of mood and affected relevance between remembered memory and mood. This study shows that early AD patients chosen song could be clue for remembering autobiographical memory. It also promoted positive memory and reduced recalling time. It can be suggested that their own chosen familiar song could be used effectively when early AD patients recall their autobiographical memory.
