암환자의 정맥 혈전 색전증 치료에서 rivaroxaban과 dalteparin의 출혈 부작용 비교와 위험 요인에 대한 연구

Abstract

암환자에게 정맥 혈전 색전증(Venous thromboembolism, VTE)은 빈번하게 발생하는 합병증이다. VTE의 치료제로는 저 분자량 헤파린(Low molecular weight heparin, LMWH)이 추천되며, 본원에서는 주로 dalteparin이 사용된다. 또한 최근 새로운 경구용 항 응고제인 rivaroxaban도 VTE 치료약으로 사용되고 있다. 본 연구는 실제로 암환자의 VTE 치료를 위해 rivaroxaban을 사용했을 때 출혈 부작용 발생 정도를 dalteparin을 사용한 환자 군과 비교하고, 출혈과 관련된 위험 요인을 분석하여 암환자에서의 항 응고제 사용 안전에 기여하는데 목표가 있다. 서울 A 병원 종양내과에서 2012년 7월부터 2014년 6월까지 2년동안 rivaroxaban 또는 dalteparin을 처방 받은 암 환자의 전자의무기록을 후향적으로 검토하였다. 약물의 종류에 따라 출혈 부작용 발생 여부를 비교하고, 두 군의 특성과 출혈 경향을 분석하였다. 총 399명의 환자가 연구에 포함되었으며, 246명은 rivaroxaban을 사용하였고, 153명은 dalteparin을 사용하였다. 출혈 부작용은 rivaroxaban군에서 통계적으로 유의하게 더 많이 발생하였다 (85건[34.6%], vs. 28건, dalteparin군 [18.3%]; OR 2.093; 95% CI 1.219-3.595; P=0.007). 그 중에서도 주요 출혈과 임상적으로 연관된 비 주요 출혈 부작용은 41건(16.7%) 이었으며, 주로 위장관 출혈과 비뇨기계 출혈 증상을 호소하였다. 시간에 따른 출혈 부작용 발생률을 비교하였을 때 rivaroxaban 군에서 dalteparin군에 비해 초기에 출혈 부작용이 더 많이 발생하였다(P=0.007). Rivaroxaban군에서 출혈에 영향을 주는 인자를 분석하였으나 통계적으로 의미 있는 변수는 도출되지 않았다. 결론적으로 암환자의 VTE치료에 rivaroxaban을 사용했을 때 dalteparin보다 출혈 부작용 발생 위험이 높고, 특히 이러한 부작용은 투여 초기에 발생하기 때문에 이 시기에 주의가 필요할 것으로 생각된다.;Venous thromembolism (VTE) is a common and life-threating condition in cancer patients. Low-molecular-weight heparins, such as dalteparin, are recommended in the treatment of VTE. Also, rivaroxaban, an orally administered direct factor Xa inhibitor, is approved for the treatment of VTE. It showed similar efficacy to standard therapy and was associated with a significantly lower rate of major bleeding. However, in the real world, bleeding occurred as often in cancer patient receiving rivaroxaban. The goal of this research is an analysis of bleeding risk between rivaroxaban and dalteparin for the treatment of VTE in cancer patients. Medical records of oncology patients who were treated with rivaroxaban or dalteparin for VTE from July 2012 to June 2014 were retrospectively reviewed. A total of 399 patients were included in the study. Of these patients, 246 were using rivaroxaban and 153 were using dalteparin. The rate of bleeding was significantly higher in the rivaroxaban group than in the dalteparin group (85 patients [34.6%], vs. 28 patients [18.3%]; OR 2.093; 95% CI 1.219-3.595; P=0.007). An episode of major bleeding or clinically relevant non major bleeding occurred in 41 patients in the rivaroxaban group and gastrointestinal bleeding was the most common symptom. In addition, bleeding occurred in the early stage of rivaroxaban treatment. There were no differences in characteristics between non-bleeding and bleeding patients in the rivaroxaban group. In conclusion, our data suggested that rivaroxaban was associated with an increased risk of bleeding in cancer patients.
