Abstract
본 연구는 크론병 환자의 삶의 질과 관련된 요인들을 탐색하고 크론병 환자들의 삶의 질을 예측하는 모형을 구축함으로써 향후 크론병 환자의 삶의 질 향상을 위한 간호 중재 개발하기 위한 근거자료를 제공하기 위하여 시도되었다. 연구 설계는 모형 검증연구이며, 연구 대상자는 2014년 5월 2일부터 2014년 10월 31일까지 서울 일개 상급종합병원의 소화기내과 외래에 내원한 크론병 환자 273명이었다. 본 연구의 가설적 모형에서 제시된 잠재변수 6개 중 외생변수는 질병중증도, 사회적 지지 2개였고, 내생변수는, 우울, 스트레스, 피로, 삶의 질 4개였다. 이러한 잠재변수를 측정하는 관측변수는 8개였으며, 가설적 모형에서 설정된 경로는 총 14개였다. 본 연구에서 수집된 자료는 SPSS 18.0 프로그램과 AMOS 18.0 프로그램을 이용하여 분석하였고, 연구 대상자의 일반적 특성은 실수, 백분율, 평균, 표준편차를 구하였고, 제 변수들의 정도는 평균과 표준편차, 제 변수들의 평균차이검정은 Mann-Whitney U-test, Kruskal-Wallis test로 분석하였다. 제 변수들의 상관관계와 다중공선성은 Pearson’s correlation coefficient을 이용하였으며, 구조방정식 모형분석을 위한 잠재변수의 타당도는 확인적 요인 분석을 실시하였다. 모형의 적합도는 구조방정식 모형의 비모수적 방법인 점근적 분포 무관Asymptotically distribution free, ADF) 추정법을 이용하여 χ2 통계량, RMSEA, SRMR, GFI, AGFI, NFI, CFI와 같은 전반적인 적합지수로 확인하였다. 연구 결과 최종 모형의 적합도 지수는 χ2 통계량=9.368 (p=.671), RMSEA=0.000, SRMR=0.033, GFI=0.990, AGFI=0.971, NFI=0.963, CFI=1.000 으로 모든 적합도 지수가 권장수준을 만족하였다. 본 구조모형에서 크론병 환자의 삶의 질은 질병중증도, 사회적지지, 우울, 스트레스, 피로에 의하여 67.9% 설명되었으며, 가설적 모형에서 설정한 14개의 각 요인이 삶의 질에 영향을 미치는 경로 중 13개가 통계적으로 유의하였다. 본 연구 결과를 통하여 크론병 환자의 삶의 질은 사회적 지지가 높고, 질병중증도, 우울, 스트레스는 낮을수록 높은 것으로 나타났다. 특히 본 연구에서 질병중증도가 크론병 환자의 삶의 질에 가장 유의한 영향 요인으로 밝혀졌고, 질병중증도는 우울, 스트레스, 피로에도 영향을 준다는 것이 모형을 통해 확인되었으므로 크론병 환자의 삶의 질을 향상시킬 수 있는 간호중재 개발을 위한 기초자료 제공에 기여할 것이다.;The purpose of the study is to investigate the factors that are associated with the quality of life in Crohn’s disease patients. A model is constructed in order to predict the factors that affect the quality of life. If the factors are known, then evidence-based nursing intervention can be developed and improve the lives of Crohn’s disease patients. A survey was given to 273 Crohn.s disease patients during their outpatient visits in the department of gastrointestinal internal medicine. The time frame was from May 2nd, 2014 to October 31st, 2014 at a university hospital in Seoul, South Korea. There are six potential variables chosen for the hypothetical model of this research. The six variables are heterogenous variables. One is disease severity and two are for social support. There are four endogenous variables of depression, stress, fatigue, and quality of life. There are eight potential variables that are observation, so the hypothetical model that was constructed has a total of 14 variables. SPSS 18.0 and AMOS 18.0 programs were used to analyze the data that was collected in this research. The relevancy of the research model χ2 statistics, RMSEA, SRMR, GFI, AGFI, NFI, CFI and overall similar congruences were confirmed. The results of the hypothetical model, for the index congruence χ2 statistics = 7.262 (p = 0.610), were RMSEA = 0.000, SRMR = 0.031, GFI = 0.993, AGFI = 0.970, NFI = 0.971, CFI = 1.000. The congruent index level shows that the suggested level of satisfaction was reached. According to the structure of the model of this research, the quality of life of Crohn’s disease patients, social support, depression, stress, and fatigue were 67.9%. But in the hypothetical model with the 14 factors set up, only 13 factors influenced the Crohn’s disease patients. The results of this research showed that the social support of Crohn’s patients was high in terms of quality of, while if disease severity, depression, and stress were low, the quality of life was high. Disease severity was especially a factor that influenced the quality of life in Crohn’s disease patients. The influence of disease severity was confirmed in this research, and it also influenced the other variables of depression, stress, and social support. Therefore, since disease severity greatly influences the quality of life in Crohn’s disease patients, nursing intervention, to treat this factor, should be developed to improve the lives of Crohn’s disease patients.
