상호적 악기 연주 프로그램이 그룹홈 거주 자폐범주성장애 성인의 사회적 상호작용 기능 향상에 미치는 효과

Abstract

본 연구는 그룹홈에 거주하는 ASD 성인들을 대상으로 상호적 악기연주 프로그램을 이용한 그룹 음악 중재를 실시함으로써 음악적 자극이 사회적 상호작용 기능 향상에 미치는 효과를 살펴보고자 하였다. 연구대상자는 발달장애 주간보호센터 K기관의 공동생활가정(그룹홈)에 거주하는 20대 후반의 ASD 1급 성인 남성 3명이고, 본 연구는 단일대상연구의 한 방법인 대상자간 중다기초선설계를 사용하여 진행되었다. 3개월 동안 주 3회 그룹 세션으로 진행되었고, 기초선(6~16회기), 중재(6~16회기), 유지 구간(3회기)을 포함하여 총 25회기 세션이 실시되었다. 사회적 상호작용 시도 및 반응 행동 시점을 알려주는 음악구조와 큐에 의해 구조화된 악기 연주에 참여함으로써 대상자의 목표행동은 음악적 환경 안에서 강화를 받게 된다. 상호작용 시도 행동과 반응 행동은 매회기 촬영된 동영상 자료 일부를 관찰하여 측정하였다. 연구 결과 상호적 악기 연주 프로그램을 이용한 그룹 음악치료 중재 이후 모든 대상자의 상호작용 시도 행동, 반응 행동 발생률이 증가하는 추세를 보였다. 이는 상호적 악기 연주가 ASD 성인의 자발적 상호작용 행동을 유도하는데 효과적임을 보여주고 있다. 음악적으로 전달된 사회적 정보는 사회적 맥락 안에서 타그룹원의 의도를 이해하고 사회적 상호작용 활동에 참여하는 것을 용이하게 한다. ASD 성인의 사회 기능 향상뿐만 아니라 삶의 질 향상을 위하여, 이와 같은 형태의 음악 기반 프로그램이 후속 연구를 통해 지속적으로 확장되어 적용될 필요가 있다.;The purpose of this study was to examine the effect of interactive instrumental playing on social interactive behaviors in adults with autism spectrum disorder (ASD). A single subject design with multiple baseline across participants was applied and three adults with severe ASD in their late 20's living at group home participated in this study. Participants received group sessions three times a week and a total of 25 sessions including the baseline, music intervention and the follow-up phases were administered during 3 months. During intervention, participants engaged in structured instrumental playing in which timing to initiate or respond to social interactive behaviors was musically cued and target behaviors were reinforced in musical environment. Behaviors to initiate to interact with other members in the group or respond to others were recorded and analyzed. The results of this study showed that the frequency of both initiation of and response to social interaction increased in all participants during intervention phase, compared to baseline phase. This result demonstrates the effects of interactive instrument playing in eliciting social behaviors of adults with ASD. It also indicates that musically delivered social information would facilitate adults with ASD to understand the intention of social partner in a social context and motivate this population to engage in social interaction. Further studies are also suggested to expand the applicability of this music-based program for adults with ASD in needs of not only continuous intervention for social behaviors, but also the quality of life.
